Upon arrival, I was greeted by the provider, Lolita Deberry, who assisted me during today’s visit. LICENSE STATUS The family child care home currently operates with a Four Star license issued on October 18, 2019. The program received seven points for Staff Education, two points for Program Standards, and one Quality Point. INSPECTIONS/DRILLS STATUS -The last annual compliance visit was conducted on 7/17/25. -The last documented monthly outdoor play area inspection was completed on 6/18/26. -The last documented monthly fire drill was conducted on 5/19/26. -The last documented quarterly lockdown/shelter-in-place drill was conducted on 4/6/26. MONITORING I completed a walk-through of the home, including the children’s space, kitchen, bathroom, and outdoor learning environment. At the time of the visit, there were four children present ranging in age from 11 months to two years old. There are 5 children enrolled on first shift and six children enrolled on second shift. The children were observed playing freely with materials, reading a book with the provider, eating lunch, and transitioning to rest time. Lunch today consisted of chicken nuggets, French fries, apple slices, and milk. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, capacity, health and safety requirements, equipment and furnishings, and program records. The provider does not transport children; therefore, this requirement was not monitored. No enrolled children have medications or medical care plans, so these requirements were not monitored. A child 12 months or younger is enrolled; therefore, safe sleep requirements were monitored and found in compliance. The provider verified that all the information on the FCCH Verification of Required Information for Operator and Additional Caregivers form was correct. INDOOR/OUTDOOR ENVIRONMENT Inside the home, children are cared for in a converted garage. A variety of items for learning through play are available, including manipulatives, art supplies, and electronic toys. The floor is covered with soft foam tiles and an area rug. There is a changing table within the children’s space, and a bathroom is located in a hallway just outside the play space. Cots and cribs and used for rest time, and high chairs as well as a table and chairs are available for eating. The outdoor play area is a fenced backyard with a large deck. You stated that you bring balls, hula hoops, and riding toys outside during outdoor play time. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. One child was missing a medical report (L.M.). GS 110-91(1); 10A NCAC 09.1721(a)(1) 911 Copy of each child's immunization record, including the operator's own preschool child(ren), was not on file. Four children were missing immunization records (N.M., L.M., T.K., and K.K.). GS 110-91(1); .1721(a)(2) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by July 8, 2026. The letter must address each violation and explain specifically when and how it was corrected, as well as how you will prevent the same violations from happening in the future. Please send this to mandy.annunziata@dhhs.nc.gov and include the following information: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today’s visit, the facility’s compliance history score was 98%. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. Failure to maintain compliance may result in an administrative action. The facility was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact your overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your program. TECHNICAL ASSISTANCE -It is important to keep accurate records for all children in your care. Children’s medical records ensure that children are healthy and able to participate in activities. Today, four children’s files were missing immunization records and one child’s file was missing a medical record. To ensure each child’s file is complete, you can use the children’s file checklist available on the DCDEE website in “Provider Documents and Forms” under the “Provider” tab. CONSULTATION -You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please review and update your EPR plan yearly. -Criminal Backgrounds Checks for the provider and all household members 16 years of age and older should be completed every five years. -You stated that you would like to improve your outdoor learning environment. I recommend contacting Early Years (https://www.earlyyearsnc.org/) to see if they have any technical assistance or grants available. You also can sign up to receive the weekly Raise NC newsletter from DCDEE that lists any available assistance for program improvement. -One resource to obtain virtual CEUs is the Southwestern Child Development Commission: https://www.swcdcinc.org/ TRAINING REMINDERS -Training from 7/17/25 to 6/10/26 was reviewed today and documented on the Verification of Required Information for Operator and Additional Caregivers form. All required ongoing training hours for this year have been completed. -CPR and First Aid must be renewed every two years. Your CPR and First Aid certification expires 4/18/28. -ITS-SIDS training must be renewed every three years. Your training expires on 4/6/27. -Health and safety training must be renewed every five years. Your training expires in 2027. RATED LICENSE ASSESSMENT INFORMATION The facility submitted an application for Pathway #2 (Classroom and Instructional Quality) in the new QRIS system. The following items have been monitored and/or completed: -The facility does not have any enrollment restrictions for the chosen pathway star level and was in compliance with license capacity and restrictions during today’s visit. -The facility has a plan to implement the five foundational practices in the Family and Community Engagement Standards. They also have a plan to implement two additional options, with at least one from each of the three categories of engagement. -The facility submitted a Continuous Quality Improvement Plan today. Each required staff member has a plan to complete individual CQI goals annually. The Facility CQI Plan and all Individual CQI Plans must be completed over the next 12 months. -The facility has chosen to implement the use of Creative Curriculum and Desired Results Developmental Profile. -All required staff at the facility will complete training on the chosen curriculum and formative assessment. Please notify me as soon as you have completed the training and I will submit your license paperwork. -The facility has a plan to share child assessment results with families once annually. -All required staff at the facility have a plan to participate in five hours of coaching/mentoring, additional on-going professional development training or CEUs. If training or CEUs are selected, it must be in addition to the staff’s required annual professional development requirement and must be completed over the next 12 months. -In order to meet four star education standards, the provider will complete 1.5 CEUs annually in addition to required ongoing training hours. The facility has a current compliance history score of 98%. Based on having completed all the requirements in Pathway #2 (Classroom and Instructional Quality) at the chosen star level, the center will earn a four star rated license that will be effective on the date your curriculum and formative assessment training is complete. Annual monitoring of all Pathway requirements will now occur to ensure that you maintain your Star Level and your next full assessment will be in 2029. The facility also has implemented four of the Quality Initiatives listed in Child Care Rule .3224. Verification of each of the implemented Quality Initiatives was monitored during the visit and are identified on the enclosed form. Upon arrival, I was greeted by the provider, Lolita Deberry, who assisted me during the visit. LICENSE STATUS The family child care home currently operates with a Four Star License issued on October 18, 2019. The program received two points for Program Standards, seven points for Staff Education, and one Quality Point. INSPECTIONS/DRILLS STATUS -The last documented outdoor play area inspection was completed on 12/19/25. -The last documented fire drill was conducted on 7/14/25. -The last documented lockdown/shelter-in-place drill was conducted on 7/20/25. -The last annual compliance visit was conducted on 7/17/25. MONITORING I completed a walk-through of the home, visiting the caregiving space, bathroom, and outdoor play area. There are four children enrolled on first shift and four children enrolled on second shift. At the time of my visit, four children were present, ranging in age from one year to three years old. The children were observed finishing breakfast, reading a book with the provider, and playing freely with materials. A partial assessment of child care requirements was conducted. I monitored all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records. Newly enrolled children’s records also were reviewed. No enrolled children have medications or medical care plans, so these requirements were not monitored. No children 12 months of age or younger are enrolled; therefore, safe sleep requirements were not monitored. INDOOR/OUTDOOR SPACE Children are cared for in a converted garage, and a variety of materials are available for play, including blocks, manipulatives, books, and infant/toddler toys. A bathroom is located just outside the caregiving space for children who are toilet trained, and there is a changing table within the caregiving space. Cots and portable cribs are used for rest time. The backyard is fenced with some basketballs for gross motor play. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 1853 The operator did not conduct a monthly fire drill. The last documented monthly fire drill was in July 2025. .1719(a)(15) & .1721( e)(2) 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. The last documented lockdown drill was in July 2025. .1719(a )(16) & .1721(e )(7) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. TECHNICAL ASSISTANCE -Emergency drills, such as fire drills and lockdown/shelter-in-place drills, must be conducted in a timely manner because they allow the provider and children to practice and prepare for an actual emergency. Today, your drill log showed a lapse in required practice drills. You stated that you had forgotten because you had been dealing with some family matters. Fire drills must be conducted at least once per month, and a shelter-in-place/lockdown drill must be conducted at least once per quarter (every three months). I suggest setting a reminder on your calendar to help you maintain compliance with this rule. CONSULTATION -We discussed the new licensure pathway options and discussed how to access the QRIS information and rated license application on the DCDEE website under “Provider Documents and Forms.” You stated that you are considering Pathway #2 at this point and are currently using the Creative Curriculum. You can find approved formative assessments here: https://ncchildcare.ncdhhs.gov/Services/NC-Pre-K-Program/Formative-Assessment -Because you serve several children who are ages three and older, be sure you have plenty of materials in your play space that support preschool play. Dramatic play props, such as puppets, pretend food, play dishes, dress up clothes, and baby dolls would enhance your home living center. Adding cars, trucks, and play people/animals would add another dimension to block play. Think about including simple arts/crafts materials, such as paper, crayons, fingerpaint, and glue. Manipulatives like puzzles and items to sort/count would also provide some age-appropriate challenges for fine motor activities. Using playdough with plenty of rollers and cookie cutters also works small muscles in children’s hands and provides sensory experiences. You stated that you have some materials in storage that you can add. I suggest referencing “Foundations of Early Learning and Development” on the DCDEE website for ideas/guidance. I also will email you a list of suggested items for preschool activity centers. You can reference Child Care Rule 10A NCAC 09 .1718 (a)(8)(C) and (D) for required daily and weekly activity offerings. -I also recommend supplementing your outdoor play materials to enhance gross motor play. You could consider adding a preschool-sized basketball hoop, riding toys, small portable climbing equipment, small balls, or hula hoops. -Criminal Backgrounds Checks for all providers should be completed every five years. -The Clean Classrooms for Carolina Kids Program provides free facility-wide testing for lead in water and inspections for asbestos and lead-based paint hazards without any out-of-pocket costs. Your water was tested on 7/27/24, and no mitigation was recommended. -You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please review and update your EPR plan yearly. TRAINING REMINDERS -CPR and First Aid must be renewed every two years. Your training is valid through 4/3/26. -ITS-SIDS training must be renewed every three years. Your training is valid through 4/6/27. -Health and Safety Training must be renewed every five years. RATED LICENSE STATUS The NC State Legislature has extended “hold harmless” status until new rated licensing guidelines are in place, and you may opt to keep your current license and delay your rated license assessment until the new guidelines are adopted. Please use this time to update your WORKS account with your education information. You can visit the North Carolina Rated License Project website (https://www.ncrlap.org) for resources regarding the rated license assessment process. Upon arrival, I was greeted by the provider, Lolita Deberry, who assisted me during today’s visit. LICENSE STATUS The family child care home currently operates with a Four Star license issued on October 18, 2019. The program received seven points for Staff Education, two points for Program Standards, and one Quality Point. INSPECTIONS/DRILLS STATUS -The last annual compliance visit was conducted on 7/22/24. -The last documented monthly outdoor play area inspection was completed on 7/14/25. -The last documented monthly fire drill was conducted on 7/14/25. -The last documented quarterly lockdown/shelter-in-place drill was conducted on 4/16/25. MONITORING I completed a walk-through of the home, including the children’s space, kitchen, bathroom, and outdoor play area. At the time of the visit, there were four children present ranging in age from one year to three years old. There are seven children enrolled on first shift and five enrolled on second shift. The children were observed eating snack, listening to a story, interacting with the provider, and playing freely with toys. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, capacity, health and safety requirements, equipment and furnishings, and program records. The provider does not transport children; therefore, this requirement was not monitored. No enrolled children have medications or medical care plans, so these requirements were not monitored. Children 12 months or younger are not enrolled; therefore, safe sleep requirements were not monitored. The provider verified that all the information on the FCCH Verification of Required Information for Operator and Additional Caregivers form was correct. INDOOR/OUTDOOR ENVIRONMENT Inside the home, children are cared for in a converted garage. A variety of items for learning through play are available, including manipulatives, blocks, books, and home living props. The floor is covered with soft foam tiles and an area rug. There is a changing table within the children’s space, and a bathroom is located in a hallway just outside the play space. Portable cribs and mats are used for rest time, and high chairs as well as a table and chairs are available for eating. The outdoor play area is a fenced backyard with a large deck. The provider states that she brings balls, hula hoops, and riding toys outside during outdoor play time. VIOLATIONS The following violation was observed and documented during today’s visit: Violation Number Comment Rule 911 Copy of each child's immunization record, including the operator's own preschool child(ren), was not on file. One child was missing an immunization record. GS 110-91(1); .1721(a)(2) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by July 31, 2025. The letter must address each violation and explain specifically when and how it was corrected, as well as how you will prevent the same violations from happening in the future. Please send this to mandy.annunziata@dhhs.nc.gov and include the following information: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today’s visit, the facility’s compliance history score was 98%. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. Failure to maintain compliance may result in an administrative action. The facility was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact the center’s overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your program. TECHNICAL ASSISTANCE -Record keeping is an important piece of a FCCH operation. Today, a child was missing an immunization record, which is required within 30 days of enrollment. You stated that you thought it was in the file. To ensure each child’s file is complete, you may reference the children’s file checklist available on the DCDEE website under the “Provider Documents and Forms” tab. Requiring all forms at the time of enrollment will help you stay in compliance. CONSULTATION -You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please review and update your EPR plan yearly. -Criminal Backgrounds Checks for the provider and all household members 16 years of age and older should be completed every five years. -Because the children in your care are young and still learning how to behave appropriately, I would recommend teaching skills and using redirection rather than time-out when children misbehave. Be specific and show them what you want them to do instead of saying general things like, “Be nice.” Time away from the group should be reserved for when children are out of control or hurting others, and they need time and space to calm down. TRAINING REMINDERS -Training from July 22, 2024 to July 17, 2025 was reviewed today and documented on the Verification of Required Information for Operator and Additional Caregivers form. Five remaining training hours should be completed and documentation emailed to me by July 22, 2025. -“Recognizing and Responding to Suspicions of Child Maltreatment” (part of the Health and Safety Training) should be renewed every five years. Your last documented training took place in 2019, so you are due for renewal. This training can be found on the Positive Child Alliance NC website: https://preventchildabusenc.org/online-trainings/ -CPR and First Aid must be renewed every two years. Your CPR and First Aid certification expires on 4/3/26. -ITS-SIDS training must be renewed every three years. Your training expires on 4/6/27. -Health and safety training must be renewed every five years. Your training is valid until 2027. RATED LICENSE STATUS The NC State Legislature has extended “hold harmless” status until new rated licensing guidelines are in place, and you may opt to keep your current license and delay your rated license assessment until the new guidelines are adopted. Please use this time to update your WORKS account with your education information. You can visit the North Carolina Rated License Project website (https://www.ncrlap.org) for resources regarding the rated license assessment process and the FCCH self-study. Go to https://ncchildcare.ncdhhs.gov/, and click on the “What’s New” tab for all agency updates, including QRIS Modernization. I recommend exploring the new pathway options to obtain a rated license. It is important to note that the new QRIS rules are not yet effective. However, the Division is preparing for a gradual transition to the new requirements. Resources, forms, and training will be provided to support you in preparing for new options to earn your Two- through Five-Star Rated License. Upon arrival I was greeted by the provider, Lolita. You assisted me with the visit. I completed a general walk through of the children’s space, kitchen, bathroom, and outdoor play area. At the time of the visit, there were a total of four (4) children present, ranging from age one (1) year to four (4) years old. I observed the children interacting with the provider, playing in centers and completing routine tasks. LICENSE STATUS The Family Child Care Home (FCCH) operates with a Four (4) star license issued October 18, 2019, earning seven (7) points in Staff Education, two (2) points in Program Standards and one (1) Quality point serving no more than two (2) infants under age one (1). The facility is approved for 1st, 2nd and 3rd shifts. INSPECTIONS *The last fire drill was conducted 2/16/24. *The last emergency drill was conducted 1/19/24. *The last playground inspection was completed 2/16/24. MONITORING During today’s visit, a partial assessment was conducted, monitoring all spaces occupied by the children including the outside play environment. The providers qualification letter was monitored as well as CPR/First Aid, SIDS. Three new children enrolled records were monitored. All spaces were monitored for supervision, child capacity, materials and equipment. The program records were reviewed for current inspections. No children enrolled have medications or medical care plans, so these requirements were not monitored. The provider does have a child under 12 months enrolled, therefore safe sleep requirements were monitored. Transportation is not provided; therefore, these requirements were not monitored. The following violation was observed and documented during today’s visit. A child enrolled on 3/10/24 and a drop in child did not have immunization records on file during today's visit. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE During today’s visit, the information below was shared with you to maintain quality within your program and to prevent further violations. CHILDREN FILES I would recommend using the children file checklist to make sure that all necessary documents are on file on or before start date. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. RATED LICENSE STATUS On Monday June 12, 2023, the hold harmless legislation was extended and signed into law. All facilities are allowed to remain at their current star level without reassessment until June 30, 2024. Your facility has been determined to be in Cohort 2. Your reassessment year will be between July 1, 2025, and June 30, 2026. I discussed with the provider to consider going through FCCERS-R assessment during the prep year which starts July 1, 2024. I also discussed that if the FCCERS-R assessment is completed during the prep year and the scores are liked they can be used immediately upon the administrators request or be held until the actual assessment year. HEALTH CONSULTANT Ellie Morrise is the new Child Care Health Consultant. If you should have any health related questions, please reach out to her at morrise@email.unc.edu and/or 919.809.9645. HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety trainings must be renewed at least every five (5) years. If you completed the trainings in 2018, you should begin to retake the training now. CLEAN CLASSROOMS FOR CAROLINA KIDS I encourage you to enroll in the Clean Classrooms for Carolina Kids program to identify and eliminate exposure to lead and asbestos hazards in building infrastructure. This program is an expansion of Clean Water for Carolina Kids. Facilities are now required to meet rule requirements NC 10A NCAC 41C. 1001-1007, NC 154A NCAC 18A .2816. Clean Classrooms for Carolina Kids is designed to ensure facilities meet all requirements, receive direct support throughout the participation process, and have access to communication resources. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years.
