Name of Operation: GLENN SAC CLUB Facility ID: 32001472 Consultant: SHANNEL HALL Operation Type: Center Case Number: Visit Date: 9/25/2025 Number Present: 16 Completed Date: 9/25/2025 Age: From 5 To 9 Total Minutes: 120 Time In: 01:45 PM Time Out: 03:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Unannounced Visit Follow-Up Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to do a follow-up from the visit conducted on September 11, 2025, where a violation for staff/child ratio was observed. Jan Smith, Site Manager, was present and assisted me when needed. The visit summary was reviewed with Ms. Smith, and a copy was left with her at the conclusion of the visit. Upon arrival, I checked in to the main office to meet the site manager. I completed a general walk-through of the spaces used by the children. I observed the children arriving at their indoor learning environment, doing routine care to prepare for snack, eating snack while listening to music. The children were having group time. They were explaining how their day was and what they were taught in school today. During today's visit, a partial assessment of the Child Care Requirements was monitored which included permit restrictions, staff/child ratios, supervision, and the use of approved space. The following violation was observed and recorded during today's visit. Violation Number Comment Rule 1756 Enhanced staff/child ratios and group sizes were not met. In space #1 one teacher had 3 five year old children, 3 six year old child, and 7 seven year old children, 1 eight year old child, and 2 nine year of children which exceeded their ratio size. 10A NCAC 09 .2818 In order to comply with the NC Laws & Rules, any violations cited today must be corrected immediately. A compliance letter should be sent to me by October 9, 2025. The letter should address each violation, and explain how and when it has been corrected. Please send the letter to shannel.hall@dhhs.nc.gov and include the following: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today's visit, the Compliance History Score for the facility was 94%. It will be updated after today's visit, and you will be informed of the new score. TECHNICAL ASSISTANCE The following items were discussed to help you prevent a reoccurrence of the violations observed during today's visit. I would suggest having a conversation with your supervisor again to discuss hiring another person to maintain with ratios. Also, decide if the number of children that are enrolled needs to decrease. Maintaining ratios at all times is required and aid in the safety of our children. CONSULTATION Staff/Child Ratio was cited during a routine unannounced visit that was conducted on September 11, 2025, and a follow-up visit that was conducted on September 25, 2025. Please be advised that your facility could be at risk of being on put on an “Administrative Action” due to two consecutive violations dealing with staff/child ratio. Jan Smith, Site Manager, was present and assisted me when needed. The visit summary was reviewed with Ms. Smith, and a copy was left with her at the conclusion of the visit. Upon arrival, I checked in to the main office to meet the site manager. I completed a general walk-through of the spaces used by the children. I observed the children arriving at their indoor learning environment, exploring different centers, having conversations with the group leaders and their afterschool mates, and having snack. RATED LICENSE STATUS The facility currently operates with a Five-star license issued on March 3, 2020. The program received 6 points in Program Standards, 7 points in Staff Education, and 1 Quality point through the programmatic option of having staff benefits package and an infrastructure of parent involvement. During today’s visit we discussed the new QRIS pathways, and you were given an opportunity to ask questions. Please visit the DCDEE website to review the QRIS Modernization requirements. Due to school being out for the summer an emergency drill will be conducted this month (September). During this visit, a partial assessment of the child care requirements was conducted. The following items were monitored and found in compliance during this visit: nurturing interactions, daily attendance, license posted & permit restrictions followed, safe indoor environment, fire drills & inspections, CBC qualifications, and CPR/First Aid certification. I would suggest looking at other subs or having extra help on site to help with ratios. Also, decide if the number of children that are enrolled needs to decrease. Maintaining ratios at all times is required and aid in the safety of our children. I would suggest doing a quick sweep of the bathrooms as well as the café to make sure all items are put away and clean to prepare for after school to begin before putting out the learning materials. I would suggest talking with your supervisor to inform them of the problems you are having with the janitor’s service-leaving cleaning items out as well as the mop pale with water in them out in the area the children will be using. I also suggest coming up with a plan that is durable for you to complete your responsibilities while maintaining the safety of the children. First and foremost, the children are the most important. Being out of ratio can cause incidents out of our control. CONSULTATION Some providers are experiencing delays in scheduling their fingerprinting appointments. Criminal record checks can be completed up to six months prior to the expiration date. We are encouraging all staff to begin their renewal process early to avoid lapses in their qualification. HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. You should use your NCID username and password to access the Moodle portal. As per Child Care Rule .1103(b) the Health and Safety trainings must be renewed at least every 5 years. Some of your staff completed the trainings in 2017, so they should begin to retake the training now. Upon arrival, I checked into the main office then proceeded to the gym to meet Ms. Smith. I completed a general walk through of the spaces used by the children and outdoor play areas. Although this DPI program does not want their playground to be monitored for child care requirements, I did check for safety concerns. I observed the children arriving to the program, transitioning to activities, and eating afternoon snack. CURRENT LICENSE STATUS Currently this program operates with a five-star license issued March 3, 2020, earning 7 points in the Staff Education Component, 5 points in the Program Standards Component and 1 Quality Point. The facility operates after school care from 2:15pm-6:00pm. Currently the children have access to the gym and cafeteria. The bathrooms are in the hallway outside of the cafeteria. INSPECTIONS *The last annual compliance visit was conducted on April 9, 20234. *The last sanitation inspection was completed on April 15, 2024. *The last documented lockdown drill was completed on September 12, 2024. *The last documented fire drill was conducted on September 30, 2024. During this visit, a partial assessment of Child Care Requirements was conducted. The items monitored were Supervision, Staff/Child Ratio, CPR/FA, Special training, Criminal Background Checks, Emergency Medical Care Plan, Storage of Hazardous Substances, Storage of Medication, General Safety, Discipline, Adequate/Approved Space, Program Records, License Posted, and Permit Restrictions. Violation Number Comment Rule 205 Storage space was not available for each child's personal belongings. The program did not have storage for the children's belongings. G.S. 110-91(6) 1757 A valid qualification letter was not on file and available to review at the facility. The qualification letter for the staff present today expired on 9/30/24. If a valid qualification letter is not on file, at that time, I will re-cite the violation, and she must leave the program until a new qualification letter is obtained and available for my review. Remember your paperwork for the 3-year re-check can be submitted six months prior to the expiration of the qualification letter. STAFF RECORDS Staff records were monitored since the last annual compliance visit in April 2024 for criminal background qualifying letters, current CPR/First Aid and BSAC training. On your next annual compliance visit training hours will be monitored.
