    Name of Operation: GRAY'S SMALL WONDERS DAYCARE Facility ID: 32001440 Consultant: KIA REID Operation Type: Family CC Home Case Number: Visit Date: 11/19/2025 Number Present: 5 Completed Date: 11/19/2025 Age: From 1 To 3 Total Minutes: 120 Time In: 10:00 AM Time Out: 12:00 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for an annual compliance visit. Barbara Gray, owner/operator, assisted me with the visit. Your program currently operates with a five-star license, issued October 12, 2017, earning 7 points in the education component, 6 points in the program standards component and 1 quality point for serving no more than two infants under age one. The program is approved for daytime and overnight care. The last annual compliance visit was conducted March 6, 2025. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven percent as of November 13, 2025. I visited each indoor and outdoor space used by the children. The five preschool children present were observed in circle time, center activities, playing a matching game, and personal care routines. Four violations were observed today. The violations are as follows: Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. Microban disinfectant was on a shelf by the diaper changing table not in locked storage. .1719 (a)(7) 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. The attendance records were not available for review. .1721(e)(6) 1920 Indoor and outdoor area was not checked daily for debris, vandalism, broken equipment, and animal waste. There was animal waste, pieces of wood, extension cord and water hose in the outdoor play area. .1719(a)(17) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violations documented must be corrected immediately. On or before December 3, 2025. I must receive a written, dated, and signed compliance letter that describes accurately and in detail how the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. Email the information to: Kia.Reid@dhhs.nc.gov If you email the compliance letter, it must be sent from the email address registered with the DCDEE (this serves as your signature) and the following information must be included: name, position, facility name, and facility ID number. An example is: TECHNICAL ASSISTANCE -Remember to check the areas used by the children daily prior to their arrival, removing any potential hazards. Safety is a priority when caring for young children. QRIS REQUIREMENTS During today’s visit we discussed the new QRIS pathways and you were given an opportunity to ask questions. You should visit the DCDEE website to review the new Pathways to the Stars. We are going away from a point-based system allowing you to have three different pathways to achieve stars. You will find the following resources in the QRIS section. -Pathways to the Stars Family Child Care Home Session Slide Deck -Pathways to the Stars Questions and Answers -Frequently asked questions As of February 1, 2025, the FCCERS-3 will be used for the assessments. Access www.ncrlap.org to participate in webinar sessions that will focus on the changes. I stated that the webinars could be used to meet yearly in-service training hour requirements. I explained to Ms. Gray that I would contact her in January 2026 to schedule a Technical Assistance visit to discuss the pathway that will be chosen. *Please check the Division of Child Development and Early Education website: ncchildcare.ncdhhs.gov frequently and click the “what’s new” tab to stay informed of all updates available to you. CHILD CARE RULES Child Care Rules have been updated as of 7/1/25. Please visit the DCDEE website to review and download a copy. The provider, Barbara Gray, was present and assisted me when needed. The visit summary was reviewed with Ms. Gray and a copy was given to her at the conclusion of the visit. Upon arrival, I was greeted by Ms. Gray. I completed a general walk through of the indoor and outdoor environments. The four (4) preschool were observed down for nap. CURRENT LICENSE STATUS Currently this family child care home (fcch) operates with a five-star license issued October 12, 2017, earning seven (7) points in staff education, six (6) points in program standards and one (1) quality point. INSPECTIONS *The last annual compliance visit was conducted on March 6, 2025. *The last documented fire drill and outdoor inspection were both completed on March 17, 2025. *The last documented lockdown drill was conducted on March 17, 2025. DESIGNATED CHILDREN’S SPACE In the children’s space, there are a variety of materials for the children to explore throughout the day. The bathroom is accessible to those who are potty training, and a changing table is available for those in diapers. *The outdoor play area is in the back yard of the home and is fenced. Two sheds are on the property and are kept locked. *The provider informed me of the following: -She is not administering medication to any of the children. -Transportation is not provided currently. During this visit, a partial assessment of Child Care Requirements was conducted. The monitoring included Supervision, Capacity, CPR/FA, ITS-SIDS training, Criminal Background Checks, General Health, Administering of Medication, Storage of Hazardous Substances, Storage of Medication, General Safety, Discipline, Adequate/Approved Space, General Licensing Requirements, Program Records, License Posted, Fire Safety, and Sanitation. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 810 Refrigerator was not maintained at a temperature of 45 degrees Fahrenheit or below and/or was not monitored by a refrigerator thermometer. The thermometer in the refrigerator was broken. 10A NCAC 09 .1725(a)(10) 908 Health questionnaire was not completed annually. The health questionnaire expired on 4/22/25. .1703(a)(1) 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. The attendance records from March-June were not available for review. .1721(e)(6) In order to comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be sent to me by June 30, 2025. The letter must address each violation and explain how it has been corrected.
