Child care licensing requirements were monitored. The last Annual Compliance visit was conducted on 11/4/25. This center operates with a five-star license, which was issued 6/2/2019. COMPLIANCE HISTORY: The compliance history for this center for the previous eighteen-month timeframe was 93%. During today’s visit all indoor and outdoor licensed space was monitored. OBSERVATIONS: During today’s visit I observed the children enjoying the outdoor environment and eating a morning snack of waffles and blueberries. I visited each classroom. Current activity plans were posted in each classroom. The required information was posted on the Parent Information board. CHILDREN’S RECORDS: Children’s records were reviewed today. STAFF RECORDS: The director was interviewing to hire a new teacher for classroom space #3/4. TRANSPORTATION: This program does not offer transportation services. INSPECTIONS: The last documented annual fire inspection was completed on 1/12/26. The last documented sanitation inspection was conducted on 2/18/26. The last documented fire drill was conducted on 2/12/26. The last documented shelter-in-place drill was conducted on 2/8/26. The last documented playground inspection was completed on The last documented water testing for Clean Water for NC Kids was completed on 2/13/24. The center completed Clean Classrooms for Carolina Kids on 9/15/23. The following violation was cited today: Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. The wooden end of one of the plant boxes on the playground had separated, leaving exposed screws accessible to the children. G.S. 110-91(6); .0601(b) Since the violation cited today was corrected during the visit, a Corrective Action letter was not requested for today’s visit. TECHNICAL ASSISTANCE was provided today in the following areas: ABCMS PROVIDER PORTAL North Carolina child care administrators can now view and edit their facility staff roster in ABCMS, the DCDEE’s criminal background check system. The provider will have access to see background check status for all employees and add/delete applicants on their roster. Access to the ABCMS Provider Portal is granted by the DCDEE Criminal Record Check Unit after completing the required Moodle training and submitting the signed form available at the conclusion of the training. NC child care providers must obtain access to the ABCMS Provider Portal to meet the requirement to record new employees within five days of hire, in accordance with NC Child Care Rules. The Division no longer recognizes or accepts the Change of Information form. RATED LICENSE: During today’s visit the director completed the QRIS conversion form, choosing Pathway II for star-rated license reassessment. The purpose of today’s unannounced visit was to monitor applicable child care requirements during an Annual Compliance visit. The last Annual Compliance visit was conducted on 11/9/23. This center operates with a five-star license, which was issued on 6/2/19. COMPLIANCE HISTORY: The compliance history for this center for the previous eighteen-month timeframe was 97%. All indoor and outdoor licensed space was monitored today. OBSERVATIONS: I observed the infants being fed and having tummy time. I observed the young preschoolers having morning snack, before returning outside to play. Morning snack consisted of graham crackers, cut grapes, and water. The children at this center bring their lunches from home, and the center provides morning and afternoon snack. All lunch boxes were stored in the refrigerator until lunch. The director stated that lunches were supplemented, as needed, from items in the snack cabinet for the lunches to meet the NC Child Care nutrition requirements. Current snack menus were posted. TRANSPORTATION: This center does not offer transportation services. CURRICULUM: This center implements Explorations for Young Children. Current activity plans were posted in each classroom. CHILDREN’S FILES: Six children’s files were reviewed today. STAFF FILES: No new staff had been employed at this center since the 11/9/23 Annual Compliance visit. Two staff files were randomly selected and reviewed today to include tallying yearly in-service training requirements. INSPECTIONS: The last documented annual fire inspection was completed on 1/8/24. The last documented sanitation inspection was conducted on 9/24/24. The last documented fire drill was conducted on 11/1/24. The last documented lockdown drill was conducted on 9/6/24. The last documented playground inspection was completed on 11/1/24. The last documented water testing for Clean Water for NC Kids was completed on 2/13/24. The center completed lead/asbestos abatement on 9/15/23 for Clean Classrooms for NC Kids. The following violations were cited today: Violation Number Comment Rule 540 An individual written feeding plan was not provided by child's parent or health care provider or was not followed and posted. (omit posting for centers located in a residence). One child, under the age of 15 months, did not have a feeding schedule. 10A NCAC 09 .0902(a) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. Of the two staff files reviewed today, both staff needed to complete additional in-service training. .1103(a) 1872 Parents were not given the changed Shaken Baby Syndrome and Abusive Head Trauma policy 14 days prior to implementation and did not sign a statement which attests that the new policy was given to and discussed with him or her. Of the six children's records reviewed today, one child did not have the signed statement on file. .0608(c) All violations must be corrected immediately. Please send me a signed Corrective Action letter by 11/21/24, stating how each violation has been corrected. TECHNICAL ASSISTANCE was provided today in the following areas: YEARLY IN-SERVICE TRAINING: Today I explained to the director how to tally yearly in-service training hours for each staff to include hours carried over from the previous year and hours to carry forward to the next Annual Compliance year. HEALTH AND SAFETY TRAINING: I reminded the director that she needed to complete the required Health and Safety trainings again in 2024, in accordance with NC Child Care Rule 10 NCAC 09. 1103 (b). I stated that the director could use 3.5 hours of the in-service training to regain compliance with yearly in-service training hour requirements. CLASSROOM LOFT: There was a loft in classroom space #4/5. The children in classroom space #4/5 were age two. The director stated that children only played in the loft when two teachers were present in the classroom for better supervision of all activities in the classroom. The director stated that the loft was never used for discipline or time out. The director asked for Technical Assistance on the loft. If the loft door remained open, there was a concern that a kid would fall from the loft and get injured. When the door was closed, the children had learned to push on the door for it to open. With momentum a child could fall out the door when he/she pushed to open it. The director asked about latching the loft door for the safety of the children while they were present in the loft. I stated that the door should not be locked while children were in the lot. I also stated that locking the loft door could be considered a violation of NC Fire Code. I stated that I would that I would confer with my supervisor and co-workers to provide resources on a safe solution. RATED LICENSE: Please know that the “hold harmless” status for Rated License reassessment has been extended until the new QRIS guidelines are ready for implementation.
