Upon arrival I was greeted by Mrs. Reed, the Director/Owner. You assisted me with a general walk-through of the center. At the time of the visit, there were a total of twenty-one (21) children present, ranging in age from one (1) to four (4) years old. The children were eating lunch, preparing for rest time, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a Five (5) star license issued January 12, 2018, earning six (6) points in education, seven (7) points in program standards and one (1) quality point. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. All staff files were monitored. 10% of enrolled children’s files were reviewed. All spaces were monitored including the outside learning environment for supervision, staff/child ratios, materials, equipment and required postings. The program records were unable to be reviewed for current inspections. The owner/director was unable to locate the inspections and emergency drills during the visit. I requested to receive the inspections and drills by the close of business in order to remain in compliance. I received the fire drill, emergency drill, fire inspection, playground inspection, CPR and First Aid for five staff members at 6:00pm. INSPECTIONS *The last fire drill was conducted on 5/19/26. *The last emergency drill was conducted 4/16/26. *The last playground inspection was completed 5/12/26. *The last sanitation was completed on 10/28/26 with a “Superior ‘’ classification. *The last fire inspection was completed on 9/23/25. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). It was observed that a staff member criminal background check expired on 2/22/26. G.S. 110-90.2(b) & .2703(n)&(o) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. Eight staff members files were observed to have medical and personnel information maintained in the same file. .0701(d) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before June 10, 2026, the letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. During today’s visit I was unable to monitor monthly drills, inspections, and records for CPR and First Aid. I discussed with Mrs. Reed having staff members to assist in getting staff files and center records organized and filed. The Criminal Background check for one staff member was observed with an expiration date of 2/22/2026. The Director stated that the criminal background check was rejected because of an additional name not placed on the application. The Director stated that the criminal background check is now in process. All staff files, personnel information and medical were filed together. This is a repeat violation from the routine unannounced visit that was conducted on 1/29/26. It was suggested during the routine unannounced visit using another file folder or binder to file all medical information. Please be reminded that all staff member’s medical report, proof of tuberculosis test or screening, and annual health questionnaire must be maintained separately from the individual personnel file. I would recommend using another file or binder to file all medical information. I would also suggest having a staff member assist you in separating and organizing staff files. I also provided the staff checklist form to assist in organizing the staff files. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. HEALTH AND SAFETY TRAINING Health and Safety training courses are now available through Moodle on the DCDEE website. This training courses are provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety training must be renewed at least every five (5) years. If you complete the trainings in 2021, you should begin to retake the training now. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. The following staff member Criminal Background Check will expire: - J. Mack: 9/21/26 - T. MONITORING During today’s visit, a partial assessment of Child Care Requirements was conducted for all classrooms. The monitoring included CPR/First Aid, criminal background checks and special training requirements of newly hired staff. I monitored two new children’s files. The monitoring also included License and permit restrictions, staff/child ratios, supervision, storage of hazardous items, medication, materials/equipment, and all space occupied by the children. The outdoor play environment was not monitored due to the playground being icy during the visit. Mrs. Reed stated to me that the office water pipe burst and she had to pack all files in boxes and bins to avoid them from getting wet. She discussed with me that she will be going to bright wheel to have a back up plan to file all files. I discussed with Mrs. Reed to make sure she has a plan in case the iPads malfunction such as using file cabinets to file the paper copies. INSPECTIONS *The last fire drill was conducted on 5/22/25. *The last emergency drill was conducted 3/25/25. *The last playground inspection was completed 12/3/25. *The last sanitation was completed on 10/28/25 with a “Superior ‘’ classification. *The last fire inspection was completed on 9/6/24. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. SAFE SLEEP DOCUMENTATION During today’s visit a child under 12 months did not have safe sleep documentation for January 21, 22, and 23rd. The child was in attendance each of those days. To ensure the safety of all children it is important to visually check sleeping infants every 15 minutes and document. I would recommend that you retrain all staff working in the infant room the policies and procedures of safe sleep and ITS-SIDs. Please view the website for Caring For Our Children regarding Chapter 3: Health Promotion and Protection pertaining to infant safe sleep. CHILDREN FILES A child enrolled on 12/4/25 did not have a medical assessment on file. I would suggest collecting all needed paperwork on the date of enrollment. I also discussed the children’s file checklist. I provided a checklist to the administrator. I would also suggest doing a monthly audit of staff and children files to make sure all needed forms have been collected and are filed. ANNUAL FIRE INSPECTION The fire inspection on file observed was dated 9/6/2024. I would suggest using your outlook calendar or a reminder to alert you when the fire inspection is due. MONTHLY DRILLS The fire drill and emergency drills are not completed monthly. I would suggest using your outlook calendar or a reminder to make sure they are completed monthly. I would recommend posting in your office or in an area you will see monthly. STAFF FILES One staff member, personnel information and medical were filed together. Upon arrival I was greeted by Chelsea Davis, a teacher. I spoke with Mrs. Reed, Director/Owner, over the phone. Mrs. Reed, the Director/Owner, arrived within twenty (20) minutes of the visit. I completed a general walk-through of the center. At the time of the visit, there were a total of twenty-six (26) children present, ranging in age from infant to five (5) years old. The children were working on playing with materials, interacting with the teachers, transitioning outside, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a Five (5) star license issued January 12, 2018, earning six (6) points in education, seven (7) points in program standards and one (1) quality point. STAFF CHILD RATIO/GROUPING I observed one (1) one year old child being cared for with two children three years of age. The teacher stated that they have two one (1) year olds, six two (2) year olds, and two three (3) year olds enrolled in space # 3. I would recommend considering transitioning either one-year olds or the three-year-olds into another class. The Director discussed with me that the two one-year olds will be turning two in March. I discussed with the Director a few options that can be considered until the two one-year olds turn two. According to “Caring For Our Children- National Health and Safety Performance Standards Guidelines for Early Care and Education Programs”, studies have found that children (particularly infants and toddlers) in groups that comply with the recommended ratio receive more sensitive and appropriate caregiving and score higher on developmental assessments, particularly vocabulary. In addition, the children’s physical safety and sanitation routines require a staff that is not fragmented by excessive demands. Both CFOC and the NC Child Care Requirements state that infants and toddlers should be cared for in groups that separate them from older children. Upon arrival I was greeted by Chelsea Davis, a teacher. Mrs. Reed, the Director/Owner, arrived within forty (40) minutes of the visit. I completed a general walk-through of the center. At the time of the visit, there were a total of twenty-one (21) children present, ranging in age from infant to five (5) years old. The children were working on playing with materials, interacting with the teachers, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a Five (5) star license issued January 12, 2018, earning six (6) points in education, seven (7) points in program standards and one (1) quality point. MONITORING During today’s visit, a partial assessment of Child Care Requirements was conducted for all classrooms. The monitoring included CPR/First Aid, criminal background checks and special training requirements of newly hired staff. The monitoring also included License and permit restrictions, staff/child ratios, supervision, storage of hazardous items, medication, materials/equipment, and all space occupied by the children. The outdoor play environment was not monitored due to rain during the time of the visit. INSPECTIONS *The last fire drill was conducted on 1/21/25. *The last emergency drill was conducted 1/21/25. *The last playground inspection was completed 1/7/25. *The last sanitation was completed on 12/9/24 with a “Superior ‘’ classification. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 318 Children between 12 and 24 months of age were grouped with children 3 years of age or older. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. STAFF CHILD RATIO/GROUPING I observed one (1) one year old being cared with three years of age. The teacher stated that they have two one (1) year olds, six two (2) year olds, and two three (3) year olds enrolled in space # 3. I would recommend considering transitioning either one year olds or the three year olds into another class. According to “Caring For Our Children- National Health and Safety Performance Standards Guidelines for Early Care and Education Programs”, studies have found that children (particularly infants and toddlers) in groups that comply with the recommended ratio receive more sensitive and appropriate caregiving and score higher on developmental assessments, particularly vocabulary. In addition, the children’s physical safety and sanitation routines require a staff that is not fragmented by excessive demands. Both CFOC and the NC Child Care Requirements state that infants and toddlers should be cared for in groups that separate them from older children. SAFE SLEEP Policy During today’s visit I observed a blanket in the crib with a sleeping infant. An infant was placed in the crib with a hoodie covering the infant’s head. I would recommend having all staff that work in the infant’s classroom review the safe sleep policy. I would also recommend viewing the website https://nrckids.org/CFOC (Caring For Our Children) in Chapter 3: Health Promotion and Protection in regards to infant safe sleep. PRESCRIPTION MEDICATION In space #1 an Albuterol medication was not stored in its original package. I discussed with the Administrator the importance of storing all medications in its original package. I would recommend sending a memo to parents and staff that all medications should be stored in their original package. STORAGE OF HAZARDOUS MATERIALS During today’s visit I observed in the children’s bathroom in an unlocked cabinet “Juicy Tropical Odor Air Freshener,” that stated, “Keep out of reach of children.” I would recommend storing the air freshener in the locked closet located in your hallway. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. HEALTH AND SAFETY TRAINING Health and Safety training courses are now available through Moodle on the DCDEE website. This training courses are provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety training must be renewed at least every five (5) years. If you complete the trainings in 2020, you should begin to retake the training now. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. Upon my arrival, I was greeted and welcomed into your program. Five (5) classrooms were opened today. I conducted a general walk-through of the indoor and outdoor learning environments. I observed the children having circle time, eating lunch, and resting during nap time. RATED LICENSE STATUS The program holds a 5-star-rated license issued on 1/12/18. The program is in the "held harmless" status, and points from the previous assessment are carried forward. The program received 7 points in Program Standards, 6 points in Staff Education, and 1 quality point through the programmatic options of having a staff benefits package and an infrastructure of parent involvement. INSPECTIONS The last sanitation inspection was conducted on 1/9/24. The last fire inspection was conducted on 9/26/23. The last fire drill was conducted on 12/18/23. The last emergency drill was conducted on 12/18/23. The last playground inspection was conducted on 1/8/24. During this visit, a partial assessment of the child care requirements was conducted. The following items were monitored and found in compliance during this visit: nurturing interactions, daily attendance, license posted & permit restrictions followed, safe indoor environment, fire drills & inspections, CBC qualifications, CPR/First Aid certification, and storage of medications & hazardous materials. The facility reassessment year will not begin until July 1, 2024. During this time, the provider will decide whether they wish to have the Environment Rating Scale Assessment conducted. We discussed making sure the provider WORKS account reflects her current degree. We discussed reviewing the NCRLAP website ERS resources (www.NCRLAP.org) for updated information. Ms. Yolanda is aware that she can request an ERS assessment free of charge during the preparation, and depending on the scores, they can be used as guidance to enhance program quality. I conducted a general walk-through of the indoor and outdoor learning environments. I observed the children resting during nap time. RATED LICENSE STATUS The program holds a 5-star-rated license issued on 1/12/18. The program is in the "held harmless" status, and points from the previous assessment are carried forward. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. A current fire inspection was not available for review during today's visit. I would suggest that you always have your inspection posted in your office just in case you are absent and someone else needs to perform that drill. I conducted a general walk-through of the indoor and outdoor learning environments. I observed the children resting during nap time. RATED LICENSE STATUS The program holds a 5-star-rated license issued on 1/12/18. The program is in the "held harmless" status, and points from the previous assessment are carried forward. The program received 7 points in Program Standards, 6 points in Staff Education, and 1 quality point through the programmatic options of having a staff benefits package and an infrastructure of parent involvement. INSPECTIONS The last sanitation inspection was conducted on 2/13/23. The last fire inspection was conducted on 4/5/22. The last fire drill was conducted on 2/9/22. The last emergency drill was conducted on 2/9/23. The last playground inspection was conducted on 2/7/23. All applicable child care requirements were monitored during today's visit. I did not assess transportation requirements because the program does not transport children. The following violations were observed and recorded during today’s visit. The number of children present during today's visit exceed the capacity according to the facility license. There were 32 children present and the permit capacity only allows 30 children. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. A current fire inspection was not available for review during today's visit. The last documented fire inspection noted by the consultant was 4/5/22. 10A NCAC 09 .0304(a) 601 Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. The thermometer in the kitchen refrigerator was reading 54 degree F. The owner/director replaced the thermometer and the consultant rechecked it after finishing the walk-through.
