    Name of Operation: GINA'S GENERATIONS Facility ID: 32001846 Consultant: KIA REID Operation Type: Family CC Home Case Number: Visit Date: 2/27/2024 Number Present: 5 Completed Date: 2/27/2024 Age: From 1 To 1 Total Minutes: 205 Time In: 09:50 AM Time Out: 01:15 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp w/Rated Lic Assess Announced/Unannounced: Unannounced The purpose of today's unannounced visit was to monitor compliance during your Annual Compliance with Rated License Visit. The provider, Jorgina Fitzgerald was present and assisted me today. The visit summary was reviewed with Ms. Fitzgerald and a copy was given to her at the conclusion of the visit. Upon arrival, I was greeted by Ms. Fitzgerald. I completed a general walk through of the children's space. The outdoor play area was not monitored due to the rain. There were five (5) preschool children present, and they were observed in center activities, music time, routine care and eating lunch. CURRENT LICENSE STATUS Currently this family child care home (fcch) operates with a four-star license issued June 12, 2017, earning seven (7) points in staff education, two (2) points in program standards and one (1) quality point. INSPECTIONS *The last annual compliance visit was conducted on March 6, 2023. *The last documented fire drill and outdoor inspections were conducted on February 9, 2024. *The last documented lockdown drill was conducted on December 20, 2023. DESIGNATED CHILDREN’S SPACE In the children’s space, there are a variety of manipulatives, pretend play toys and books for the children to explore throughout the day. All activities take place in the child care space including meals. The bathroom is located in the child care space for those that are potty trained, and a changing table is available for those in diapers. *The outdoor play area is in the backyard of the home and is fenced. *The provider informed me of the following: -She is not administering medication to any of the children. -Transportation is not provided at this time. -There are no children with allergies. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 906 Authorization to seek medical care in the parent's absence was not available. The hospital preference and health care provider was not on file for one child currently enrolled. .1721(a)(3)(I) 1893 Application did not include all required information including, but not limited to: child's full name, named to be called, child’s date of birth, any allergies, including symptoms and the type of response required, and any fears or behavior characteristics a child has. This information was not on file for one child currently enrolled. 10A NCAC 09 .1721 (a)(3)(A)(B)(C)(E) 1922 Plastic bags, toys, and toy parts small enough to be swallowed, including but not limited to materials that can be easily torn apart such as foam rubber and styrofoam, were accessible to children under the age of three. Diapers in plastic bags and curtains wrapped in plastic were accessible to the children one year of age. .1719(a)(18) 1979 Unflavored skim milk and/or unflavored low-fat milk, was served to children under the age of 24 months. Unflavored low-fat milk (1%) was served to the children one year of age. .1706(u)(5) In order to comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be sent to me by March 12, 2024. The letter must address each violation and explain how it has been corrected. Please send this to Kia.Reid@dhhs.nc.gov and include the following: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today's visit, the Compliance History Score for the facility was 99%. According to NC General Statute 110-90 (4) (d) all facilities must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months. ON-GOING TRAINING HOURS The on-going training requirements were monitored from March 23-February 24 and was found to be in compliance. Her contact information is below. morrise@email.unc.edu, (919) 809-9645 RATED LICENSE INFORMATION Rated License assessments have resumed, and you are in Co-Hort 1. Your planning (preparation) period started on 7/1/23 and the assessment period begins 7/1/24. We discussed the following: -Completing the FCCERS-R assessment in the preparation year. -If you are satisfied with your scores, you can move forward with the rated license reassessment. If you are not satisfied with the scores received, you can complete the assessment again and use the scores as guidance. Ms. Fitzgerald stated that she would like to proceed with the Rated License process and not complete the FCCERS assessment at this time. I left the Application for A Two Component Star Rated License for the provider to complete and return to me by 3/12/24. STAFF EDUCATION The program will receive seven (7) points in the Education Standards component. The provider has a BS/CD and 18 months experience. PROGRAM STANDARDS The program will receive two (2) points in this component. Meets minimum licensing requirements, has written operational policies and procedures, and completed the self-study during the months of September, December, February. QUALITY POINT The Quality Point chosen was the Education Option by having a BS/BA in EC/CD. Pending final approval, this program will be eligible for 10 points and 4 stars.
