Angela Sherrill, Owner/Operator, was present and assisted me as needed. Your program currently operates with a five-star license, issued June 12, 2024, earning 5 points in the education component, 7 points in the program standards component and 1 quality point for serving no more than two infants under age one. The program is approved for 1st and 2nd shift. There are currently no children enrolled on 2nd shift. The license was posted, and the restrictions were in compliance. A walk-through of the fcch was completed today, monitoring all areas used by the children indoors and outdoors. There were four (4) preschool children present. The children were engaged in center play, in personal care routines and preparing for lunch. Prior to today's visit I reviewed the NC Secretary of State's website and observed that the owner of this facility, Future Leaders Preschool, LLC, is listed as current/active. INSPECTIONS *The last annual compliance visit was conducted on 1/7/26. *The last documented outdoor inspection was completed on 5/6/26. Two violations were observed today and must be corrected immediately. The violations are as follows: Violation Number Comment Rule 1853 The operator did not conduct a monthly fire drill. The last documented fire drill was conducted on 4/28/26. .1719(a)(15) & .1721( e)(2) 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. The last documented shelter-in-place drill was completed on 2/6/26. .1719(a )(16) & .1721(e )(7) In order to comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be sent to me by July 2, 2026. The letter must address each violation and explain how it has been corrected. Please send this to Kia.Reid@dhhs.nc.gov and include the following: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY You are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The program’s compliance history was 94% as of 6/10/26. OPERATOR RECORDS Your file was monitored for criminal background qualifying letters, current CPR/First Aid, and other specialized training. On your next annual compliance visit, on-going training hours will be monitored. I verified in the ABCMS portal that your qualification letter was issued on 5/27/26. You must log into the portal and print the updated qualification letter. TECHNICAL ASSISTANCE Emergency drills, such as fire drills and lockdown/shelter-in-place drills, must be conducted in a timely manner because they allow staff and children to practice and prepare for an actual emergency. Fire drills must be conducted at least once per month, and a shelter-in-place/lockdown drill must be conducted every three months. I suggest setting a reminder on your calendar to help you maintain compliance with this rule. The provider, Angela Sherrill was present and assisted me as needed. This documentation was reviewed with Ms. Sherrill, and a copy of the visit summary documentation was given to her at the conclusion of the visit. I completed a general walk through of the areas used by the children inside and outside. There were five (5) preschool children and two (2) school-age children present. The children were participating in free play activities and routine care. CURRENT LICENSE STATUS Currently this family child care home operates with a five-star license issued June 12, 2024, earning five (5) points in Staff Education, seven (7) points in Program Standards and one (1) Quality Point. INSPECTIONS *The last annual compliance visit was conducted on January 1, 2025. *The last fire drill was conducted on May 21, 2025. *The last lockdown drill was completed on May 19, 2025. DESIGNATED CHILDREN’S SPACE In the children’s space, there are a variety of materials for the children to explore throughout the day. The bathroom is accessible to those that are potty trained. There is a changing table outside of the bathroom for those in diapers. All activities take place in the child care space. During this visit, a partial assessment of Child Care Requirements was conducted. The items monitored were Supervision, Capacity, Discipline, License Posted, Permit Restrictions, Adequate & Approved space, Program Records, Staff Records, CPR, First Aid, ITS-SIDS, Criminal Background Checks, Storage of Hazardous Substances, and Storage of Medication. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division The playground inspections were not available for review. 10A NCAC .1721(e)(5)(A-F) 929 Developmentally appropriate activities for the ages of children in care, were not provided as planned on the written schedule and activity plan. The provider, Angela Sherrill was present and assisted me as needed. This documentation was reviewed with the provider, and a copy of the visit summary documentation was given to her at the conclusion of the visit. Upon arrival, I was greeted by Ms. Sherrill. I completed a general walk through of the areas used by the children inside and outside. There were four (4) preschool children present observed in routine care, free play activities, and outdoor play. CURRENT LICENSE STATUS Currently this family child care home operates with a five-star license issued June 12, 2024, earning five (5) points in Staff Education, seven (7) points in Program Standards and one (1) Quality Point. DESIGNATED CHILDREN’S SPACE In the children’s space, there are a variety of materials for the children to explore throughout the day. The bathroom is accessible to those that are potty trained. There is a changing table outside of the bathroom for those in diapers. All activities take place in the child care space. *The outdoor play area is in the back yard of the home and is fenced. One shed is on the property and is kept locked. *The provider informed me of the following: -Transportation is not provided at this time. -There have not been any incident/accident reports written since my last visit. -There are no children with allergies. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 1986 Breast milk, formula, and other bottled beverages sent from home were not fully prepared, dated, and labeled with individual child's names. Two bottles were not dated and labeled with the child's name. .1706(l) 2048 Products that are labeled “keep out of reach of children” with an additional warning(s) on the label, were not kept in locked storage while children were in care. A bottle of white out and disinfecting wipes were on a shelf not in locked storage. 10A NCAC 09 .1719(a)(7) The violations cited were corrected during the visit, therefore no letter of compliance is needed. ANNUAL COMPLIANCE RECORDS I monitored the provider's file, children files and required program records. The on-going training requirements were monitored from January 24-25 and was found to be in compliance. Please remember that all trainings must be documented on the on-going training log. CRIMINAL BACKGROUND CHECK The qualification letter for your husband will expire on 9/10/25. Remember your paperwork for the 3-year re-check can be submitted six months prior to the expiration of the qualification letter. CPSC RECALLS The U.S. Consumer Product Safety Commission has recalled numerous items. You can find more information about each item by accessing www cpsc.gov. The purpose of today’s visit was to monitor applicable child care requirements during a Routine Unannounced visit. The last Annual Compliance visit was conducted on 2/2/23. This Family Child Care Home operates with a five-star license, which was issued on 3/29/22. The compliance history for the previous eighteen-month time frame is 92%. During today’s visit all indoor child care space and the outdoor environment were monitored. There seven children present today (4 preschool age and 3 school age). I observed the school age children involved in a variety of games in the family room. I observed the preschool age children playing with materials in the child care room. I observed the operator feeding the infant a bottle of milk. I later observed 6 or the 7 children doing aerobics with the operator in child care space. By reviewing the documentation from the 2/9/23 Annual Compliance visit, I verified that the operator had current CPR, First Aid, and Safe Sleep training, along with current Criminal Record Check clearance. I verified that adult household members had current Criminal Record Check clearance. CHILDREN: The operator stated that none of the children had a food allergy or were on medication. During today’s visit I reviewed the files for the infant and three school age children. CURRICULUM: The operator stated that she had Creative Curriculum on hand for implementation when preschool age children (ages 4 and above) were present. I observed current activity plans for this week. INSPECTIONS: The last documented fire drill was conducted on 6/19/23. The last documented lockdown/shelter-in-place drill was conducted 5/25/23. The last outdoor inspection was completed on 6/16/23. The following violations were cited today: Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. The three school age children did not have a medical assessment on file. GS 110-91(1); 10A NCAC 09.1721(a)(1) 911 Copy of each child's immunization record, including the operator's own preschool child(ren), was not on file. Two of the three school age children did not have an immunization record on file. GS 110-91(1); .1721(a)(2) 1934 Operator was not positioned in a manner that would maximize his/her ability to hear or see the school age children in care, at all times, and be able to render immediate assistance. Upon arrival there was a group of preschool age children in the child care room. There were also three school age children in the family room. The sliding glass door between the two rooms was closed and the curtain on the sliding glass room was drawn together. .1711(a )(2) All violations must be corrected immediately. Please send me a signed Corrective Action letter by 7/27/23, explaining how the violations have been corrected. Since a violation of supervision was cited today, a return visit will be made to monitor supervision requirements. TECHNICAL ASSISTANCE WAS PROVIDED TODAY IN THE FOLLOWING AREAS: SUPERVISION: NC Child Care Rule 10 NCAC 09. 1711 (a) and (b) was reviewed with the operator. I explained that having a visual baby monitor was advantageous it did not a substitute for visually seeing or hearing the children. The sliding glass door and curtain between the child care space and the family room should always remain open, when there were children present in both spaces. This Family Child Care Home was licensed in the fall of 2021. The operator stated that was her first experience with providing care for school age children during the summer months. Technical Assistance was provided by making suggestions for the operator to consider to maintain supervision of all the children easier. (1) Arrange equipment/materials in child care space for the summer months to create a specific space for the school age children. I discussed storage of school age materials for safety. (2) I explained that the operator’s license permitted her to change the designated child care space in the home for better supervision of all space used by children. RATED LICENSE: The Rated License reassessment process is resuming. According to the information I have today, this program will begin the reassessment process in July 2026.
