Joan Evans-Bass, Owner/Operator, had to leave for an appointment. Inetta Strudwick, Additional Caregiver, assisted me with the visit. Ms. Bass-Evans gave permission for Ms. Strudwick to sign the paperwork. Your program currently operates with a five-star license, issued July 19, 2017, earning 7 points in the education component, 6 points in the program standards component and 1 quality point for no more than four children under the age of one. The program is approved for daytime and overnight care. *The last annual compliance visit was conducted on 12/4/24. *The last documented fire drill was conducted on 11/3/25. *The last shelter-in-place drill was conducted on 8/4/25. *The last documented outdoor inspection was completed on 11/1/25. Children throughout the program participated in group time, free play in activity areas, transitions, personal care routines, eating lunch and preparing for nap. There are currently no infants enrolled. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. In the children's bathroom there was Lysol All Purpose Cleaner and Clorox Tilex Mold & Mildew cleaner not in locked storage. .1719 (a)(7) 1894 Application did not include names and contact information of individuals to whom the operator may release the child as authorized by the person who signs the application. Three children's applications did not include this information. .1721(a)(3)(F) 1922 Plastic bags, toys, and toy parts small enough to be swallowed, including but not limited to materials that can be easily torn apart such as foam rubber and styrofoam, were accessible to children under the age of three. Trash bags and Walmart bags were underneath the kitchen cabinet accessible to the children under the age of three. .1719(a)(18) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. Your compliance history was ninety-one percent as of 11/26/25. The violations documented must be corrected immediately. On or before 12/17/25 I must receive a written, dated, and signed compliance letter that describes how the violations were corrected. If sufficient information is not received by the due date, a follow-up visit will be conducted. Email the information to: Kia.Reid@dhhs.nc.gov. The email used to send the compliance letter must be sent from the email address registered with the DCDEE (this serves as your signature), and the following information must be included: -Name, position -Facility name -Facility ID number CONSULTATION Children’s records and staff files were monitored. The on-going training requirements were monitored from December 2024-2025 and was in compliance for Ms. Bass-Evans. I have contacted our training coordinator to verify if two training organizations are approved for Ms. Strudwick. TECHNICAL ASSISTANCE -I suggested Ms. Bass-Evans use the children’s file checklist to ensure all paperwork is in the file. Best practice would be to check the files twice per year to make sure all forms are present. I left a copy of the children’s file checklist updated 6/23. -A walk-through of the children’s space should be conducted prior to arrival each day, removing any potential hazards and placing them at least five feet high or in locked storage. Safety is a priority when caring for young children. QRIS REQUIREMENTS Before Ms. Bass-Evans left for her appointment I asked if she had reviewed the information on the QRIS- Pathways to the Stars and she stated no. I explained that she would need to visit the DCDEE website and review the three pathways needed to achieve stars. I will contact you in January 2026 to schedule a technical assistance visit to discuss the pathway that will be chosen. As of February 1, 2025, the FCCERS-3 will be used for the assessments. Access www.ncrlap.org to participate in webinar sessions that will focus on the changes. I stated that the webinars could be used to meet yearly in-service training hour requirements. CHILD CARE RULES Child Care Rules have been updated as of 7/1/25. Please visit the DCDEE website to review and download a copy. FCCH CAPACITY INCREASE In order for you to begin the process, please send me verification of the following: *Approval from the local Zoning Office/Department for capacity increase. *Rooms and areas within the family child care home where occupants receive care are on the same level of exit discharge. *Rooms and areas within the family child care home where occupants receive care are located on the same level with, and within a maximum of 40 feet travel distance to at least one, 2A:10B:C fire extinguisher. *The family child care home has and maintains a Fire Safety, Evacuation, and Lockdown Plan compliant with Section 404 of the North Carolina Fire Code. *The family child care home has a carbon monoxide alarm and detection system compliant with Section 915 of the North Carolina Building Code and Section R315 of the North Carolina Residential Code. NA is allowed only if the dwelling unit does not contain a fuel-fired appliance (stove, oven, water heater, furnace, clothes dryer, etc.), fireplace, or have an attached garage with a door/entrance that opens into with the dwelling unit *The family child care home has smoke alarms compliant with Section R907 of the North Carolina Building Code and Section R314 of the North Carolina Residential Code. More information on each requirement above can be found here: https://ncchildcare.ncdhhs.gov/Whats-New/important-updates-for-family-child-care-homes Once I have received verification of the above, I will schedule a visit to your home to monitor compliance and begin the process to issue a new license, with the increased capacity. You will need to ensure that you have sufficient materials and equipment for the increased number of children, including cribs/mats/cots and play materials. You may NOT increase the number of children in your care until a new license is issued. REMINDERS The following requirements will come due in the near future; please make arrangements to complete the items before the deadline. *You must conduct an emergency drill by 12/31/25. *The qualification letter for your HHM will expire on 2/22/26. Paperwork for the 5 year recheck can be submitted six months prior to the expiration of the qualification letter. *The vaccination for your pet will expire on 3/11/26. *Please check the Division of Child Development and Early Education website: ncchildcare.ncdhhs.gov frequently and click the “what’s new” tab to stay informed of all updates available to you. At the completion of the visit, this visit summary was printed, reviewed, and a copy was left with Ms. Strudwick. The purpose of today’s visit was to monitor applicable childcare requirements during your Annual Compliance visit. The last Annual Compliance visit was conducted on 1/23/23. This Family Child Care Home was issued a five-star license on 7/19/17. COMPLIANCE HISTORY: The compliance history for the previous 18-month timeframe was 95%. During today’s visit the indoor child care space and the outdoor environment were monitored. OBSERVATIONS: There were four preschool age children present today. I observed the children resting during naptime and playing in child care space. CURRICULUM: This Family Child Care Home implements the Mother Goose curriculum. The theme for the week was “Twinkle Twinkle Little Star”. Current menus were posted. During today’s visit the operator’s and additional caregiver’s verification form was reviewed, along with the enrollment worksheet and the statements of responsibility. CHILDREN’S RECORDS: The records for new children were reviewed today. SMOKE DETECTOR: The electrically-operated smoke detector with battery-backup was tested during the visit. TRANSPORTATION: The operator stated that she was not currently conducting transportation services, but she asked me to monitor her vehicle in case she decided to resume the transportation services. The Lincoln Navigator with license number WYJ-5671 was monitored during the visit. The tags on the vehicle had expired in November2023. The operator was aware that transportation could not be resumed until the vehicle met all NC transportation requirements. INSPECTIONS: The last documented fire drill was conducted on 1/2/24. The last documented lockdown/shelter-in-place drill was conducted 1/2/24. The last outdoor inspection was completed on 1/1/24. The following violations were cited today. Violation Number Comment Rule 538 The written schedule did not reflect daily opportunities for both free choice and guided activities. The written schedule was not posted for review during today's visit. .1718(a )(7)(C ) 1409 Operator did not provide a physically safe and healthy indoor and outdoor environment that meets the developmental needs of the children in care. (!) The wooden ladder for the outdoor climber had deteriorated at the bottom, leaving splintered wood and an exposed nail. (2) There were exposed roots that needed to be covered at the climber and under the swings. 10A NCAC 09 .1719 (a) All violations must be corrected immediately. Please send me a signed Corrective Action by 1/22/24, stating how each violation has been corrected. TECHNICAL ASSISTANCE WAS PROVIDED TODAY IN THE FOLLOWING AREAS: RATED LICENSE: I stated that Rated License reassessments had resumed. I made the operator aware that Kadejah’s Playhouse was in Cohort 1 for the reassessment. I discussed that the preparation year for the reassessment began 7/1/23 and that the year for the reassessment would begin on 7/1/24. During today’s visit I discussed the option and advantage of having the FCCERS-S conducted during the preparation year. The operator stated that two Technical Assistants from CCSA had been assigned to this facility to prepared the operator for the Environment Rating Scales. The operator stated that the two Technical Assistants (one for the outdoor environment and one for the indoor environment) had not made their first visit. The operator was interested in having the Environment Rating Scales conducted during the preparation year. The operator was given an application for the Rated License reassessment, along with a request for the Environment Rating Scales.
