    Name of Operation: HILLANDALE SAC CLUB Facility ID: 32001477 Consultant: TONI FULLER Operation Type: Center Case Number: Visit Date: 5/15/2025 Number Present: 43 Completed Date: 5/15/2025 Age: From 5 To 12 Total Minutes: 120 Time In: 01:45 PM Time Out: 03:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced Upon arrival I was greeted by the Tuesday/Thursday site manager. The purpose of today’s unannounced visit was to monitor applicable child care requirements during a Routine Unannounced visit. The last Annual Compliance visit was conducted on 9/6/24. This facility operates with a five-star license, which was issued on 9/7/17. This program operates on a traditional school schedule. The compliance history for the previous eighteen-month timeframe was 96%. Today’s visit was facilitated by the Tuesday/Thursday site manager, who was also a group leader. OBSERVATIONS: During today’s visit the indoor primary space (cafeteria and gymnasium) and the designated outdoor spaces were monitored. The children were operating in three groups. Upon dismissal at the end of the school operating day, one group of children assembled in the school art room; one group assembled in the gymnasium; and one group gathered in the cafeteria for attendance. I observed the children completing homework assignments, having a group discussion in the art room, and enjoying the outdoor environment. The following activity areas were set up in the cafeteria. Books, cozy area, math, science, music, tools, language, writing, social studies, housekeeping, dramatic play, and art. The required information was posted on the Parent Information Board to include current activity plans and current snack menus. The theme for the week was May Showers. OUTDOOR ENVIRONMENT: I observed that the children playing on the playground at the back of the school building and the kindergarten age children played on the playground at the front entrance to the school. This facility has the following restriction on their license: PLAYGROUND AREA DOES NOT MEET CHILD CARE STANDARDS. Based on this restriction the outdoor play area was monitored for health and safety concerns only. CHILDREN’S RECORDS: Children’s records were not reviewed today. STAFF FILES: Today I reviewed staff records to verify current Criminal Record Check clearance, CPR training, and First Aid training. INSPECTIONS: The last annual fire inspection was conducted on 2/11/25. The last sanitation inspection was conducted on 3/13/25. The last fire drill was conducted on 4/14/25. The last documented lockdown drill was conducted on 4/14/25. The last documented outdoor inspection was completed on 4/11/25. The following violations were cited today: Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. The climber on the front playground was damaged. A safety violation, regarding this climber, was cited during the 9/6/24 Annual Compliance visit. Today I observed that the climber had not been repaired, and the children were playing on it. This is a repeat violation. G.S. 110-91(6); .0601(b) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. One child had Albuterol for asthma. The permission to administer the Albuterol was dated 8/26/24, and the permission form needed to be filled out more completely. .0803(6)(a-i); .0803(7)(a-g); .0803(8)(a-d) Violations must be corrected immediately. Please send me a signed Corrective Action letter by 5/29/24, stating how the violation cited today has been corrected. TECHNICAL ASSISTANCE was provided in the following areas: OUTDOOR ENVIRONMENT: During the 9/6/24 Annual Compliance visit, a safety violation was cited regarding the stationary climber on the front playground. In response to this violation, the program administrator stated in the Corrective Action letter for the 9/6/24 visit that the climber would not be used until it was repaired. During today’s visit I observed that the climber had not been repaired. The climber was not inaccessible to the children, and I observed the afterschool children playing on the climber. It is important that I am contacted about this matter to avoid a Written Warning from DCDEE. MEDICATION STORAGE: Today medicine was stored in a box with a locking mechanism in the site manager’s office. The site manager stated that children were not permitted in the office, and the office door remained lock when unoccupied. I stated that the locking mechanism on the medicine box should be used at all times, because the lock on the site manager’s office was not working properly today. MEDICATION ADMINISTRATION: NC Child Care Rule 10NCAC 09 .0803(6) was reviewed with the site manager, explaining that parents could grant permission to administer medication for a chronic medical condition for a timeframe not to exceed six months. After six months, an updated medical permission was needed. RATED LICENSE: Rated License reassessments are not required currently. Hold harmless has been extended until the new QRIS process has been developed and ready for implementation (SB 425). I arrived prior to the end of the school operating day. The purpose of today’s unannounced visit was to monitor applicable child care requirements during the Annual Compliance visit. The last Annual Compliance visit was conducted on 9/14/23. This facility operates with a four-star license, which was issued on 9/7/17. This facility operates on a traditional school schedule. The 2024/2025 school year began on 8/19/24. The compliance history for the previous eighteen-month timeframe was 100%. During today’s visit the indoor primary space (cafeteria and gymnasium), the school art classroom (secondary space), and two designated outdoor play areas were monitored. OUTDOOR ENVIRONMENT: This facility has the following restriction on their license: PLAYGROUND AREA DOES NOT MEET CHILD CARE STANDARDS. Based on this restriction the outdoor play area was monitored for health and safety concerns only. OBSERVATIONS: I observed the children arriving for afterschool care. The kindergarten age group went to the Art room for a short circle time and for attendance to be taken. Grades 1 and 2 went to the cafeteria. Grades 3 – 5 went to the gymnasium. The site manager stated that the three groups rotated, so each group could spend time in the cafeteria with the activity areas, in the gymnasium, in the art room, and outside daily. I observed the children having afternoon snack and engaged in the activity areas in the cafeteria. The following activity areas were set up in the cafeteria: music, arts/crafts, dramatic play, cozy area with books, arts/crafts, housekeeping, math, dramatic play, science, and manipulatives. The required information was posted on the Parent Information Board to include current activity plans and current snack menus. Food allergy information was posted. CHILDREN’S RECORDS: Ten children’s files were reviewed for the required health and safety requirements. STAFF FILES: Prior to today’s visit I received the staff/training worksheet. During the visit it was determined by the site manager that I had not received the staff/training worksheet that provided the most current information. There were no new staff at this location. There was staff that had transferred to this location from another Durham SAC CLUB location. During the visit I reviewed the staff files for on-site requirements. Yearly in-service training requirements were tallied for two staff. INSPECTIONS: The last annual fire inspection was conducted on 2/12/24. The last sanitation inspection was conducted on 3/20/24. The last fire drill was conducted on 8/26/24. The last documented shelter-in-place drill was conducted on 8/27/24. The last documented outdoor inspection was completed on 9/4/24. The following violations were cited today: Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. The multi-purpose climber on the front playground had a circular ladder for climbing. The steps on the ladder were wire with a thick rubber coating. The coating was damaged and hanging down on several steps, diminmized safety in using the steps to climb to the top of the structure. G.S. 110-91(6); .0601(b) 853 Incident logs were not completed and maintained as required. Incident reports were in children's files, but no incident log was being maintained. .0802(g)(1-6) Violations must be corrected immediately. Please send me a signed Corrective Action letter by 9/20/24, stating how the violation cited today has been corrected. TECHNICAL ASSISTANCE was provided in the following topic areas: CRIMINAL RECORD CHECK CLEARANCE: I reminded the site manager that one group leader needed to complete the Criminal Record Check re-qualification process before 10/17/24, and the site manager needed to complete the process before 11/13/24. STAFF FILES: I explained the importance of maintaining verification of the completion of the required Health and Safety trainings for each five-year timeframe in staff files. RATED LICENSE: Rated License reassessments are not required at this time. Hold harmless has been extended until the new QRIS process has been developed and ready for implementation (SB 425).
