Upon arrival, I was greeted by the directors, Miranda Marshall and Ashley Litchfield, who assisted me during today’s visit. The owner, Lynn Rasmussen, also was present. LICENSE STATUS The center currently operates with a Five Star license issued on February 22, 2018. The program received seven points for Staff Education, six points for Program Standards, and one Quality Point. It is licensed to serve 263 children on first shift. Prior to today’s visit, I reviewed the NC Secretary of State’s website and noted that the owner of this facility, Permo, Inc., is listed as current/active. INSPECTIONS/DRILLS STATUS -The last sanitation inspection was completed on 7/16/24, and the center scored a superior rating with seven demerits. -The last fire inspection was completed on 4/29/25. -The last documented outdoor play area inspection was completed on 4/1/25. -The last documented fire drill was conducted on 3/24/25. -The last documented lockdown/shelter-in-place drill was conducted on 2/11/25. -The last annual compliance visit was conducted on 5/15/24. MONITORING I completed a walk-through of the center, visiting twelve classrooms as well as the outdoor play areas. The infants were playing on the playground. The toddlers were observed participating in music enrichment and being read a story in circle time. Preschoolers were observed eating breakfast, playing in centers, transitioning from the playground, and washing hands. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records. Sixteen staff and 15 children’s files were reviewed. I also monitored requirements for transporting children, medication administration and storage, and safe sleep records. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 103 The number of children present was not within permit capacity. Twenty-four (24) children were present in Rm. 500, which has a capacity of 21 children at 35 square feet per child. GS 110-91(7) & .1401(f) 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection on file was dated 4/18/24. This violation was corrected today during the visit when the fire marshal conducted an inspection. 10A NCAC 09 .0304(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Clorox wipes and cleaning spray were stored in unlocked under-sink cabinets in the children's bathrooms in room 300. This violation was corrected during the visit. .2820(b) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. An employee with a start date of 9/9/24 had a medical report on file dated 5/3/23. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. An employee with a start date of 9/9/24 had a Tb test on file dated 5/8/23. .0701(a) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. Children's photos were not included in the files of those being transported. 10A NCAC 09 .1003(d) 1921 Staff working in part-time, full-day or track out school age care programs required to complete BSAC training, did not complete it within three months of employment. A lead teacher for school-age students, employed since 1/2/24, has not completed BSAC training. .2510(j) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by May 13, 2025. The letter must address each violation and explain specifically when and how it was corrected, as well as how you will prevent the same violations from happening in the future. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. Failure to maintain compliance may result in an administrative action. The center was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact the center’s overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your center. TECHNICAL ASSISTANCE -Room 500 was over capacity today when two groups were combined for a total of 24 children. You stated that you were not aware the capacity of that room was 21 children. I provided you a copy of the floor plan with the capacities listed for each classroom. You stated that you have the staff and an extra room to split up the group moving forward to fix the capacity issue. -Basic School-Age Care training is required for all staff who work with school-age children. Today, the lead teacher of your school-age group did not have this certification on file. In order to provide high-quality, age-appropriate programming, it is important that staff are well-trained to understand the development and care of school-aged children. -When new staff are hired, they are required to submit a medical report and Tb test that were completed no more than 12 months prior to the first day of employment. One staff member recently hired (Z. McKinnon) has a medical report and Tb test on file that were completed 16 months prior to her date of employment. Also, five staff members submitted documentation after their first day of work. Moving forward, please be sure staff members do not begin work in classrooms until the medical report and Tb test are on file and are no more than 12 months old. -Safety is a top priority when providing care for young children. Today I observed cleaning spray and Clorox wipes stored in an unlocked cabinet under the sink in both bathrooms of Rm. 300. You stated that it was possible the cleaning crew had left the cleaners behind since teachers know not to store anything under bathroom sinks. I suggest conducting a walk-through of the children’s space prior to children’s arrival each morning, removing any hazardous products and placing them in locked storage. -Regularly inspecting the building for potential fire hazards should be a priority when caring for young children. It had been more than a year since your last fire inspection. You called the fire marshal today and he was able to come by for an inspection during my visit. I suggest adding a reminder note on your calendar a month before the next inspection is due so you don’t miss this deadline. -Transporting children is both a big responsibility and a great liability. In the event of an accident, it is important to always have emergency information for the children readily accessible to provide appropriate care. Today, the vehicles used to transport children did not contain photos of the children attached to their emergency information forms. Please add the photos and be sure that all parents have fully completed the medical information, indicating doctor and hospital of preference. CONSULTATION -You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please review and update your EPR plan yearly. -Criminal Backgrounds Checks for all staff members should be completed every five years. Several staff members’ qualifying letters will expire in 2025. I recommend starting the renewal process soon. Providers may request new background checks up to six months in advance of their qualifying letter’s expiration date. TRAINING REMINDERS -Training from 5/15/24 to 4/28/25 was reviewed today and results can be found on the attached Staff/Training Worksheet. All required staff ongoing training hours for the current year should be completed by 6/1/25. -Health and safety training must be completed within the first year of hire and renewed every five years. Staff members who completed training in 2020 are due to renew in 2025. -ITS-SIDS training must be renewed every three years. Several staff members are due to renew their certification this year: C. Couch, Z. Cabuk, D. Jones, and S. Woods. -CPR and First Aid must be renewed every two years. Several staff members are due for CPR and First Aid renewal this year: A. Fortugno, B. Pettiford, A. Litchfield, L. Hayden-Clive, A. Howard, and S. Woods. RATED LICENSE STATUS The NC State Legislature has extended “hold harmless” status until new rated licensing guidelines are in place, and you may opt to keep your current license and delay your rated license assessment until the new guidelines are adopted. Please use this time to update your WORKS account with your staff members’ education information. You can visit the North Carolina Rated License Project website (https://www.ncrlap.org) for resources and training regarding the rated license assessment process. AGENCY UPDATES Please check the Division of Child Development and Early Education’s website frequently for any new policies or rules. It is important to note that the new QRIS rules are not yet effective. However, the Division is preparing for a gradual transition to the new requirements. Today I observed cleaning spray and Clorox wipes stored in an unlocked cabinet under the sink in both bathrooms of Rm. 300. You stated that it was possible the cleaning crew had left the cleaners behind since teachers know not to store anything under bathroom sinks. I suggest conducting a walk-through of the children’s space prior to children’s arrival each morning, removing any hazardous products and placing them in locked storage. -Regularly inspecting the building for potential fire hazards should be a priority when caring for young children. It had been more than a year since your last fire inspection. You called the fire marshal today and he was able to come by for an inspection during my visit. I suggest adding a reminder note on your calendar a month before the next inspection is due so you don’t miss this deadline. -Transporting children is both a big responsibility and a great liability. In the event of an accident, it is important to always have emergency information for the children readily accessible to provide appropriate care. Today, the vehicles used to transport children did not contain photos of the children attached to their emergency information forms. Please add the photos and be sure that all parents have fully completed the medical information, indicating doctor and hospital of preference. CONSULTATION -You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please review and update your EPR plan yearly. -Criminal Backgrounds Checks for all staff members should be completed every five years. Several staff members’ qualifying letters will expire in 2025. I recommend starting the renewal process soon. Providers may request new background checks up to six months in advance of their qualifying letter’s expiration date. TRAINING REMINDERS -Training from 5/15/24 to 4/28/25 was reviewed today and results can be found on the attached Staff/Training Worksheet. Upon arrival, I was greeted by the directors, Miranda Marshall and Ashley Litchfield, who assisted me during today’s visit. The owner, Lynn Rasmussen, also was present. LICENSE STATUS The center currently operates with a Five Star license issued on February 22, 2018. The program received seven points for Staff Education, six points for Program Standards, and one Quality Point. It is licensed to serve 263 children on first shift. Prior to today’s visit, I reviewed the NC Secretary of State’s website and noted that the owner of this facility, Permo, Inc., is listed as current/active. INSPECTIONS/DRILLS STATUS -The last sanitation inspection was completed on 7/16/24, and the center scored a superior rating with seven demerits. -The last fire inspection was completed on 4/29/25. -The last documented outdoor play area inspection was completed on 4/1/25. -The last documented fire drill was conducted on 3/24/25. -The last documented lockdown/shelter-in-place drill was conducted on 2/11/25. -The last annual compliance visit was conducted on 5/15/24. MONITORING I completed a walk-through of the center, visiting twelve classrooms as well as the outdoor play areas. The infants were playing on the playground. The toddlers were observed participating in music enrichment and being read a story in circle time. Preschoolers were observed eating breakfast, playing in centers, transitioning from the playground, and washing hands. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records. Sixteen staff and 15 children’s files were reviewed. I also monitored requirements for transporting children, medication administration and storage, and safe sleep records. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 103 The number of children present was not within permit capacity. Twenty-four (24) children were present in Rm. 500, which has a capacity of 21 children at 35 square feet per child. GS 110-91(7) & .1401(f) 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection on file was dated 4/18/24. This violation was corrected today during the visit when the fire marshal conducted an inspection. 10A NCAC 09 .0304(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Clorox wipes and cleaning spray were stored in unlocked under-sink cabinets in the children's bathrooms in room 300. This violation was corrected during the visit. .2820(b) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. An employee with a start date of 9/9/24 had a medical report on file dated 5/3/23. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. An employee with a start date of 9/9/24 had a Tb test on file dated 5/8/23. .0701(a) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. Children's photos were not included in the files of those being transported. 10A NCAC 09 .1003(d) 1921 Staff working in part-time, full-day or track out school age care programs required to complete BSAC training, did not complete it within three months of employment. A lead teacher for school-age students, employed since 1/2/24, has not completed BSAC training. .2510(j) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by May 13, 2025. The letter must address each violation and explain specifically when and how it was corrected, as well as how you will prevent the same violations from happening in the future. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. The center was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact the center’s overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your center. TECHNICAL ASSISTANCE -Room 500 was over capacity today when two groups were combined for a total of 24 children. You stated that you were not aware the capacity of that room was 21 children. I provided you a copy of the floor plan with the capacities listed for each classroom. You stated that you have the staff and an extra room to split up the group moving forward to fix the capacity issue. -Basic School-Age Care training is required for all staff who work with school-age children. Today, the lead teacher of your school-age group did not have this certification on file. In order to provide high-quality, age-appropriate programming, it is important that staff are well-trained to understand the development and care of school-aged children. -When new staff are hired, they are required to submit a medical report and Tb test that were completed no more than 12 months prior to the first day of employment. I was greeted by Ashley Litchford, one of the directors. Miranda Marshall, one of the directors, assisted me with the visit. A checklist was used to note the requirements monitored today. At the time of the visit, there were a total of One hundred fifty-two (152) children present, ranging in age from infant to five (5) years old. The children were observed participating in circle time, playing with materials, transitioning to lunch, playing outside, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a five (5) star license issued February 22, 2018, earning seven (7) points in education, six (6) points in program standards and one (1) quality point. INSPECTIONS *The last fire drill was conducted 4/24/24. *The last emergency drill was conducted 3/27/24. *The last playground inspection was completed 4/1/24. *The last sanitation was completed on 6/20/23 with a ‘Superior’ classification. *The last fire inspection was conducted on 4/18/24. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. New staff CPR/FA, qualification letters and newly enrolled children files were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. Transportation is provided; therefore, these requirements were monitored and found in compliance. The following violations were observed and documented during today’s visit. Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. Bottles were labeled with a date of 5/12/24 that were stored in the refrigerator. 15A NCAC 18A .2804(d) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. Aquaphor and Destin was stored in the bathroom that the children use in room 300 in the clear pockets that diapers/pull ups are stored. 15A NCAC 18A .2820(d) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. Aquaphor and Desitin did not have written authorization from the parent/guardian. 10A NCAC 09 .0803(1)(a & b) 843 A drug or medicine was administered after its expiration date. Aquaphor in room 300 had an expiration date of 7/2022. 10A NCAC 09 .0803(1)(d) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Small pieces of crayons and sidewalk chalk were in the art area in room 250 that had children two (2) years of age. .0604(q) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The last documented update of the EPR plan was 9/2020. .0607(e) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE During today’s visit, the information below was shared with you to maintain quality within your program and to prevent further violations. MEDICATION Aquaphor and Desitin were stored in the bathroom that the children use in the clear pockets that diapers/pull ups are stored in room 300. The Aquaphor expired with no authorization form from a parent/guardian and the Destin did not have an authorization form from a parent/guardian. It is important to make sure that all medications are stored in a locked storage for the safety of the children in care. It is important to have parents complete a non-prescription medical authorization form to avoid any serious issues. I would recommend checking on a weekly basis that medications are not expired, and medical authorization forms are completed correctly. CHOKING HAZARDS Small pieces of crayons and sidewalk chalk were in the art center in room 250 with children two (2) years old. Those items can potentially cause a choking hazard. The items were removed during the visit. I would recommend that you check daily for small toys, crayons, and sidewalk chalk to avoid any choking hazards. INFANT BOTTLES In room 100 three bottles were labeled with a date of 5/12/24. The teachers were unaware of the date. They stated that the parents may have thought they were labeling with the right date. It is important that all bottles are labeled with the proper date and name daily. I would recommend that the teachers check the bottles before placing the bottles in the refrigerator. EMERGENCY PREPAREDNESS RESPONSE PLAN The EPR plan was not updated annually or when a change occurred in the system. The administrator stated that she was unaware that she needed to go into the system and update items. She stated that the locations for emergencies are still the same. I discussed with the administrator that she should go in the system annually to ensure all items are up to date. I discussed that she will need to go to rmp.nc.gov/portal and use her NCID to complete/update an EPR plan. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. RATED LICENSE INFORMATION This program is in Cohort 1 and the provider would like to proceed with the rated license process. I received the Application for A Two Component Star Rated License and the Center went through the Environmental Rating Scale. --ITERS-R (4/11/2024 score: 4.9) -Item 7: Meals/Snacks Teachers must be engaged with the children at mealtimes. It is best practice that teachers sit and use mealtimes to encourage learning, establish good food habits, and provide a positive experience. -Item 8: Nap Nap schedules should be appropriate for each child. When a child shows signs of tiredness the child must be allowed to rest/sleep in a crib/mat even if the rest of the class is involved in another activity. -Item 9: Diapering/toileting Indicator: 3.1 If the same sink is used for diapering routines and food related use; the faucet handles, and sink must be disinfected after diapering routines and before the sink is used for other purposes. Indicator: 3.2 It is best practice to make sure that each child’s diaper is checked or changed every 2 hours. Indicator:3.3 Proper diapering procedures must be followed to cut down on the spread of germs. Please refer to the NC Child Care Health & Safety Resource Center Diapering Poster for information on the required diaper changing procedures. Also, refer to the Hand Washing Poster for proper hand washing procedures. -Item 12: Listening and Talking It is important to stay away from the words ‘no,’ ‘don’t,’ and ‘that’s not nice.’ Positive language, and positive redirection should be used when correcting children’s behaviors. -Item 13: (This item is addressed in Item 8) -Item 26: Peer Interaction Indicator: 5.1 Staff should always facilitate positive peer interactions instead of stating ‘be nice,’ ‘share,’ and ‘use gentle touches.’ Indicator 5.2 Staff should at no time have negative interactions with each other. It was reported that only one teacher works with 2–3-year-old children for one hour in the morning and in the afternoon each day. It is important to arrange the classroom for proper visual supervision of each center. -Item 7: Space for gross motor play The mulch around the climber must be 6 inches deep to keep the children safe. -Item 10: Meals/Snacks Water should be offered throughout the day; however, water is not nutritionally comparable to milk. Even though a child prefers water it is best practice to offer milk to meet the nutritional component. -Item 11: Nap/Rest It is important to ensure that each child is provided clean bedding prior to nap. Best practice that teachers should assist the children with their bedding to ensure that the children are not dragging the bedding across the floor. -Item 12: Toileting/diapering Please refer to the North Carolina Child Care Health and Safety Resource Center for proper diapering procedures. -Item 13: Health practices To cut down on the spread of germs it is important that the foot pedal is used for the trash can. It is recommended that teachers should teach the children how to operate the foot pedal of the trash can. Mouthed toys should be cleaned and sanitized before being used by others or returned to the storage bins. Cleaning and sanitizing mouthed toys will assist in not spreading germs. -Item 14: Safety practices (This item is addressed in item 7) **Sanitizer should not be sprayed on a table when children are standing at the table. The mist from the solution is a potential eye/lung irritant for children. **Small pieces of chalk and crayons should be removed from the art area. These small pieces are considered a choking hazard. **Electrical cords should not be available to the children. I was greeted by Ashley Litchford, one of the directors. Miranda Marshall, one of the directors, assisted me with the visit. A checklist was used to note the requirements monitored today. At the time of the visit, there were a total of One hundred fifty-two (152) children present, ranging in age from infant to five (5) years old. The children were observed participating in circle time, playing with materials, transitioning to lunch, playing outside, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a five (5) star license issued February 22, 2018, earning seven (7) points in education, six (6) points in program standards and one (1) quality point. INSPECTIONS *The last fire drill was conducted 4/24/24. *The last emergency drill was conducted 3/27/24. *The last playground inspection was completed 4/1/24. *The last sanitation was completed on 6/20/23 with a ‘Superior’ classification. *The last fire inspection was conducted on 4/18/24. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. New staff CPR/FA, qualification letters and newly enrolled children files were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. Transportation is provided; therefore, these requirements were monitored and found in compliance. The following violations were observed and documented during today’s visit. Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. Bottles were labeled with a date of 5/12/24 that were stored in the refrigerator. 15A NCAC 18A .2804(d) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. Aquaphor and Destin was stored in the bathroom that the children use in room 300 in the clear pockets that diapers/pull ups are stored. 15A NCAC 18A .2820(d) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. Aquaphor and Desitin did not have written authorization from the parent/guardian. 10A NCAC 09 .0803(1)(a & b) 843 A drug or medicine was administered after its expiration date. Aquaphor in room 300 had an expiration date of 7/2022. 10A NCAC 09 .0803(1)(d) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Small pieces of crayons and sidewalk chalk were in the art area in room 250 that had children two (2) years of age. .0604(q) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The last documented update of the EPR plan was 9/2020. .0607(e) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before May 29, 2024, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE During today’s visit, the information below was shared with you to maintain quality within your program and to prevent further violations. MEDICATION Aquaphor and Desitin were stored in the bathroom that the children use in the clear pockets that diapers/pull ups are stored in room 300. The Aquaphor expired with no authorization form from a parent/guardian and the Destin did not have an authorization form from a parent/guardian. It is important to make sure that all medications are stored in a locked storage for the safety of the children in care. It is important to have parents complete a non-prescription medical authorization form to avoid any serious issues. I would recommend checking on a weekly basis that medications are not expired, and medical authorization forms are completed correctly. CHOKING HAZARDS Small pieces of crayons and sidewalk chalk were in the art center in room 250 with children two (2) years old. Those items can potentially cause a choking hazard. The items were removed during the visit. I would recommend that you check daily for small toys, crayons, and sidewalk chalk to avoid any choking hazards. INFANT BOTTLES In room 100 three bottles were labeled with a date of 5/12/24. The teachers were unaware of the date. They stated that the parents may have thought they were labeling with the right date. It is important that all bottles are labeled with the proper date and name daily. I would recommend that the teachers check the bottles before placing the bottles in the refrigerator. EMERGENCY PREPAREDNESS RESPONSE PLAN The EPR plan was not updated annually or when a change occurred in the system. The administrator stated that she was unaware that she needed to go into the system and update items. She stated that the locations for emergencies are still the same. I discussed with the administrator that she should go in the system annually to ensure all items are up to date. I discussed that she will need to go to rmp.nc.gov/portal and use her NCID to complete/update an EPR plan. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. RATED LICENSE INFORMATION This program is in Cohort 1 and the provider would like to proceed with the rated license process. I received the Application for A Two Component Star Rated License and the Center went through the Environmental Rating Scale. --ITERS-R (4/11/2024 score: 4.9) -Item 7: Meals/Snacks Teachers must be engaged with the children at mealtimes. It is best practice that teachers sit and use mealtimes to encourage learning, establish good food habits, and provide a positive experience. -Item 8: Nap Nap schedules should be appropriate for each child. When a child shows signs of tiredness the child must be allowed to rest/sleep in a crib/mat even if the rest of the class is involved in another activity. -Item 9: Diapering/toileting Indicator: 3.1 If the same sink is used for diapering routines and food related use; the faucet handles, and sink must be disinfected after diapering routines and before the sink is used for other purposes. Indicator: 3.2 It is best practice to make sure that each child’s diaper is checked or changed every 2 hours. Indicator:3.3 Proper diapering procedures must be followed to cut down on the spread of germs. Please refer to the NC Child Care Health & Safety Resource Center Diapering Poster for information on the required diaper changing procedures. Also, refer to the Hand Washing Poster for proper hand washing procedures. -Item 12: Listening and Talking It is important to stay away from the words ‘no,’ ‘don’t,’ and ‘that’s not nice.’ Positive language, and positive redirection should be used when correcting children’s behaviors. -Item 13: (This item is addressed in Item 8) -Item 26: Peer Interaction Indicator: 5.1 Staff should always facilitate positive peer interactions instead of stating ‘be nice,’ ‘share,’ and ‘use gentle touches.’ Indicator 5.2 Staff should at no time have negative interactions with each other. Staff should always model positive social interactions. -Item 28: Discipline (This item is addressed in Item 8) -Item 29: Schedule (This item is addressed in Item 8) --ECERS-R (4/2/2024 score 6.11) --ECERS-R (4/2/2024 score 5.00) -Item 4: Room arrangement for play Visual supervision is required throughout the day. It was reported that only one teacher works with 2–3-year-old children for one hour in the morning and in the afternoon each day. It is important to arrange the classroom for proper visual supervision of each center. -Item 7: Space for gross motor play The mulch around the climber must be 6 inches deep to keep the children safe. -Item 10: Meals/Snacks Water should be offered throughout the day; however, water is not nutritionally comparable to milk. Even though a child prefers water it is best practice to offer milk to meet the nutritional component. -Item 11: Nap/Rest It is important to ensure that each child is provided clean bedding prior to nap. Best practice that teachers should assist the children with their bedding to ensure that the children are not dragging the bedding across the floor. -Item 12: Toileting/diapering Please refer to the North Carolina Child Care Health and Safety Resource Center for proper diapering procedures. -Item 13: Health practices To cut down on the spread of germs it is important that the foot pedal is used for the trash can. It is recommended that teachers should teach the children how to operate the foot pedal of the trash can. Mouthed toys should be cleaned and sanitized before being used by others or returned to the storage bins. Cleaning and sanitizing mouthed toys will assist in not spreading germs. -Item 14: Safety practices (This item is addressed in item 7) **Sanitizer should not be sprayed on a table when children are standing at the table. The mist from the solution is a potential eye/lung irritant for children. **Small pieces of chalk and crayons should be removed from the art area. These small pieces are considered a choking hazard. **Electrical cords should not be available to the children. The cords could be considered a strangulation hazard for children. --SACERS-U (4/2/2024 5.69) -Item 18: Personal Hygiene Facial tissues should be provided for children in each classroom. -Item 25: Science/nature activities One instance must be observed of science/nature books being used to extend learning about science. HEALTH AND SAFETY TRAINING Health and Safety training is now available through Moodle on the DCDEE website. This training is provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety trainings must be renewed at least every five (5) years. If you completed the training in 2019, you should begin to retake the training now. This program is an expansion of Clean Water for Carolina Kids. Facilities are now required to meet rule requirements NC 10A NCAC 41C. 1001-1007, NC 154A NCAC 18A .2816. Clean Classrooms for Carolina Kids is designed to ensure facilities meet all requirements, receive direct support throughout the participation process, and have access to communication resources. Upon arrival I was greeted by Lynn Rasmussen, the owner. Miranda Marshall assisted me with the visit. I completed a general walk-through of the center. At the time of the visit, there were a total of 147 children present, ranging in age from infant to five (5) years old and 26 staff. The children were observed participating in circle time, playing with materials, interacting with the teachers, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a five (5) star license issued February 22, 2018, earning seven (7) points in education, six (6) points in program standards and one (1) quality point. INSPECTIONS *The last fire drill was conducted 11/3/23. *The last emergency drill was conducted 9/26/23. *The last playground inspection was completed 12/1/23. *The last sanitation was completed on 6/20/23 with a “Superior‘’ classification. *The fire inspection was completed on 3/2/23, and the facility is approved for daytime care only. MONITORING During today’s visit, a partial assessment of Child Care Requirements was conducted for all classrooms. The monitoring included CPR/First Aid, criminal background checks and special training requirements of newly hired staff. The monitoring also included License and permit restrictions, staff/child ratios, supervision, storage of hazardous items, medication, materials/equipment, and all space occupied by the children including outdoor play environment. The following violations were observed and documented during today’s visit. A staff member in suite 300 did not wash hands in between diapering. 15A NCAC 18A .2803(a) 871 Center staff did not comply with the safe sleep policy. SAFE SLEEP POLICES & PRACTICES I discussed with the directors and owner about the safety of placing a child in their individual crib. I would recommend that training is given to the teachers concerning safe sleep policies and practices. NUTRITION FOR INFANTS AND TODDLERS I discussed with the directors and owner strategies to use to ensure that all bottles are labeled properly. I would recommend having the teachers check all bottles to make sure the bottles are labeled correctly. HANDWASHING I discussed proper handwashing methods with the administrators. I would recommend re-training all staff on proper handwashing methods. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. RATED LICENSE STATUS On Monday June 12, 2023, the hold harmless legislation was extended and signed into law. All facilities are allowed to remain at their current star level without reassessment until June 30, 2024. Your facility has been determined to be in Cohort 1. Your reassessment year will be between July 1, 2024, and June 30, 2025. Please use this time to make sure all your staff have a WORKS account with all education information updated. Also review updated resources on the North Carolina Rated License Project (NCRLAP) website (www.ncrlap.org). I discussed with the administrators going through Rated License March 2024. We discussed the timeline and the process. HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge.
