The director, Shyamini Monaco, was present and assisted me during today’s visit. The visit summary was reviewed with Ms. Monaco, and a copy was given at the conclusion of the visit. Upon arrival, you greeted me and welcomed me into your program. I completed a general walk through of the five (5) classrooms indoor learning environment. The children were observed having free playing, finishing up music class, and having circle time. RATED LICENSE STATUS The program holds a 1-star-rated license re--issued on September 14, 2022. INSPECTIONS The last fire drill was conducted on 11/12/25. The last emergency drill was conducted on 9/22/25. The last playground inspection was conducted on 11/12/25. The last fire inspection was conducted on 11/19/24. The last sanitation inspection was conducted on 11/24/25. During today’s visit, a partial assessment of the Child Care Requirements was monitored. The following items monitored were Supervision, Staff/Child Ratio, CPR/FA, Special training, Criminal Background Checks, Emergency Medical Care Plan, Administering of Medication, Storage of Hazardous Substances, Storage of Medication, General Safety, Discipline, Adequate/Approved Space, Staff Records, Program Records, License Posted, and Permit Restrictions. The following violation was observed and recorded during today's visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. A current fire inspection was not documented and available for review during today's visit. The last fire inspection was conducted on 11/19/2024. 10A NCAC 09 .0304(a) In order to comply with the NC Laws & Rules, any violations cited today must be corrected immediately. We are encouraging all staff to begin their renewal process early to avoid lapses in their qualification. All medications need to be label with the child’s name. It is fine to label the bag as well, however the ointment needs to be labeled. Make sure all staff is using the correct form for diaper cream, which is an over-the-counter ointment. I would suggest doing a general walk-through of the building daily to ensure that all plastic bags are stored five feet high and out of the reach of children that are less than three years of age. I would suggest putting a monthly reminder on your calendar for all required inspections that are due monthly, every three months, or yearly. I would suggest having the teachers to check the thermometer in the refrigerator daily to ensure that the temperature is reading 45 degrees or lower. Then the teachers can go behind to make sure the task is done and if not, they can make the corrections and notify the parent regarding what needs to take place on a daily basis. The owner/director, Mary Opatha, was present and assisted me during the visit. The visit summary was completed off-site, emailed, and reviewed with Ms. Opatha. Upon arrival I monitored the two classrooms that were open. At approximately 9:20am, I completed a walk-through of five(5) classrooms. There were six (6) staff, and forty-seven (47) children present during the visit. The children were observed during morning drop-off, having a morning snack, and in center activities. Limited monitoring of child care requirements was conducted during the visit. Supervision of children, staff/child ratios, use of adequate/approved space, permit restrictions and nurture/care were monitored. The license and emergency care plan were posted. During today’s visit, I discussed the allegations with the owner/director and two staff. They were each given an opportunity to provide information surrounding the allegations. ALLEGATION There is a concern regarding staff/child ratio. FINDINGS I arrived to the facility at 8:30am and was greeted by the teacher in space #6. There were two classrooms open at this time. Space #1 was open with seven children present: two infants, two children one year of age and three children two years of age with one staff. Space #6 was open with five children present: four children four years of age and one child six years of age with one staff. Ms. Opatha arrived to the facility around 8:40am and immediately provided coverage in space #1. Ms. Opatha stated that she was late due to traffic, and usually arrives to the center around 8:20am. If anyone is late or will be absent she would provide coverage. Ms. Opatha stated that two staff members were present in the center at 8:30am conducting a conference with parents and didn’t know she wasn’t present to provide coverage for the classrooms. I asked Ms. Opatha if she called the center to let them know she would be late and she stated no. According to the information provided, Ms. Opatha and five staff are present at 8:30am each day when the center opens to provide adequate coverage for the classrooms. Today was not a typical day due to several staff not getting to work on time due to traffic. RESOLUTION Based on my observations this allegation is SUBSTANTIATED. ALLEGATION #2 There is a concern regarding supervision. FINDINGS During the interviews I asked if anyone was aware of children being left unattended. The owner/director and staff were not aware of this. When children have to be moved to another classroom to maintain ratios Ms. Opatha or another teacher will take the child/children to the appropriate classroom. Ms. Opatha stated the children are never left unattended. I did not observe the staff leave any children unsupervised during the visit. RESOLUTION Based on my interview findings and observations, I was not able to determine if children are left unattended, therefore the allegation has been UNSUBTANTIATED. ALLEGATION #3 There is a concern regarding nurture/care. During the interviews I asked if there were any concerns from anyone regarding negative staff/child interactions. The owner/director and staff were not aware of this. During my walk through I did not observe inappropriate staff/child interactions in the classrooms. The staff were observed greeting children during morning drop-off, assisting children with washing hands, and assisting children with morning snack. During center activities staff walked around the classroom being attentive to the children’s needs, engaged in appropriate conversations. Positive interactions between staff and children was observed during the visit. Based on my interview findings and observations, I was unable to determine concerns of nurture and care, therefore this allegation is UNSUBTANTIATED. The following violation was observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. In space #1 there were seven children present, infants-two years of age with one staff. GS 110-91(7);.0713(a-d) In order to comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be sent to me by May 16, 2024. The letter must address each violation and explain how it has been corrected. According to NC General Statute 110-90 (4)(d) all facilities must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months. TECHNICAL ASSISTANCE Since today was not a typical day with several staff being late, I suggested the following to Ms. Opatha. -If she is late calling the center so staff are aware and can adjust staffing as needed, even if she is just 5 minutes away. -Schedule conferences for a different time of the day. A return visit will be made in the near future due to the staff/child ratio violation that was cited. You are also at risk of being issued an administrative action due to the substantiated allegation. REMINDERS Your Annual Compliance Visit is due soon. INSPECTIONS *The last annual compliance visit was conducted on June 6, 2023. *The sanitation inspection was completed on May 19, 2023, with a "Superior" classification. *The last documented fire drill October 15, 2023. *The last documented playground inspection October 30, 2023. *The last documented shelter-in-place drill was conducted on September 29, 2023. During this visit, a partial assessment of Child Care Requirements was conducted.
