Ebonie Alston-Jenkins, Owner/Operator, assisted me with the visit. CURRENT LICENSE STATUS Currently this fcch operates with a four-star license issued July 19, 2017, earning seven points in Staff Education, two points in Program Standards and one Quality Point. The fcch is approved for daytime and night care. There are currently no children enrolled on 2nd and 3rd shift. The license was posted, and the restrictions were in compliance. A walk-through of the fcch was completed today, monitoring all areas used by the children indoors and outdoors. The five (5) preschool children present were observed in outdoor play, in center play activities, engaged in story time with the provider, and eating lunch. *The last annual compliance visit was conducted on August 11, 2025. *The last outdoor inspection was completed on February 19, 2026. *The last fire drill was conducted on March 4, 2026. Three violations were observed today and must be corrected immediately. One was corrected during the visit. The violations are as follows: Violation Number Comment Rule 1821 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of 30 days. The safe sleep charts were not completed consistently for the infants currently enrolled. .1724(a)(8)&(f) 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. The last documented lockdown or shelter-in-place drill was conducted on 11/2/25. .1719(a )(16) & .1721(e )(7) 1922 Plastic bags, toys, and toy parts small enough to be swallowed, including but not limited to materials that can be easily torn apart such as foam rubber and styrofoam, were accessible to children under the age of three. Ziploc bags were stored in a bin by the refrigerator within the children's reach. .1719(a)(18) You are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The program’s compliance history was 98% as of 3/6/26. The violations documented must be corrected immediately. On or before March 23, 2026. I must receive a written, dated, and signed compliance letter that describes accurately and in detail how the violations were corrected. Email the letter to: Kia.Reid@dhhs.nc.gov and include the following information. -Name, position -Facility name -Facility ID number -Your Signature TECHNICAL ASSISTANCE -A walk-through of the children’s space should occur prior to arrival each day, removing any potential hazards and placing them at least five feet high or in locked storage. -I suggest setting a reminder on your calendar to help you maintain compliance with completing a shelter-in-place or lockdown drill every 3 months. -You must observe and record sleep positions for all infants who are sleeping. If the infants do not sleep document this on the safe sleep chart. Please remember that what is listed in the policy must be followed to maintain compliance. A safe sleep environment is essential in reducing the risk of SIDS. OPERATOR RECORDS Your file was monitored for criminal background qualifying letters, current CPR/First Aid, and other specialized training. The provider, Ebonie-Alston Jenkins was present and assisted me as needed. Th visit summary was reviewed with Ms. Alston-Jenkins and a copy was given to her at the conclusion of the visit. Upon arrival, I was greeted by Ms. Alston-Jenkins. I completed a general walk through of the indoor and outdoor environment. There were four (4) preschool children present, and they were observed engaged in activities with the provider and eating lunch. CURRENT LICENSE STATUS Currently this fcch operates with a four-star license issued July 19, 2017, earning seven points in Staff Education, two points in Program Standards and one Quality Point. INSPECTIONS *The last annual compliance visit was conducted on September 5, 2023. *The last documented fire drill was conducted on July 8, 2024. *The last documented outdoor inspection was completed on June 17, 2024. *The last documented lockdown drill was conducted on January 15, 2024. INDOOR ENVIRONMENT In the children’s space, there are a variety of materials for the children to explore throughout the day. The bathroom is accessible to those that are potty trained. A diaper changing table is near the bathroom for those in diapers. *The outdoor play area is in the backyard of the home and is fenced. The provider informed me of the following: -Medication is not administered at this time. -There have not been any accident/incident reports written. -Transportation is not provided. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. The on-going training requirements were monitored from September 2023-August 2024 and was found to be in compliance. You might find it helpful to set a timer for 15 minutes when infants are sleeping and reset the timer until all infants are awake. This gives an auditory reminder to check on the infants while they are sleeping. A safe sleep environment is essential in reducing the risk of SIDS. The last Annual Compliance visit was conducted on 9/12/22. The compliance history for the previous eighteen-month time frame is 95%. This Family Child Care Home operates with a four-star license which was issued on 7/19/17. During today’s visit the indoor child care space and the outdoor environment were monitored. I observed the children preparing for naptime and napping. During the visit I received the operator verification form, enrollment worksheet, and signed statements of responsibility. The information recorded on the forms was reviewed with the operator. CURRICULUM: This Family Child Care Home implements the “Gee Whiz” curriculum. SMOKE DETECTORS: The smoke detectors were not tested during today’s visit, since the children were sleeping. TRANSPORTATION: During today’s visit the BMW with license #KCR-1575 was monitored for transportation requirements. The operator stated that on days when the attendance was low, she and the children could go on a short field trip. I reviewed the transportation requirements with the operator during the visit. INSPECTIONS: The last outdoor inspection was completed on 8/23/23. The last documented lockdown drill was conducted on 7/13/23. The last documented fire drill was conducted on 8/21/23. IN-SERVICE TRAINING: Yearly in-service training hours for the operator were tallied during today’s visit. HEALTH AND SAFETY TRAINING: The operator completed a portion of the required Health and Safety trainings again, in accordance with NC Child Care Rule 10NCAC 09. 1703 (d)(2). Please remember to complete the remaining trainings by 12/31/23. I gave the operator a form listing the required Health and Safety trainings for easy tracking. The remaining training can be used during the next Annual Compliance visit toward meeting yearly in-service training requirements. The following violations were cited today. Violation Number Comment Rule 1409 Operator did not provide a physically safe and healthy indoor and outdoor environment that meets the developmental needs of the children in care. The other items on the playground needed to be corrected: (1) the doors to the shed were missing (2) there were too many items in the fall zones around the swings for safe play; (3) there were adult shovels, hoes, etc. accessible to the children: (4) there were two lawn mowers accessible to the children; (5) were was debris that needed to be removed from the area; (6) were was a plastic tarp lying on the ground in the fenced-in space; (7) that was a twin size Little Tykes bed frame standing on end in the fenced-in space; (8) the fencing was in disrepair in one section; (9) the doors to the storage shed were missing. 10A NCAC 09 .1719 (a) 1831 A valid qualification letter was not on file and available for review at the facility. There was no CRC clerance letter on file for the operator's son, who turned 16 on 6/10/23. G.S. 110-90.2 (b) & (d) & .2703(e) .1702(b)(1) 2030 Operator did not provide a written statement to parents regarding the smoking and tobacco restriction. One child needed a signed statement on file. .1719(a)(11) All violations must be corrected immediately. Please send me a signed Corrective Action letter by 9/19/23, stating how each violation has been corrected. TECHNICAL ASSISTANCE WAS PROVIDED TODAY IN THE FOLLOWING AREAS: MENUS: Please be diligent in keeping menus dated. OUTDOOR ENVIRONMENT: Today I talked with the operator about the outdoor environment concerns. The operator stated that she had applied for Shape NC and Expansion and Assess grant programs for assistance in enhancing the outdoor environment. CRIMINAL RECORD CHECK: During today's visit I verified that the operator's son had completed the CRC process. During today's visit the operator texted her son at his school. The son responded that he had received the clearance letter via email. The operator will print the clearance letter and place it on file. RATED LICENSE: The three-year Rated License assessments are resuming. Your Family Child Care Home will begin the Rated License reassessment process 7/1/24. I explained that if the operator wanted to complete her self-study over the next three months, we could complete the Rated License assessment process during the next visit, which would occur in approximately six months.
