Upon arrival, I was greeted by the provider, Ethel Burton, who assisted me during the visit. LICENSE STATUS The family child care home currently operates with a Three Star License issued on November 20, 2017. The program received four points for Program Standards, three points for Staff Education, and one quality point. INSPECTIONS/DRILLS STATUS -The last documented outdoor play area inspection was completed on 10/3/24. -The last documented fire drill was conducted on 10/3/24. -The last documented lockdown/shelter-in-place drill was conducted on 8/12/24. -The last annual compliance visit was conducted on 5/14/24. MONITORING I completed a walk-through of the home, visiting the caregiving space and outdoor play area. Four children were present, ranging in age from 10 months to four years. The children were napping during the visit. A partial assessment of child care requirements was conducted. I monitored all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records. One newly enrolled child’s information also was reviewed. No enrolled children have medications or medical care plans, so these requirements were not monitored. One child under 12 months of age is enrolled; therefore, safe sleep requirements were monitored and found in compliance. INDOOR/OUTDOOR SPACE Children are cared for in a room by the home’s entrance, and a variety of materials are available for learning through play, including blocks, manipulatives, books, and puppets. A bathroom is located down the hall for children who are toilet trained. Cots and pack-n-plays are used for rest time. The backyard is fenced with a variety of toys for gross motor play, including a small slide and riding toys. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 508 Operator did not successfully complete a first aid course as referenced in Rule.1702(b)(2) First aid training was not renewed on or before the expiration of the certification. The provider's First Aid certification expired on 10/22/24. .1703(a)(2) 511 Operator did not successfully complete a CPR course as referenced in Rule.1702(b)(2) CPR training was not renewed on or before the expiration of the certification. The provider's CPR training expired on 10/22/24. .1703(a)(3) 1705 Summary of the NC Child Care Law was not posted with the facility's license in a prominent place in the home. The provider had not posted the Summary of NC Child Care Law in the child care space. GS 110-102 In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by December 5, 2024. The letter must address each violation and explain how it has been corrected. Please send this to mandy.annunziata@dhhs.nc.gov and include the following information: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today’s visit, the FCCH’s compliance history score was 90%. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. Failure to maintain compliance may result in an administrative action. The FCCH was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact the provider’s overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your facility. TECHNICAL ASSISTANCE -CPR and First Aid must be renewed every two years. Your certification expired on 10/22/24. You stated that you thought you had renewed it, but you were unable to find the certificate. If you locate it, please send it to me. If you need to retake the class, please visit the DCDEE website for a list of approved training agencies: https://ncchildcare.ncdhhs.gov/Provider/Training-and-Professional-Development/Health-and-Safety-Trainings/Be-a-Smart-Consumer-of-First-Aid-and-CPR-Training -Providers are required to post a copy of the Summary of NC Child Care Law. Today, your posting was missing. You can download and print a copy from the DCDEE website under “Provider Documents and Forms.” Please post it immediately. CONSULTATION -Criminal Backgrounds Checks for all providers should be completed every five years. Your qualification letter will expire on 2/13/25. You stated that you had tried to renew your background check recently but never received a new qualification letter. Please call the Division’s Criminal Background Check Unit at 919-814-6401 during business hours for assistance. -Be sure to conduct a lockdown/shelter-in-place drill as well as an outdoor play inspection by the end of November. -The Clean Classrooms for Carolina Kids Program provides free facility-wide testing for lead in water and inspections for asbestos and lead-based paint hazards without any out-of-pocket costs. Visit www.cleanwaterforuskids.org/carolina to enroll. -You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please review and update your EPR plan yearly. TRAINING REMINDERS -ITS-SIDS training must be renewed every three years. Your certification is valid until 5/15/27. -Health and Safety Training must be renewed every five years. You can find this training in Moodle on the DCDEE website. Also, “Recognizing and Responding to Suspicions of Child Maltreatment” (part of the Health and Safety Training) should be renewed every five years. A checklist was used to note the requirements monitored today. The Family Child Care Home Provider, Ethel Burton, was present and assisted me during today’s visit. I completed a general walk-through of the children’s space, outside play area, kitchen area and bathroom. At the time of the visit, there were three (3) children present aging in range from infant to three (3) years old. The children were observed having free play, and completing routine tasks. LICENSE STATUS Currently this program operates with a Three-Star License issued November 20, 2017. The program earned three (3) points in staff education, four (4) points in program standards, and one (1) quality point. MONITORING During today’s visit, a full assessment was conducted monitoring all spaces occupied by the children, including the outdoor play environment. The provider’s and children files were reviewed. All spaces were monitored for supervision, child capacity, materials, and equipment. The provider does have a child under 12 months enrolled at this time, therefore safe sleep requirements were monitored. No children enrolled have medications or medical care plans at this time, so these requirements were not monitored. Transportation is not provided; therefore, these requirements were not monitored. The FCCH provider verified the information on the Verification of required information for operator and additional caregiver’s form to be accurate. --PROGRAM RECORDS *The last documented fire drill was conducted 4/19/24. *The last documented shelter-in-place or lockdown drill was conducted 5/1/24. *The last documented playground inspection was conducted 4/19/24. The following violations were observed and cited on today’s visit: Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. GermX, and Germicidal disposable wipes were stored in an unlocked cabinet in the bathroom used by the children in care. Glad air freshener was stored in the bathroom in reach of the children. .1719 (a)(7) 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. A child enrolled on 2/1/24 did not have a medical health assessment on file within 30 days of enrollment. GS 110-91(1); 10A NCAC 09.1721(a)(1) 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. Accurate daily attendance was not recorded. During today's visit I monitored only one child signed in and only one child signed in and out for yesterday, 5/13/24. .1721(e)(6) 1804 Safe sleep policy and poster about safe sleep practices was not posted in a prominent place in the infant sleeping room or area where parents and caregivers are able to view daily. Safe sleep policy was not posted in a prominent place in the area of the sleeping infant. .1724(b) 1821 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of 30 days. Documentation of visually checking on the sleeping infant was not consistently documented and was not maintained for 30 days. .1724(a)(8)&(f) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. TECHNICAL ASSISTANCE: During today’s visit, the information below was shared with you to maintain quality within your program and to prevent further violations. HAZARDOUS MATERIALS/CLEANING SUPPLIES Hazardous Materials and cleaning supplies were stored in an unlocked cabinet under the sink in the bathroom used by the children in care. I would recommend storing all hazardous materials and cleaning supplies in a locked storage. Hazardous Materials/cleaning supplies and items that state ‘keep out of reach of children,’ could cause severe situations in the health of children. SAFE SLEEP POLICY/SAFE SLEEP CHECK The safe sleep policy was not posted in a prominent space. The safe sleep check documentation was not completed daily. I was unable to monitor thirty days of safe sleep check documentation. I would recommend posting the safe sleep policy near the sleep area of the infant along with the documentation of the safe sleep. I would also recommend using a timer to ensure that you are checking the sleeping infant every 15 minutes. Documentation of a sleeping infant is important for the safety of the sleeping child. I provided the safe sleep policy form during today’s visit. ATTENDANCE Attendance was not observed being consistently documented. During today’s visit only one child was recorded in attendance. I would recommend signing the children in at the time of arrival. Attendance is very important to document, especially if an emergency happens. CHILDREN FILES The medical assessment of a child enrolled February 1, 2024 did not have a medical on file. I would recommend that all forms are collected on or before enrollment to ensure you have collected all forms. I would also recommend that you use the Children’s Checklist. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. ONGOING TRAINING You are required to have ten (10) ongoing training hours a year. You had two (2) training hours carried over from last year. You have completed 7.5 hours as of 5/14/24. Please submit .5 training hours to me on or before May 31, 2024. RATED LICENSE STATUS On Monday June 12, 2023, the hold harmless legislation was extended and signed into law. All facilities are allowed to remain at their current star level without reassessment until June 30, 2024. Your facility has been determined to be in Cohort 1. Your reassessment year will be between July 1, 2024, and June 30, 2025. I discussed with the provider to consider going through FCCERS-R assessment in the prep year. I also discussed that if the FCCERS-R assessment is completed during the prep year and the scores are liked they can be used immediately upon the administrators request or be held until the actual assessment year. HEALTH CONSULTANT Ellie Morrise is the new Child Care Health Consultant. If you should have any health related questions, please reach out to her at morrise@email.unc.edu and/or 919.809.9645. HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge.
