The purpose of today’s unannounced visit was to monitor applicable child care requirements for your Annual Compliance visit. The center owner was also present during today’s visit. The last Annual Compliance visit was conducted on 3/4/25. This center operates with a four-star license, which was issued 8/16/18. COMPLIANCE HISTORY: The compliance history for this center for the previous eighteen-month timeframe was 92%. During today’s visit all indoor and outdoor licensed space was monitored. OBSERVATIONS: I observed the children in classroom space A dancing to music and completing a teacher-directed activity. I observed the younger children enjoying the outdoor environment and resting during naptime. CHILDREN’S RECORDS: Twelver children’s records were reviewed today. STAFF RECORDS: I received the staff/training worksheets prior to today’s visit. The information on the worksheets was reviewed during the visit. TRANSPORTATION: The bus, used for transportation, with license #JC-1135 was monitored today for transportation requirements. The bus driver’s license was checked. INSPECTIONS: The last documented annual fire inspection was completed on 9/29/25. The last documented sanitation inspection was conducted on 10/8/25. The last documented fire drill was conducted on 1/6/26. The last documented shelter-in-place drill was conducted on 12/12/25. The last documented playground inspection was completed on 2/11/26. The last documented water testing for Clean Water for NC Kids was completed on 6/28/24. The center is still working with EPA on steps for completion of the Clean Classrooms for Carolina Kids program. The following violation was cited today: Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. There were broken pieces and discarded pieces of equipment that needed to be removed from fenced-in space. G.S. 110-91(6); .0601(b) All violations must be corrected immediately. Please send me a signed Corrective Action letter by 3/9/26, stating how the violation has been corrected. TECHNICAL ASSISTANCE was provided today in the following areas: INDOOR ENVIRONMENT: During today’s visit I made the following requested: • Add chapter books in classroom space A to accommodate school age children. • Re-secure the electrical cord to the wall in classroom space E. • Consider removing the wooden blocks from classroom space B, which is the classroom for infant and toddler age children. ABCMS PROVIDER PORTAL North Carolina child care administrators can now view and edit their facility staff roster in ABCMS, the DCDEE’s criminal background check system. The provider will have access to see background check status for all employees and add/delete applicants on their roster. Access to the ABCMS Provider Portal is granted by the DCDEE Criminal Record Check Unit after completing the required Moodle training and submitting the signed form available at the conclusion of the training. NC child care providers must obtain access to the ABCMS Provider Portal to meet the requirement to record new employees within five days of hire, in accordance with NC Child Care Rules. The Division no longer recognizes or accepts the Change of Information form. RATED LICENSE: The owner and director attended the training on 9/18/25 on the new QRIS, star-rated license process. During the 9/22/25 visit, the director stated that she and the owner were processing the information discussed in the training to make their pathway decision. During today’s visit the owner and director stated that they wanted to select Pathway I for the star-rated license reassessment. The director signed a conversion form during the visit. The owner stated that the center wanted to achieve a four-star license. Technical Assistance was provided by reviewing all the requirements to achieve a four-star license through Pathway I. Technical Assistance was provided on preparing for the Environment Rating Scales assessment to include the ITERS-3, ECERS-3, and SACERS-U. I stated that the center should purchase the assessment tool books as soon as possible. The Environmental Rating Scales had been conducted at this center in the past. I suggested that the center contact Early Years, regarding the availability of training on Autism. I stated that free webinars were offered on the ECERS-3 and ITERS-3 through the NC Rated License Assessment Project. www.ncrlap.org. I stated that the webinars could be used to meet yearly in-service training hour requirements. I requested that the center send me their application for reassessment on 4/1/26. Iley Palmer, Owner, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Palmer, Monet Palmer, Director and additional staff members. Limited monitoring of child care requirements occurred during today’s visit. Violation Number Comment Rule 303 Children were not adequately supervised at all times. On August 22, 2025, a staff member left a 13-month-old child unsupervised in the center after closing. .1801(a)(1-5) 1810 There was a substantiation of child maltreatment. On November 5, 2025, the Division substantiated child maltreatment based on the supervision to a child. The purpose of today’s unannounced visit was to monitor applicable child care requirements during the center’s Annual Compliance visit. This center operates with a four-star license, which was issued on 8/16/18. The last Annual Compliance visit was conducted on 3/12/24. COMPLIANCE HISTORY: The compliance history for this center for the previous eighteen-month timeframe was 90%. During today’s visit I monitored all indoor and outdoor licensed spaces. OBSERVATIONS: Upon arrival all three groups of children were outside, enjoying the outdoor environment. The preschool age children were playing a running game with Hola-hoops. I later listened to the children singing while I reviewed records. CHILDREN’S RECORDS: Eleven children’s records were reviewed. STAFF RECORDS: Staff records were reviewed today, and yearly in-service training was tallied. CURRICULUM: This center is implementing the Creative Curriculum. TRANSPORTATION: The bus with license # JC1135 was monitored for transportation requirements. INSPECTIONS: The last documented annual fire inspection was completed on 10/16/24. The last documented sanitation inspection was conducted on 1/21/25. The last documented fire drill was conducted on 2/13/25. The last documented shelter-in-place drill was conducted on 3/3/25. The last documented playground inspection was completed on 2/11/25. The last documented water testing for Clean Water for NC Kids was completed on 6/28/24. The following violations were cited today: Violation Number Comment Rule 316 Children under one year of age were not kept separate from children two years and older. The enrollment in classroom space #B was one infant, 2 toddlers, and 3 two-year old children. 10A NCAC 09 .0713(a)(5) All violations must be corrected immediately. Please send me a signed Corrective Action letter by 3/18/24, stating how each violation has been corrected. TECHNICAL ASSISTANCE was provided today in the following areas: GROUPING OF CHILDREN: To assist the center in deciding how to re-group the children in classroom space #B, I explained that children ages 0 and 1 could be grouped together, and children ages 1and above can be grouped together. CLASSROOM SPACE #C: Classroom space #C was currently being used in the mornings and afternoons for school age children. The classroom was set up for young pre-school age children. As Technical Assistance I discussed setting up the classroom with materials that were appropriate for school age children. I stated that these changes would be essential if the center cares for school-age children during the summer months. The new director stated that she had ordered materials that would be appropriate for school age children. ADMINISTRATOR: During today’s visit the owner/director’s daughter completed the pre-service administration form to be the director at this center. The owner/director was present during today’s visit. RATED LICENSE: Please know that the “hold harmless” status for Rated License reassessment has been extended until the new QRIS guidelines are ready for implementation. AGENCY UPDATES: Child Care facilities are required to test their drinking and cooking water for lead every three years. For information you should contact Clean Water for Carolina Kids www.cleanwaterforcarolinakids.org. The operator stated ___ Child care facilities are now required to test for lead and asbestos hazards in their buildings. The Clean Classrooms for Carolina Kids program is designed to identify and eliminate exposure to lead and asbestos hazards in building infrastructure, while providing child care facilities with the support needed to meet all rule requirements. The program is free, and the first step is to watch the pre-enrollment webinar, which are available at www.cleanwaterforUSkids.org/carolina. The director stated that they completed the video and submitted their responses approximately two weeks ago. CPSC RECALLS The U.S. Consumer Product Safety Commission has recalled numerous items. The program is in the "held harmless" status and points from the previous assessment are carried forward. The program received 5 points in Program Standards, 4 points in Staff Education, and 1 quality point through the programmatic option of having a staff benefits package and an infrastructure of parent involvement. INSPECTION/SAFETY DRILL STATUS: The last sanitation inspection was completed on 12/20/23. The last fire inspection was completed on 10/6/23. The last outdoor play area inspection was completed on 3/11/24. The last fire drill was conducted on 2/12/24. The last documented lockdown/shelter-in-place was conducted on 12/11/23. The children were observed participating in outdoor play, group time activities, and personal routine care. The caregivers were interacting with the children, meeting their developmental needs. The following violations were observed and cited during today’s visit: Violation Number Comment Rule 615 Beds, cots and mats were not in good repair, properly handled, stored, or clean and sanitized between users. Three mats in space "A" was not in good repair. 15A NCAC 18A .2821(a) 807 A safe indoor and outdoor environment was not provided for the children. On the large playground a mop bucket with dirty water was stored accessible children. 10A NCAC 09 .0601(a) 811 Potentially hazardous items including but not limited to power tools, nails, chemicals, propane stoves, lawn mowers, gasoline, or kerosene were not stored in locked areas, removed from the premises, or made inaccessible to children. A bottle of hand sanitizer was stored in the drink holder of the bus. .0604(a) 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. On the large playground a mop bucket with dirty water was stored accessible children. 10A NCAC 09 .0601(a) 811 Potentially hazardous items including but not limited to power tools, nails, chemicals, propane stoves, lawn mowers, gasoline, or kerosene were not stored in locked areas, removed from the premises, or made inaccessible to children. A bottle of hand sanitizer was stored in the drink holder of the bus. .0604(a) 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. One employee did not have verification on file for EMC annual review. 10A NCAC 09 .0802(a) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. One child's file did not have a health care professional listed. .0802(c)(2) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. TECHNICAL ASSISTANCE During my visit today we reviewed the staff training worksheet. When you complete the computerized staff training worksheet it will improve your work skills and knowledge of staff needs. Also, there will only be a couple of items that you will have to change so this will save you time when you must update yearly. EMERGENCY PREPAREDNESS/ EMERGENCY MEDICAL CARE All staff must review the emergency preparedness response plan and emergency medical care plan on an annual basis. The review should list procedures to be followed to ensure that any child who becomes ill or is injured and requires medical attention while in care at the center receives appropriate attention. RATED LICENSE You are in your planning period and your Assessment period starts 7/1/25. Your request has been confirmed for the Environment Rating Scale Assessment. Make sure that staff have updated their WORKS accounts to reflect all current coursework. Iley Palmer, Owner, accompanied me during a walk-through of the facility . During the visit, I discussed the allegations with Ms. Palmer and three additional staff members. Limited monitoring of child care requirements occurred during today’s visit. The following violations were confirmed during today’s visit. Violation Number Comment Rule 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file.
