Upon arrival, I was greeted by the provider, Darlene Mitchell, who assisted me during today’s visit. LICENSE STATUS The family child care home currently operates with a Four Star license issued on August 11, 2020. The program received seven points for Staff Education, two points for Program Standards, and one Quality Point. INSPECTIONS/DRILLS STATUS -The last annual compliance visit was conducted on 4/10/25. -The last documented monthly outdoor play area inspection was completed on 2/6/26. -The last documented monthly fire drill was conducted on 2/6/26. -The last documented quarterly lockdown/shelter-in-place drill was conducted on 1/9/26. MONITORING I completed a walk-through of the home, including the children’s space, kitchen, bathroom, and outdoor learning environment. At the time of the visit, there were four children present ranging in age from five months to four years old. There are eleven children total enrolled on first and second shift. The children were observed having circle time with the provider, playing freely with materials, getting a diaper change, cleaning up, eating lunch, and transitioning to rest time. Lunch consisted of chicken nuggets, green bean, pineapple, and milk. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, capacity, health and safety requirements, equipment and furnishings, and program records. The provider does not transport children; therefore, this requirement was not monitored. No enrolled children have medications or medical care plans, so these requirements were not monitored. Children 12 months or younger are enrolled; therefore, safe sleep requirements were monitored and a violation was cited. The provider verified that all the information on the FCCH Verification of Required Information for Operator and Additional Caregivers form was correct. INDOOR/OUTDOOR ENVIRONMENT Inside the home, children are cared for in a living room adjoining the kitchen. A variety of items for learning through play are available, including manipulatives, blocks, books, art materials, and home living props. The bathroom and changing table are located within sight of the children’s play area. The outdoor play area is a unfenced backyard with a variety of gross motor equipment. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. An infant who enrolled on 12/1/25 did not have a medical report on file. GS 110-91(1); 10A NCAC 09.1721(a)(1) 911 Copy of each child's immunization record, including the operator's own preschool child(ren), was not on file. An infant who enrolled on 12/1/25 did not have an immunization record on file. GS 110-91(1); .1721(a)(2) 1821 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of 30 days. Sleep checks for an infant enrolled on 12/1/25 were not recorded in January, February, or March 2026. .1724(a)(8)&(f) 1876 The operator did not review the EPR Plan annually or when information in the plan changed to ensure all information is current. The EPR plan was last reviewed on May 20, 2024. .1714(e ) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by March 25, 2026. The letter must address each violation and explain specifically when and how it was corrected, as well as how you will prevent the same violations from happening in the future. Please send this to mandy.annunziata@dhhs.nc.gov and include the following information: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today’s visit, the facility’s compliance history score was 98%. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. Failure to maintain compliance may result in an administrative action. The facility was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact your overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your program. TECHNICAL ASSISTANCE -It is important to have a clear action plan in case of an emergency. Your EPR plan was last updated 5/20/24. You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please update your EPR plan and review it yearly. -When caring for infants, it’s important to follow safe sleep practices because the consequences for failing to follow procedures can be life-threatening. One required component of safe sleep policy is visually checking infants and documenting these sleep checks on a sleep log, which must be retained for 30 days. Today, the logs revealed that the sleep checks have not been documented regularly. You said you usually write the nap times on the daily note you give to the infant’s parent. In order to comply with this rule, you must document the sleep checks every 15 minutes in a way that they can be reviewed when I visit. Using the infant sleep log found under "Provider Documents and Forms" can help you remember to record all the required information. -Record keeping is an important piece of a FCCH operation. Today, an infant was missing a medical report and immunization record, which are required within 30 days of enrollment. You stated that you have given the child’s parent several reminders. To ensure each child’s file is complete, you may reference the children’s file checklist available on the DCDEE website under the “Provider Documents and Forms” tab. Requiring all forms at the time of enrollment will help you stay in compliance. CONSULTATION -We reviewed your progress preparing for your upcoming rated license assessment. If you have completed additional coursework since your last submission to WORKS, please submit your education transcripts to DCDEE WORKS as soon as possible to allow sufficient time for processing and plan to send your application to me by 4/15/26. The rated license application can be found on the DCDEE website under “Provider Documents and Forms,” and links to all required forms are on that document. All ongoing training hours for this year have been completed, and four hours will carry over to next year. -CPR and First Aid must be renewed every two years. Your CPR and First Aid certification expires 5/16/26. -ITS-SIDS training must be renewed every three years. Your training expires on 1/12/27. -Health and safety training must be renewed every five years. Your training expires May 2029. RATED LICENSE STATUS New QRIS rules are in effect, and the Division is transitioning programs to their new rated license in 2026. Resources, forms, and training will be provided to support you in preparing for new pathway options to earn your Two- through Five-Star Rated License. Upon arrival, I was greeted by the provider, Darlene Hester, who assisted me during the visit. LICENSE STATUS The family child care home currently operates with a Four Star License issued on August 11, 2020. The program received two points for Program Standards, seven points for Staff Education, and one Quality Point. The center was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact the center’s overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your center. TECHNICAL ASSISTANCE -A newly enrolled student’s file was incomplete today. To ensure you have all required forms, I recommend collecting them from the parent before you begin caring for the child. You can also use the children’s file checklist available on the DCDEE website under the “Provider” tab. -Providers are required to keep daily attendance records for all children in their care, including drop-off and pick-up times. Accurate attendance records show that you remain in compliance with child care ratios as well as provide a list of children present in the event of an emergency. I recommend placing the attendance log next to your entry door for parents to sign children in and out each day. -Monthly outdoor inspections should be completed to ensure that children have a safe place to play. Any hazards should be removed or repaired promptly. I recommend conducting the inspections on the same day of each month (for example, the first day of each month) to help you remember when inspections are due. -No activity plan was available for review today. It is important to have a lesson plan posted to be sure all domains of learning are covered and to show differentiation in planning between the ages of the children you serve. In addition, parents can be informed about what their children are learning. CONSULTATION -Criminal Backgrounds Checks for all staff members should be completed every five years. You are due to renew your qualifying letter before 2/13/25. I recommend starting the process now. -Since children are cared for on the lower level of the home, I would recommend finding a spot to keep important files downstairs so they are easily accessible. If they are stored upstairs, you would have to leave children unsupervised to retrieve them, which would be a violation of child care rules. -Today you had plenty of toys on shelves for your younger children, but not as many for older preschoolers. I recommend setting up a book center and adding in some art and manipulative areas to supplement your block and housekeeping centers. TRAINING REMINDERS -CPR and First Aid must be renewed every two years. -ITS-SIDS training must be renewed every three years. -Health and safety training must be renewed every five years. Be sure to include “Recognizing and Responding to Suspicions of Child Maltreatment” which can be found on the Prevent Child Abuse NC website. RATED LICENSE STATUS The NC State Legislature has extended “hold harmless” status until new rated licensing guidelines are in place, and you may opt to keep your current license and delay your rated license assessment until the new guidelines are adopted. The program received two points for Program Standards, seven points for Staff Education, and one Quality Point. INSPECTIONS/DRILLS STATUS -The outdoor play area inspection log was not available for review. -The last documented fire drill was conducted on 11/8/24. -The last documented lockdown/shelter-in-place drill was conducted on 11/8/24. -The last annual compliance visit was conducted on 5/7/24. MONITORING I completed a walk-through of the home, visiting the caregiving space, kitchen, bathroom, and outdoor play area. Four children were present, ranging in age from one year to four years old. The children were engaged in free play, getting a diaper change, using the restroom, washing hands, and eating lunch. Lunch was chicken and cheese tacos, corn, pineapple, and milk. A partial assessment of child care requirements was conducted. I monitored all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records.
