Upon arrival, I was greeted by the director, Soraya Galdamez, who assisted me during today’s visit. LICENSE STATUS The center currently operates under a Notice of Compliance issued on February 7, 2020. It serves children on first shift. Prior to today’s visit, I reviewed the NC Secretary of State’s website and noted that the owner of this facility, Fellowship Baptist Church of Durham, North Carolina, Inc., is listed as current/active. INSPECTIONS/DRILLS STATUS -The last sanitation inspection was completed on 9/10/25, and the center scored a superior rating with two demerits. -The last fire inspection was completed on 11/4/24. -The last documented outdoor play area inspection was completed on 8/22/25. -The last documented fire drill was conducted on 8/5/25. -The last documented lockdown/shelter-in-place drill was conducted on 7/21/25. -The last annual compliance visit was conducted on 9/26/24. MONITORING I completed a walk-through of the center, visiting five classrooms as well as the outdoor play area. The infants were being fed bottles and napping. The toddlers and preschoolers were observed playing on the playground. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records. Five staff and six children’s files were reviewed. This program does not provide transportation; therefore, this requirement was not monitored. I reviewed the expirations dates, permission forms, and storage of all medications and diaper creams and found them in compliance. The center serves children under one year of age, thus, safe sleep requirements were monitored and found in compliance. VIOLATIONS The following violation was observed and documented during today’s visit: Violation Number Comment Rule 1947 An individual on a provisional criminal background check status was not supervised by a staff person who was determined to be qualified. One of the teachers working alone in a classroom for preschool children had a provisional qualification letter and was not supervised by another qualified staff member. G.S. 110-90.2(b) & .2703(f) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by September 25, 2025. The letter must address each violation and explain specifically when and how it was corrected, as well as how you will prevent the same violations from happening in the future. Please send this to mandy.annunziata@dhhs.nc.gov and include the following information: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today’s visit, the center’s compliance history score was 98%. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. Failure to maintain compliance may result in an administrative action. The center was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact the center’s overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your center. TECHNICAL ASSISTANCE -For the safety of the children, all staff are required to have a valid qualification letter on file before being left alone with children. Today, one of your teachers who was working alone in a classroom had a provisional qualification letter, which requires supervision by another qualified staff member. You stated that the teacher had moved from another state and that state background check is delaying the full qualification letter. You said you will work in the classroom with the teacher until she is fully qualified. Teachers with a provisional qualification letter may not supervise a group of children alone. In the future, please place all teachers with a provisional qualification letter in a classroom with a teacher who has valid qualification letter. CONSULTATION -The Clean Classrooms for Carolina Kids Program provides free facility-wide testing for lead in water and inspections for asbestos and lead-based paint hazards without any out-of-pocket costs. Your water was tested on 9/19/23. Two taps tested positive for lead levels above 1 ppb, and voluntary mitigation measures including faucet fixture replacement and/or certified filter installation were recommended. Water used for drinking and cooking should be tested every three years. -You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please review and update your EPR plan yearly. -Criminal Backgrounds Checks for all staff members should be completed every five years. One staff member’s (G.C.) qualifying letter will expire 2/11/26. To access the background check portal, go to https://ncabcms.nc.gov/DCDEE/Applicant. You may also contact the CBC Unit via telephone (919) 814-6401 or by email DHHS.CBC.Unit@dhhs.nc.gov if you have questions or need assistance. Providers may request new background checks up to six months in advance of their qualifying letter’s expiration date. TRAINING REMINDERS -CPR and First Aid must be renewed every two years. Several staff members are due for CPR and First Aid renewal in 2026. -ITS-SIDS training must be renewed every three years. Staff members who completed training in 2023 are due to renew in 2026. AGENCY UPDATES Please check the Division of Child Development and Early Education’s website frequently for any new policies or rules. Go to https://ncchildcare.ncdhhs.gov/, and click on the “What’s New” tab for all agency updates, including QRIS Modernization. New QRIS rules are in effect, and the Division is preparing for a gradual transition to the new requirements. Resources, forms, and training will be provided to support you if you ever decide you wish to obtain a rated license for your center. Corrine Fulp, Child Care Consultant accompanied me on this visit. Upon arrival we were greeted by Soraya Galdamez, the Director. I completed a general walk-through of the center. At the time of the visit, there were a total of forty-seven (47) children present, ranging in age from infant to five (5) years old. I observed the children completing routine tasks and resting during rest time. LICENSE STATUS Currently this center operates with a Notice of Compliance issued February 7, 2020. Prior to today’s visit I reviewed the NC Secretary of State’s website and observed that the owner of this facility, Fellowship Baptist Church of Durham, North Carolina, Inc., is listed as current/active. INSPECTIONS *The last fire drill was conducted 3/25/24. *The last emergency drill was conducted 4/3/24. *The last playground inspection was completed 3/13/24. MONITORING During today’s visit, a partial assessment was conducted, monitoring all space occupied by the children. The outdoor play environment was not monitored during today’s visit due to the rain. The staff’s qualifications letters, CPR/FA, ITS-SIDS and one (1) new staff file was reviewed. All spaces were monitored for supervision, child capacity, materials, and equipment. The following violations were observed and documented: Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. One bottle was not labeled with a name and date. 15A NCAC 18A .2804(d) 538 Baby bottles were not stored to protect from contamination. One bottle was stored in the refrigerator without a bottle top to protect from contamination. 15A NCAC 18A .2804(d) 824 Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. The fence on the back right side and the left side of the playground measured 3ft and 1/4 inches. GS 110-91(6); .0605((i) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. INFANT BOTTLES I would recommend that the teacher check all bottles for bottle caps, and proper labeling of all bottles each day before placing them in the refrigerator. FENCE The fence will need to extend up to meet the 4ft. requirement on each side of the fence on the playground. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. HEALTH CONSULTANT Ellie Morrise is the new Child Care Health Consultant. HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety trainings must be renewed at least every five (5) years. If you completed the trainings in 2019, you should begin to retake the training now. CLEAN CLASSROOMS FOR CAROLINA KIDS I encourage you to enroll in the Clean Classrooms for Carolina Kids program to identify and eliminate exposure to lead and asbestos hazards in building infrastructure. This program is an expansion of Clean Water for Carolina Kids. Facilities are now required to meet rule requirements NC 10A NCAC 41C. 1001-1007, NC 154A NCAC 18A .2816. Clean Classrooms for Carolina Kids is designed to ensure facilities meet all requirements, receive direct support throughout the participation process, and have access to communication resources. A checklist was used to note the requirements monitored today. Upon arrival I was greeted by Soraya Galdamez, the director. There was a total of 45 children present, aging in range from infant to five (5)years old. The children were observed playing in centers and with materials, participating in story time, and transitioning to lunch. LICENSE STATUS Currently this center operates with a Notice of Compliance issued February 7, 2020. Prior to today’s visit I reviewed the NC Secretary of State’s website and observed that the owner of this facility, Fellowship Baptist Church of Durham, North Carolina, Inc., is listed as current/active. MONITORING During today’s visit, a full assessment was conducted monitoring all spaces occupied by the children, including the outdoor play environment. All staff and a selection of children files were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. The facility holds a notice of compliance and does not serve children with subsidy therefore, the staff are not required to complete the health and safety trainings. Although you are not required to complete the health and safety trainings you may access the trainings in MOODLE on the DCDEE website. --PROGRAM RECORDS * The last sanitation was conducted on 2/22/23. This facility received zero (0) demerits and a “Superior” classification. *The last documented fire drill was conducted 10/23/23. *The last fire inspection was conducted 10/5/22. *The last documented shelter-in-place or lockdown drill was conducted 10/26/23. *The last documented playground inspection was conducted 10/25/23.
