    Name of Operation: FIRST PRESBYTERIAN DAY SCHOOL Facility ID: 3250526 Consultant: TONI FULLER Operation Type: Center Case Number: Visit Date: 8/28/2025 Number Present: 49 Completed Date: 8/28/2025 Age: From 0 To 4 Total Minutes: 150 Time In: 09:30 AM Time Out: 12:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Rated License Assessment Announced/Unannounced: Unannounced Upon arrival the purpose of today’s visit was to monitor applicable child care requirements during a Routine Unannounced visit. After arrival and discussion with the director, the purpose of this visit changed to Rated License assessment. This center operates with a five-star license, which was issued on 4/5/22. The license was re-issued on 10/4/24 to make an abbreviation change to the name of the licensee on the license. CORPORATION STATUS: According to the NC Secretary of State web site, the corporation status of this facility is “current-active”. The last Annual Compliance visit was conducted on 2/11/25. COMPLIANCE HISTORY: The compliance history for this center for the previous eighteen-month timeframe was 94%. During today’s visit all indoor and outdoor licensed space was monitored. OBSERVATIONS: I observed the children having circle time, preparing to go outside, engaged in a variety of indoor activities, and enjoying the outdoor environment. This center has a Durham Pre-K classroom that began the 2025-2026 school year on Monday, 8/25/25. Each classroom had current activity plans posted. This center implements The Creative Curriculum in the Durham Pre-K classroom, and Play Base Learning curriculum in the other four classrooms. CHILDREN’S RECORDS: Children’s records were not reviewed today. STAFF RECORDS: During today’s visit I reviewed the files for three new staff. I also verified that all staff had current Criminal Record Check clearance and current CPR/First Aid training. TRANSPORTATION: This program does not offer transportation services. INSPECTIONS: The last documented annual fire inspection was completed on 2/27/25. The last documented sanitation inspection was conducted on 6/12/25. The last documented fire drill was conducted on 7/30/25. The director stated that a fire drill would be conducted tomorrow for the month of August. The last documented shelter-in-place drill was conducted on 6/27/25. The last documented playground inspection was completed on 8/1/25. The last documented water testing for Clean Water for NC Kids was completed on 4/4/24. The following violation was cited today: Violation Number Comment Rule 847 Parent's medication authorization did not include required information. Permission to administer emergency medication to a child in classroom space #5 expired on 7/29/25. 10A NCAC 09 .0803(4)(6-9) All violations must be corrected immediately. Please send me a signed Corrective Action letter by 9/11/25, stating how each violation has been corrected. TECHNICAL ASSISTANCE WAS PROVIDED ON THE FOLLOWING TOPICS: RATED LICENSE: During today's visit the director stated that the center wanted to be assessed for an updated star-rated license, using Pathway #3. The director completed the request for assessment during the visit. The consultant was given a copy of the center's NAEYC accreditation. CLEAN CLASSROOM FOR CAROLINA KIDS: The director stated that the asbestos abatement process had not been completed during the summer months of 2025, as planned, because the center had been unable to secure a temporary location to operate, while the asbestos work was being completed. During today’s visit I requested that the director reach out to Clean Classrooms for Carolina Kids and to put updated information in their online portal for this project. OUTDOOR ENVIRONMENT: I requested that the director monitor the area at the entrance to the fenced-in space, regarding root exposure, to eliminate tripping hazards for young children. MEDICATION: During today’s visit we reviewed the requirements for administering emergency information, including the rule that the permission slip to administer the medication could be written for a timeframe not to exceed six months. MEDICATION STORAGE: I stated that I had reached out to Environmental Health after the 2/11/25 visit. During today’s visit NC Sanitation Rule 15A NCAC 18A .2820(d) was reviewed with the director.
