The purpose of today’s unannounced visit was to monitor applicable child care requirements for your Annual Compliance visit to include the Rated License assessment requirements. NC pre-K requirements were also monitored today. The last Annual Compliance visit was conducted on 9/3/25. This center operates with a five-star license, which was issued 1/30/19. COMPLIANCE HISTORY: The compliance history for this center for the previous eighteen-month timeframe was 97%. During today’s visit the licensed classroom (B3) was monitored, along with the designated outdoor environment. OBSERVATIONS: The teacher in classroom B3 was absent today. The visit was facilitated by the assistant teacher and reviewed with the school principal. A substitute teacher was assisting in the classroom today. I observed the children eating lunch and enjoying the outdoor environment. The required information was posted on the Parent Information Board to include daily schedule, current activities, current menus and summary of NC Child Care Laws. The food allergy list was posted. The teacher stated that none of the children were taking medication. NC PRE-K MONITORING: NC pre-K operating hours: 8:45 am to 2:45 pm, Monday through Friday. STAFF/CHILD RATIOS: This center maintains 1/9 staff/child ratios. PROGRAM ATTENDANCE: Of the 17 children enrolled in the licensed classroom, eight children were NC Pre-K. Fourteen of the seventeen children were present today. NURTITION: Current breakfast, snack, and lunch menus were posted. Lunch today consisted of baked chicken, mashed potatoes, string beans, fruit cobbler, and milk. Three children brought their lunches from home. The children’s files contained a signed “opted-out” form. The assistant teacher stated that the teachers still checked the lunches and offered additional food if needed. CURRICULUM: This center implements Teaching Strategies Gold. STAFF RECORDS: Staff files were reviewed today to include in-service training hour requirements. NC PRE-K STAFF: (Admin, LT and T. Qualification) The lead teachers and teachers are registered in the NC PreK plan and in WORKS. CHILDREN’S RECORDS: Eight children’s records were reviewed today. Five of the eight files reviewed today were for NC Pre-K children. INSPECTIONS: The last documented fire drill was conducted on 4/30/26. The last documented lockdown drill was conducted on __ The last documented playground inspection was completed on 5/4/26. The following violations were cited today: Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. The substitute staff, who was working in the licensed classroom today, did not have the required state Criminal Record Check clearance to work in a licensed classroom. G.S. 110-90.2(b) 1768 The health assessment did not include a hearing screening. Of the eight children's files reviewed today, two children did not complete a hearing screening. .3005 (a)(4) All violations must be corrected immediately. Please send me a signed Corrective Action letter by May 19, 20026, stating how each violation has been corrected. TECHNICAL ASSISTANCE was provided today in the following areas: CRIMINAL RECORD CHECK: The substitute teacher had Criminal Record Check clearance through Durham Public Schools. Today I explained that the substitute needed to complete the state Criminal Record Check process before substituting in a licensed classroom. HEARING ASSESSMENTS: I suggested that the teacher contact the coordinator of Durham Public School Pre-K program about extended follow-up procedures when hearing assessments are not successful. RATED LICENSE: This facility chose to complete the Rated License assessment process, using Pathway 1. The assistant teacher stated that the “window” for their ECERS-3 assessment started next week. The purpose of today’s unannounced visit was to monitor applicable child care requirements during your Annual Compliance visit. The last Annual Compliance visit was conducted on 9/14/23. This school operates on a traditional school schedule. The 2024/2025 school year began for the Pre-K children on 8/29/24. I stated that since school had not been in session for 30 days, I would monitor NC Pre-K requirements during the next visit. This center operates with a five-star license, which was issued on 1/30/19. The licensed classroom (space B-3) was monitored today, along with the outdoor environment. All the required information was posted for easy reference by parents. There were fourteen children enrolled in classroom B-3. Eleven of the children were present today. The teacher stated that she was just receiving Pre-K identifying information on the children. The teacher stated that she thought six of the fourteen children were NC Pre-K. OBSERVATIONS: I observed nine of the children walking to the cafeteria with a teacher to get their lunch. The nine children returned to the classroom to eat their lunch, after they washed their hands. The teacher assistant stayed in the classroom with two children, who brought their lunches from home. Lunch today consisted of Doritos with taco meat sauce, cheese, mixed fruit cup, broccoli, and milk. The teacher stated that the children did not eat in the cafeteria, based on the height of the seating that was available in the cafeteria. I later observed the children enjoying the outdoor environment. INSPECTIONS: The last documented annual fire inspection was completed on 11/2/23. The last documented sanitation inspection was conducted on 4/15/24. The last documented fire drill was conducted on 8/30/24. The last documented playground inspection was completed on 8/23/24. A shelter-in-place/lockdown drill had not been conducted yet for the 2024/2025 school year. CHILDREN’S RECORDS: Five children’s records were reviewed today. STAFF FILES: Staff files were reviewed during the visit and in-service training hours were tallied for trainings completed since the 9/14/23 Annual Compliance visit. The teacher was aware that she needed to complete the Criminal Record Check re-qualification process prior to 10/9/24. COMPLIANCE HISTORY: The compliance history for this center for the previous eighteen-month timeframe was 90%. The following violations were cited today: Violation Number Comment Rule 503 Food brought from home, did not meet the nutritional requirements and necessary supplements were not provided by center. Two children brought their lunches from home. One child had cheese puffs, applesauce, juicebox, and strawberries in her lunchbox. The second child had cheese curls, pita bread, cookies, and water in her lunchbox. The lunches were not supplemented today. 10A NCAC 09 .0901(c) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). Criminal Record Check clearance expired for the school administrator on 8/27/24. G.S. 110-90.2(b) & .2703(n)&(o) 1791 The child care provider did not provide the required beverage(s). During today's visit, one child drank juice for lunch; one child drank water; and six children drank chocolate milk. .0901(e)(1-7) All violations must be corrected immediately. Please send me a signed Corrective Action letter by 9/23/24, stating how each violation has been corrected. Today’s visit was reviewed with the teacher and the school administrator. TECHNICAL ASSISTANCE was provided today in the following areas: CRIMINAL RECORD CHECK: The school administrator did not realize that his Criminal Record clearance had expired. He began the online process while I was present in his office. NUTRITION: I explained that lunches brought from home should be checked for the child care nutrition requirements, prior to walking to cafeteria. The teacher could bring back supplemental food from the cafeteria to add to the lunches from home, if needed. I also explained that preschool age children should drink unflavored milk. While reviewing the visit with the school administrator, I explained the “opted-out form”. RATED LICENSE: Please know that the “hold harmless” status for Rated License reassessment has been extended until the new QRIS guidelines are ready for implementation.
