You Deborah Harris, Owner/Operator was present and assisted me as needed. Your program currently operates with a three-star license, issued October 18, 2019, earning 4 points in the education component, 2 points in the program standards component and 1 quality point for serving no more than two infants under age one. The program is approved for daytime and overnight. INSPECTIONS *The last annual compliance visit was conducted on January 28, 2025. *The last fire drill and outdoor inspection were both completed on December 12, 2025. *The last lockdown drill was conducted on September 22, 2025. I visited each indoor and outdoor space used by the children. The two preschool children present were eating breakfast, participating in group time, and free play in activity areas. Two violations were observed today. The violations are as follows: Violation Number Comment Rule 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. The last lockdown drill was conducted on 9/22/25. .1719(a )(16) & .1721(e )(7) To comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be sent to me by January 22, 2026. The letter must address each violation and explain how it has been corrected. Please send this to Kia.Reid@dhhs.nc.gov and include the following: -Facility name -Facility ID number -Each item number -Your signature Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The program’s compliance history was ninety-three percent as of 1/5/26. TECHNICAL ASSISTANCE *Your shelter-in-place or lockdown drill must be conducted every three months. I suggested Ms. Harris add an alert on her phone as a reminder to conduct the drill. ANNUAL COMPLIANCE RECORDS I monitored the provider's and children’s files, as well as required program records. The on-going training requirements were monitored from January 2025-2026 and was in compliance. *Your qualification letter will expire on 4/13/26. Paperwork for the 5 year recheck can be submitted six months prior to the expiration of the qualification letter. I suggest you start the process early to avoid a lapse in compliance. RATED LICENSE INFORMATION During today’s visit we discussed the new QRIS pathways to the Stars, and you were given an opportunity to ask questions. You should visit the DCDEE website to review the new Pathways to the Stars. You will find the following resources in the QRIS section. -Pathways to the Stars Family Child Care Home Session Slide Deck -Pathways to the Stars Questions and Answers -Frequently asked questions A technical assistance visit was scheduled for 2/25/26 to discuss the pathway that will be chosen. REMINDERS *Your CPR and First Aid training will expire on 12/20/26. *Children are enrolled on 1st, 2nd, and 3rd shift. I reminded the provider to ensure children leave on time to prevent shift overlap. I did not observe shift overlap during the visit or on the attendance reviewed. *Child Care Rules have been updated as of 7/1/25. The provider, Deborah Harris, was present and assisted me as needed. The visit summary was completed off-site due to technical issues. The visit summary was emailed and reviewed with Ms. Harris. Upon arrival, I was greeted by Ms. Harris. I completed a general walk through of the areas used by the children inside and outside. There were three (3) preschool children present, observed listening to the provider read stories. The Infant was drinking a bottle and having tummy time. CURRENT LICENSE STATUS Currently this family child care home operates with a three-star license issued October 18, 2019, earning four (4) points in Staff Education, two (2) points in Program Standards and one (1) Quality Point. INSPECTIONS *The last annual compliance visit was conducted on January 17, 2024. *The last fire drill, lockdown drill and playground inspections were completed on January 21, 2025. DESIGNATED CHILDREN’S SPACE In the children’s space, there are a variety of materials for the children to explore throughout the day. The bathroom is accessible to those that are potty trained. There is a changing pad inside of the bathroom for those in diapers. Activities take place in the child care space and dining area. *The outdoor play area is in the backyard of the home and is not fenced. There are no sheds on the property. *The provider informed me of the following: -Medication is not being administered. -Transportation is not provided at this time. -There have not been any incident/accident reports written. -There is one pet in the home. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 1804 Safe sleep policy and poster about safe sleep practices was not posted in a prominent place in the infant sleeping room or area where parents and caregivers are able to view daily. This information was not posted. .1724(b) 1821 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of 30 days. The safe sleep checks were not completed for the infant enrolled. .1724(a)(8)&(f) 1876 The operator did not review the EPR Plan annually or when information in the plan changed to ensure all information is current. The EPR plan was not available for review. .1714(e ) 2027 The professional development plan was not maintained in the personnel file. The professional development plan was not completed. .1703(i)(5) 2048 Products that are labeled “keep out of reach of children” with an additional warning(s) on the label, were not kept in locked storage while children were in care. Clorox wipes were o the kitchen counter and Dawn powerwash was on top of the refrigerator both not in locked storage. 10A NCAC 09 .1719(a)(7) To comply with the NC Laws and Rules any violations cited today must be corrected immediately. According to NC General Statute 110-90 (4) (d) all facilities must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months. ANNUAL COMPLIANCE RECORDS I monitored the provider's file, three children’s files, and required program records. The on-going training requirements were monitored from January 2024-2025 and was found to be in compliance. TECHNICAL ASSISTANCE *You must observe and record sleep positions for all infants who are sleeping. Some programs find it helpful to set a timer for 15 minutes, when infants are sleeping and reset the timer until all infants are awake. This gives an auditory reminder to check on the infants while they are sleeping. A safe sleep environment is essential in reducing the risk of SIDS. *To ensure products are stored properly check the label for warnings. Items labeled "keep out of reach of children" plus other warnings must be in locked storage. Products with "keep out of reach of children" without additional warnings must be stored 5 feet out of reach. REMINDERS The following requirements will come due in the near future; please make arrangements to complete the items before the deadline. *Your ITS-SIDS training will expire on 2/8/25. *Please check the Division of Child Development and Early Education website: ncchildcare.ncdhhs.gov frequently and click the “what’s new” tab to stay informed of all updates available to you. The purpose of today’s visit was to monitor applicable childcare requirements during your Annual Compliance visit. The last Annual Compliance visit was conducted on 2/8/23. This Family Child Care Home operates with a three-star license, which was issued on 10/18/19. COMPLIANCE HISTORY: The compliance history for the previous 18-month timeframe was 96%. During today’s visit the indoor child care space and the outdoor environment were monitored. OBSERVATIONS: Upon arrival there were no children present. The operator’s adult resident daughter was present during the visit. One child, age 2, arrived during the visit. The operator stated that the school age children, who attend before school, did not attend today because of the public school two-hour opening delay. ACTIVITY PLANS: Current activity plans were posted in child care space. FOOD ALLERGIES: Food allergy information was posted on the refrigerator. CHILDREN’S RECORDS: No new children had enrolled in this facility since the 2023 Annual Compliance visit. SMOKE DETECTORS: Both smoke detectors (electric with battery back-up) were tested during the visit. OPERATOR REQUIREMENTS: I was given the signed statements of understanding, enrollment worksheet, and operator’s verification form. The information on the three forms was reviewed and discussed with the operator during the visit. TRANSPORTATION: The operator stated that she was not conducting transportation. INSPECTIONS: The last documented fire drill was conducted on 12/11/23. The last outdoor inspection was completed on 12/11/23. The last documented lockdown drill was conducted on 10/13/23. Please remember to conduct another lockdown drill in January 2024. The following violation was cited today: Violation Number Comment Rule 1301 Operator did not complete the required number of on-going training hours as specified in rule. Verification of six additional hours of in-service training was needed to determine that the operator had completed her yearly in-service training hour requirement. GS 110-91(11); 10A NCAC 09 .1705(b)(5) All violations must be corrected immediately. Please send me a signed Corrective Action letter by 1/31/24 explaining how the violation cited today was corrected. I made the operator aware that God’s Victorious Army was in Cohort II for the reassessment. I explained that the preparation year for the reassessment began 7/1/24 and that the year for the reassessment would begin on 7/1/25. The operator was aware of the requirement of completing a three-month self-study for two points in the programming component of the Rated License assessment process. The Environment Rating Scales have never been conducted at this Family Child Care Home. Since the operator was thinking about the Environment Rating Scales, Technical Assistance was provided on the option of having the Environment Rating Scales conducted during the preparation year. I stated that information and resources about the Family Child Care Home assessment tool (FCCERS-R) was available on the NC Rated License Assessment Project website (www.ncrlap.org).
