Upon arrival, I was greeted by the provider, Florence Jackson, who assisted me during today’s visit. LICENSE STATUS The family child care home currently operates with a Three Star license issued on January 13, 2020. The program received five points for Staff Education, two points for Program Standards, and one Quality Point. INSPECTIONS/DRILLS STATUS -The last annual compliance visit was conducted on 7/17/24. -The last documented monthly outdoor play area inspection was completed on 5/11/25. -The last documented monthly fire drill was conducted on 5/11/25. -The last documented quarterly lockdown/shelter-in-place drill was conducted on 5/16/25. MONITORING I completed a walk-through of the home, including the children’s space, kitchen, bathroom, and outdoor play area. At the time of the visit, there were three children present ranging in age from 14 months to six years old. There are four children enrolled. The children were observed engaging in free play with toys, coloring, interacting with the caregiver, and playing outside. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, capacity, health and safety requirements, equipment and furnishings, and program records. The provider does not transport children; therefore, this requirement was not monitored. No enrolled children have medications or medical care plans, so these requirements were not monitored. There are no children 12 months or younger are enrolled; therefore, safe sleep requirements were not monitored. The provider verified that all the information on the FCCH Verification of Required Information for Operator and Additional Caregivers form was correct. INDOOR/OUTDOOR ENVIRONMENT Inside the home, children are cared for in the living room adjoining the kitchen. Basic materials for learning through play are available, including manipulatives, books, electronic toys, and home living props. The bathroom is located down the hall, and cots and a portable crib are used for rest time. The outdoor play area is a fenced backyard with portable gross motor toys. You stated that you sometimes use the front yard as well, which is partially enclosed with a low ornamental fence. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 1893 Application did not include all required information including, but not limited to: child's full name, named to be called, child’s date of birth, any allergies, including symptoms and the type of response required, and any fears or behavior characteristics a child has. One child's application did not have a date of birth listed or parental authorization to obtain emergency medical care. 10A NCAC 09 .1721 (a)(3)(A)(B)(C)(E) 1919 A First Aid Information sheet, complete with required information, was not posted for quick referral by staff. No first aid information was posted. .1719(a )(14) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by July 1, 2025. The letter must address each violation and explain specifically when and how it was corrected, as well as how you will prevent the same violations from happening in the future. Please send this to mandy.annunziata@dhhs.nc.gov and include the following information: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today’s visit, the facility’s compliance history score was 89%. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. Failure to maintain compliance may result in an administrative action. The facility was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact the center’s overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your program. TECHNICAL ASSISTANCE -It’s best practice to have easily accessible materials, including a first aid kit and instructions, that you can use in the event of an emergency. Your first aid poster was not available for reference. You can order a free copy of this poster from healthychildcare.unc.edu. Click on “Resources” and then “Posters.” Be sure to post it in your child care area. -It’s important to have parental authorization to obtain medical treatment in the event of an emergency. Today, one child’s application did not contain all the required information, including the child’s date of birth or medical authorization. I gave you a children’s application for enrollment from the DCDEE website that includes all required information. Please ask the child’s parent to complete it today. To ensure each child’s file is complete, you can use the children’s file checklist available on the DCDEE website in “Provider Documents and Forms” under the “Provider” tab. CONSULTATION -You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please review and update your EPR plan yearly. -Criminal Backgrounds Checks for the provider and all household members 16 years of age and older should be completed every five years. TRAINING REMINDERS -Training from 7/17/24 to 6/16/25 was reviewed today and documented on the Verification of Required Information for Operator and Additional Caregivers form. You still need to complete eight hours of training. Please send documentation of completed hours to me by 7/17/25. -It is important that all adults working with young children are trained to spot potential signs of abuse. “Recognizing and Responding to Suspicions of Child Maltreatment” (part of the Health and Safety Training) should be renewed every five years. This training can be found on the Positive Child Alliance NC website: https://preventchildabusenc.org/online-trainings/ -CPR and First Aid must be renewed every two years. Your CPR and First Aid certification expires on 3/8/27. -ITS-SIDS training must be renewed every three years. Your training expires on 8/16/26. -Health and safety training must be renewed every five years. You are due to complete all the training topics again by 2029. RATED LICENSE STATUS The NC State Legislature has extended “hold harmless” status until new rated licensing guidelines are in place, and you may opt to keep your current license and delay your rated license assessment until the new guidelines are adopted. Upon arrival, I was greeted by the provider, Florence Jackson, who assisted me during the visit. LICENSE STATUS The family child care home currently operates with a Three Star License issued on January 13, 2020. The program received two points for Program Standards, five points for Staff Education, and no quality point. INSPECTIONS/DRILLS STATUS -The last documented outdoor play area inspection was completed on 12/17/24. -The last documented fire drill was conducted on 12/17/24. -The last documented lockdown/shelter-in-place drill was conducted on 11/6/24. -The last annual compliance visit was conducted on 7/17/24. MONITORING I completed a walk-through of the home, visiting the caregiving space, kitchen, bathroom, and outdoor play area. Three children were present, ranging in age from 8 months to five years old. The preschool children were coloring and playing with materials in the caregiving space. The infant was moving around in a walker and being held by the provider. The provider’s adult daughter also was present. A partial assessment of child care requirements was conducted. I monitored all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records. No enrolled children have medications or medical care plans, so these requirements were not monitored. A child 12 months of age or younger is enrolled; therefore, safe sleep requirements were monitored and found in compliance. INDOOR/OUTDOOR SPACE Children are cared for in the living room of the home, and basic materials for learning through play are available, including home living props, books, electronic toys, and manipulatives. A bathroom is located down the hall, and cots and a portable crib are used for rest time. The outdoor play area in the front yard is partially enclosed with a low ornamental fence. The provider stated that she brings a variety of toys from the back yard to the front yard for gross motor play. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). The provider's qualification letter expired 10/29/24. G.S. 110-90.2 & .2703(n) & (o) 1846 An individual over the age of 15, who resides in a family child care home or a center located in a residence on a permanent or temporary basis, or those who had their 16th birthday after the initial licensing of a family child care home; did not complete and submit required forms within five business days. A household member's qualification letter expired on 10/29/24. G.S. 110-90.2 & .2703(q) 1889 Products that are labeled "keep out of reach of children" without any other warnings, were not stored on a shelf or in an unlocked cabinet that is five feet above the finished floor. TECHNICAL ASSISTANCE -Criminal Backgrounds Checks for all providers and household members ages 16 and older should be completed every five years. Your and your husband’s last qualifying letters expired on 10/29/24 and must be renewed immediately. You stated that you and your husband had completed the fingerprinting portion of the background check in October 2024, but you didn’t realize you still needed to submit your application. I showed you where to find criminal background check information on the DCDEE website. You have 15 days to obtain new qualifying letters; after that, you will not be able to keep children in child care until you receive your new qualifying letters. -Safety is a top priority when providing care for young children. Today I observed plastic grocery bags and sanitizing spray within reach of children in the bathroom. The bag poses a potential suffocation and choking hazard for the baby in your care, and the spray can make children sick if swallowed. I suggest conducting a walk-through of the children’s space prior to children’s arrival each morning, removing any potentials hazards and placing them at least five feet high. -It’s best practice to have easily accessible materials, including a first aid kit and instructions, that you can use in the event of an emergency. Please review and update your EPR plan yearly, and be sure to add my name as your consultant. -Your pet’s vaccinations will expire on 4/29/25. At the time of the visit, three children ranging in age from three months to five years old were present. There are four children enrolled, only one is not related to the operator. The outdoor play area is in the front yard. There is a low ornamental fence around the side bordering the street as well as the side opposite the driveway. The provider stated that she brings a variety of gross motor equipment from the backyard to the front yard for the children to play on, including a seesaw, slide, and climbing cubes. She stated that the children do not currently play in the backyard because there is an old swing set that has fallen into disrepair and needs to be removed. To ensure each child’s file is complete, you may reference the children’s file checklist available on the DCDEE website under the “Provider Documents and Forms” tab. Requiring all forms at the time of enrollment will help you stay in compliance. CONSULTATION -I recommend using the “NC Foundations for Early Learning and Development” document on the DCDEE website when planning children’s activities to be sure you address all five domains of child development. -The NC Department of Public Health is now requiring testing for lead in water in FCCH. You may visit cleanwaterforcarolinakids.org to obtain testing information. We used a checklist to monitor all applicable requirements, including but not limited to supervision, capacity, health and safety requirements, safe sleep requirements, equipment and furnishings, and program records. The children were observed arriving at the home toward the end of the visit. The caregiver greeted them and held the baby while the older children played with toys and watched tv. The provider does not transport children; therefore, this requirement was not monitored. No enrolled children have medications or medical care plans, so these requirements were not monitored. The provider verified that all the information on the FCCH Verification of Required Information for Operator and Additional Caregivers form was correct. INDOOR/OUTDOOR ENVIRONMENT Inside the home, children are cared for in the living room adjoining the kitchen. Basic items for learning through play are available, including home living props, books, and manipulatives. The bathroom is located within sight of the children’s play area. The outdoor play area is in the front yard. There is a low ornamental fence around the side bordering the street as well as the side opposite the driveway. The provider stated that she brings a variety of gross motor equipment from the backyard to the front yard for the children to play on, including a seesaw, slide, and climbing cubes. She stated that the children do not currently play in the backyard because there is an old swing set that has fallen into disrepair and needs to be removed. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. The last documented monthly outdoor check was on 04/29/2024. There was no outdoor inspection documented in May or June 2024. 10A NCAC .1721(e)(5)(A-F) 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. A child who enrolled on 04/30/2024 did not have a health assessment on file. GS 110-91(1); 10A NCAC 09.1721(a)(1) 1889 Products that are labeled "keep out of reach of children" without any other warnings, were not stored on a shelf or in an unlocked cabinet that is five feet above the finished floor. A spray bottle containing a sanitizing solution of bleach and water was stored within children’s reach inside an unlocked cabinet under the bathroom sink. .1719(a)(7) 1919 A First Aid Information sheet, complete with required information, was not posted for quick referral by staff. A First Aid poster was not posted in the caregiving area or available for review. .1719(a )(14) 1922 Plastic bags, toys, and toy parts small enough to be swallowed, including but not limited to materials that can be easily torn apart such as foam rubber and styrofoam, were accessible to children under the age of three. Plastic grocery bags were stored within children’s reach inside an unlocked cabinet under the bathroom sink. .1719(a)(18) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by 07/31/2024. The letter must address each violation and explain how it has been corrected. Please send this to mandy.annunziata@dhhs.nc.gov and include the following information: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today’s visit, the facility’s compliance history score was 94%. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. Failure to maintain compliance may result in an administrative action. The facility was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact the center’s overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your program. TECHNICAL ASSISTANCE -It’s important to maintain a safe environment and be aware of potential hazards for young children. Today there were plastic grocery bags and sanitizing solution left under a bathroom sink within children’s reach that could be harmful to young children if ingested. I recommend conducting a daily walk-through of the caregiving space before children arrive to remove hazards or place them at least five feet high. -It’s best practice to have easily accessible materials, including a first aid kit and instructions, that you can use in the event of an emergency. Your first aid poster was not available for reference. You can download and print a free copy of this poster from healthychildcarenc.unc.edu. Be sure to post it in your child care area. -Record keeping is an important piece of a FCCH operation. Today, an infant was missing a medical report, which is required within 30 days of enrollment. You stated that the infant was at a doctor’s appointment today and that her mother should be able to give you an updated form soon. To ensure each child’s file is complete, you may reference the children’s file checklist available on the DCDEE website under the “Provider Documents and Forms” tab. Requiring all forms at the time of enrollment will help you stay in compliance. CONSULTATION -I recommend using the “NC Foundations for Early Learning and Development” document on the DCDEE website when planning children’s activities to be sure you address all five domains of child development. -The NC Department of Public Health is now requiring testing for lead in water in FCCH. You may visit cleanwaterforcarolinakids.org to obtain testing information. TRAINING REMINDERS -Training from 07/26/24 to 07/16/24 was reviewed today and documented on the Verification of Required Information for Operator and Additional Caregivers form. -CPR and First Aid must be renewed every two years. Your CPR and First Aid expires on 08/16/2025. -Health and safety training must be renewed every five years. Your training expires on 02/08/2029. -ITS-SIDS training must be renewed every three years. Your training expires on 08/16/2026. -Criminal Backgrounds Checks for all staff members should be completed every five years. You and your husband need to complete this by 10/29/2024. RATED LICENSE STATUS The NC State Legislature has extended “hold harmless” status until new rated licensing guidelines are in place, and you may opt to keep your current license and delay your rated license assessment until the new guidelines are adopted. Please use this extra time to update your WORKS account with your education information. You can visit the North Carolina Rated License Project website (https://www.ncrlap.org) for resources regarding the rated license assessment process and the FCCH self-study.
