The family childcare provider, Felecia Powell, assisted me with today’s visit. Upon arrival, you greeted me and welcomed me into your program. Four (4) children were present during today’s visit. I conducted a general walk-through of the childcare spaces. I observed the children wrapping up lunch, having small talk with their teacher about a story they came up with about Aniya, exploring their environment, and preparing for a nap in their indoor learning environment. RATED LICENSE STATUS The program holds a 5-star-rated license issued on 2/14/24. The program received 7 points in Program Standards, 7 points in Staff Education, and 1 quality point through having a BS/BA or higher in Early Childhood/Child Development. INSPECTIONS The last fire drill was conducted on 6/2/25. The last emergency drill was conducted on 6/2/25. The last playground inspection was conducted on 6/2/25. During this visit, a full assessment of Child Care Requirements was conducted with the following exceptions. I was informed that you do not administer medication, have any children with allergies, and do not transport at this time, therefore, these items were not monitored during today’s visit. The following violations were observed and recorded during today's visit. Violation Number Comment Rule 912 Written feeding schedule was not on file for each child under 15 months of age and/or was not updated regularly to reflect changes in the child's needs. A child that was six (6) months did not have a feeding schedule available for review during today's visit. .1706(i) 925 Signed and dated statement which attests that a copy of the discipline policy was given to and discussed with a parent/guardian of each child prior to enrollment was not on file in the home. A child did not have a sign and dated discipline policy available for review during today's visit. G.S.110-91(10); .1727(a)&(b) In order to comply with the NC Laws & Rules, any violations cited today must be corrected immediately. A compliance letter should be sent to me by June 17, 2025. The letter should address each violation, and explain how and when it has been corrected. Please send the letter to shannel.hall@dhhs.nc.gov and include the following: -Facility name -Facility ID number -Each item number TECHNICAL ASSISTANCE The following items were discussed to help you prevent a reoccurrence of the violations observed during today's visit. I would suggest using the children’s checklist to chart what each child has in their folder. When the children enroll, I would double check the file as parents bring in their paperwork. COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 93%. It will be updated after today's visit, and you will be informed of the new score. CONSULTATION Some providers are experiencing delays in scheduling their fingerprinting appointments. Criminal record checks can be completed up to six months prior to the expiration date. We are encouraging all staff to begin their renewal process early to avoid lapses in their qualification. ANNUAL IN-SERVICE TRAINING The in-service hours were monitored for the annual year June 5, 2024 – June 3, 2025. HEALTH AND SAFETY TRAININGS Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings dare provided free of charge. You should use your NCID username and password to access the Moodle portal. As per Child Care Rule .1103(b) the Health and Safety trainings must be renewed at least every 5 years. If you completed the trainings in 2017, you should begin to retake the training. I reviewed the visit summary with the provider, Felecia Powell, and a copy was left with her at the conclusion of the visit. Mandy Annunziato, Child Care Consultant accompanied me on the visit today. Upon arrival we were greeted by Ms. Powell. We completed a general walk through of the children's space, kitchen, bathroom, and outdoor play area. The children were observed down for nap initially then engaged in free play activities. The provider’s daughter was present visiting from Florida. CURRENT LICENSE STATUS Currently this family child care home operates with a five-star license issued February 14, 2024, earning seven (7) points in Staff Education, seven (7) points in Program Standards points and one (1) Quality Point. INSPECTIONS *An initial visit due to a change of location was conducted on February 13, 2024. *The last fire drill and playground inspections were both completed on June 1, 2024. *The last shelter-in-place drill was conducted in February 2024. INDOOR ENVIRONMENT *All activities take place in the child care space, excluding meals, which takes place in the kitchen. In the children’s space, there are a variety of materials that are age appropriate for the children to explore throughout the day. The bathroom is accessible to those that are potty trained. The provider stated that diaper changing takes place in the bathroom as well. *The children are prevented access to the bedrooms by closed doors. *The outdoor play area is in the front yard of the home and is fenced. There are no other buildings on the property. -Medication is not administered to the children. -No incident/accident reports have been written since the last visit. -There are no children with allergies. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. In the bathroom disinfecting wipes and two bottles of air freshener was not in locked storage. .1719 (a)(7) 919 Accurate records were not maintained for all staff and children. The two drop-in children did not have the following on file: an application, receipt of discipline policy, receipt of shaken baby syndrome policy, receipt of summary of the law, receipt of written plan of care on file, feeding schedule, receipt of operational policies or transportation authorization. G.S. 110-91(9) 1544 The child care operator did not comply with all State laws, federal laws and/or local ordinances that pertain to child health, safety, and welfare, as required by General Statute 110-91. The registration for the vehicle expired on 4/30/24. GS 110-91 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. The last documented shelter-in-place drill was conducted in February 2024. .1719(a )(16) & .1721(e )(7) 1922 Plastic bags, toys, and toy parts small enough to be swallowed, including but not limited to materials that can be easily torn apart such as foam rubber and styrofoam, were accessible to children under the age of three. Plastic bags storing toys and sponges were accessible to the children under the age of three. .1719(a)(18) 1986 Breast milk, formula, and other bottled beverages sent from home were not fully prepared, dated, and labeled with individual child's names. One bottle and one sippy cup was not dated and labeled with the child's name. .1706(l) In order to comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be sent to me by June 19, 2024. The letter must address each violation and explain how it has been corrected. Please send this to Kia.Reid@dhhs.nc.gov and include the following: -Facility name -Facility ID number -Each item number TECHNICAL ASSISTANCE *Plastic bags, material made of foam, and materials small enough to be swallowed must be kept up out of the children’s reach. The child care space should be checked daily to ensure all materials are age appropriate for the children in care. *All bottles must be labeled, with the child’s name and current date. The best practice would be to check the bottles before parents depart in the morning and have them label/date if necessary. If not, then you as the provider must label the bottles. *Two children were present for drop-in on a trial basis before enrolling in the program. The children did not have any paperwork on file. I explained to Ms. Hopkins that all paperwork with the exception of health assessments and immunizations are due on the 1st day the child is present. You should always have emergency information on file for the children. CONSULTATION *You must complete your EPR Plan by 2/13/25. Please access North Carolina Risk Management Portal rmp.nc.gov/portal/ to update your information. *I explained to Ms. Powell that her daughter would need to complete a criminal background check, TB test, ITS-SIDS training, CPR and First Aid training if she wanted to assist with caregiving. REMINDERS The following requirements will come due in the near future; please make arrangements to complete the items before the deadline. *You must complete your health questionnaire by 1/2/25. *Please continue to check the Division of Child Development and Early Education website: ncchildcare.ncdhhs.gov frequently and click the “what’s new” tab to stay informed of all updated information available to you.
