Upon arrival, I was greeted by the provider, Taryn Worley, who assisted me during today’s visit. A second caregiver, Freda Channelle, also was present. LICENSE STATUS The family child care home currently operates with a Four Star license issued on December 4, 2018. The program received seven points for Staff Education, two points for Program Standards, and one Quality Point. INSPECTIONS/DRILLS STATUS -The last annual compliance visit was conducted on 1/15/25. -The last documented monthly outdoor play area inspection was completed on 1/1/25. -The last documented monthly fire drill was conducted on 12/1/25. -The last documented quarterly lockdown/shelter-in-place drill was conducted on 10/25. MONITORING I completed a walk-through of the home, including the children’s space, kitchen, bathroom, and outdoor learning environment. At the time of the visit, there were three children present ranging in age from two years to four years old. There are seven children enrolled on first shift, eight on second shift, and seven on third shift. The children were observed listening to a book, playing with playdough, playing outside, eating lunch, and resting. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, capacity, health and safety requirements, equipment and furnishings, and program records. The provider does transport children; therefore, this requirement was monitored and found in compliance. No enrolled children have medications or medical care plans, so these requirements were not monitored. Children 12 months or younger are enrolled; therefore, safe sleep requirements were monitored and a violation was cited. The provider verified that all the information on the FCCH Verification of Required Information for Operator and Additional Caregivers form was correct. INDOOR/OUTDOOR ENVIRONMENT Inside the home, children are cared for in a room to the right of the home’s entrance. A variety of items for learning through play are available, including manipulatives, blocks, books, art materials, and home living props. The bathroom is located within the children’s play area. The provider stated they had recently rearranged the caregiving space and added more toys with help from a technical assistance specialist from Early Years. The outdoor play area is a fenced front yard with a variety of gross motor equipment. The outdoor area has undergone lead abatement to remove lead from the soil. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 919 Accurate records were not maintained for all staff and children. Four children did not records available for review. G.S. 110-91(9) 1821 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of 30 days. Sleep logs had not been kept for two infants. .1724(a)(8)&(f) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by January 26, 2026. The letter must address each violation and explain specifically when and how it was corrected, as well as how you will prevent the same violations from happening in the future. Please send this to mandy.annunziata@dhhs.nc.gov and include the following information: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today’s visit, the facility’s compliance history score was 97%. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. Failure to maintain compliance may result in an administrative action. The facility was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact your overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your program. TECHNICAL ASSISTANCE -Providers are required to maintain files for all children in their care. Today, you were missing files for four children, including two infants and two other children under your guardianship. You said one infant is a drop-in student, and the other has only attended a couple of days since enrolling in December 2025. You said you weren’t sure if you needed to maintain files for children living with you. I clarified that all enrolled preschool children should have records on file. Please use the children’s file checklist to be sure you have all required information on file when children attend on their first day. -When caring for infants, it’s important to follow safe sleep practices because the consequences for failing to follow procedures can be life-threatening. One required component of safe sleep policy is visually checking infants and documenting these sleep checks on a sleep log, which must be retained for 30 days. Today, you were missing the sleep logs for two infants. You stated that these infants have not been attending regularly. Even if they are drop-in students, you must still maintain sleep logs if they are napping at your facility. On the days that they attend but do not nap, please document it on the sleep log for that day. CONSULTATION -We reviewed QRIS information and completed a form stating that you will pursue Pathway #2, with a planned license transition date of July 2026. -The Clean Classrooms for Carolina Kids Program provides free facility-wide testing for lead in water and inspections for asbestos and lead-based paint hazards without any out-of-pocket costs. You had an identified lead hazard in your drip line. You completed the lead abatement in your outdoor learning environment in October 2025 and submitted your report to RTI. Your water was tested for lead on 11/25/24 and found within acceptable limits. -You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please review and update your EPR plan yearly. -Criminal Backgrounds Checks for the provider and all household members 16 years of age and older should be completed every five years. TRAINING REMINDERS -Training from 1/15/25 to 1/12/26 was reviewed today and documented on the Verification of Required Information for Operator and Additional Caregivers form. All required training hours for this year have been completed, and four hours will carry over to next year. -CPR and First Aid must be renewed every two years. Your CPR and First Aid certification expires 1/2/28. -ITS-SIDS training must be renewed every three years. Your training expires on 1/2/29. -Health and safety training must be renewed every five years. Your training expires 3/12/27. -You are due to renew “Recognizing and Responding to Suspicions of Child Maltreatment” (part of the Health and Safety Training) now. This training must be renewed every five years and can be found on the Positive Child Alliance NC website: https://preventchildabusenc.org/online-trainings/ -You inquired about possibly increasing your capacity. I will email you the capacity increase requirements later this afternoon. RATED LICENSE STATUS New QRIS rules are in effect, and the Division is transitioning programs to their new rated license in 2026. Resources, forms, and training will be provided to support you in preparing for new pathway options to earn your Two- through Five-Star Rated License. A transition plan will be created for your facility with a target month for your rated license assessment. I completed a general walk-through of the children’s space, outside play area, kitchen area and bathroom. At the time of the visit, there were three (3) children present aging in range from two (2) to four (4) years old. The children were observed having rest time and completing routine tasks. LICENSE STATUS The Family Child Care Home (FCCH) operates with a Four (4) star license issued December 4, 2018 earning seven (7) points in staff education, two (2) points in program standards and one (1) quality. The facility is approved for first, second and third shift with a maximum of five preschool children. MONITORING During today’s visit, a full assessment was conducted monitoring all spaces occupied by the children, including the outdoor play environment. The provider’s and children files were reviewed. All spaces were monitored for supervision, child capacity, materials and equipment. The provider does have a child under 12 months enrolled, therefore safe sleep requirements were monitored. No children enrolled have medications or medical care plans, so these requirements were not monitored. Transportation is not provided; therefore, these requirements were not monitored. The FCCH provider verified the information on the Verification of required information for operator and additional caregiver’s form to be accurate. --PROGRAM RECORDS *The last documented fire drill was conducted 2/1/24. *The last documented shelter-in-place or lockdown drill was conducted 1/2/24. *The last documented playground inspection was conducted 2/1/24. RATED LICENSE STATUS On Monday June 12, 2023, the hold harmless legislation was extended and signed into law. All facilities are allowed to remain at their current star level without reassessment until June 30, 2024. Your facility has been determined to be in Cohort 1. Your reassessment year will be between July 1, 2024, and June 30, 2025. I discussed with the provider to consider going through FCCERS-R assessment during May of 2024. I also discussed that if the FCCERS-R assessment is completed during the prep year and the scores are liked they can be used immediately upon the administrators request or be held until the actual assessment year. The provider stated that she did not want to go through the FCCERS-R assessment and the three-month self-study has been completed. I stated to the provider I will be emailing her the Rated License Application to be completed and emailed back to me. HEALTH CONSULTANT Ellie Morrise is the new Child Care Health Consultant. CRIMINAL BACKGROUND CHECK Individuals who turn 16 that lives in the home must complete the criminal background check process. I would recommend reviewing the ages of all who live in the home to make sure that you remain in compliance. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. RATED LICENSE STATUS On Monday June 12, 2023, the hold harmless legislation was extended and signed into law. All facilities are allowed to remain at their current star level without reassessment until June 30, 2024. Your facility has been determined to be in Cohort 1. Your reassessment year will be between July 1, 2024, and June 30, 2025. I discussed with the provider to consider going through FCCERS-R assessment during May of 2024. I also discussed that if the FCCERS-R assessment is completed during the prep year and the scores are liked they can be used immediately upon the administrators request or be held until the actual assessment year. The provider stated that she did not want to go through the FCCERS-R assessment and the three-month self-study has been completed. I stated to the provider I will be emailing her the Rated License Application to be completed and emailed back to me. HEALTH CONSULTANT Ellie Morrise is the new Child Care Health Consultant. CLEAN CLASSROOMS FOR CAROLINA KIDS I encourage you to enroll in the Clean Classrooms for Carolina Kids program to identify and eliminate exposure to lead and asbestos hazards in building infrastructure. Upon arrival I was greeted by the provider, Taryn Worley. I completed a walk-through with you. At the time of the visit, there were a total of five (5) children present, ranging in age from one (1) to four (4) years old. I observed the children playing in centers, eating lunch, completing routine tasks, and preparing for rest time. LICENSE STATUS Currently, this Family Child Care Home (FCCH) operates with a Four (4) star license issued December 4, 2018. INSPECTIONS *The last fire drill was conducted 9/1/23. *The last emergency drill was conducted 7/3/23. *The last playground inspection was completed 9/1/23. MONITORING During today’s visit, a partial assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. The staff’s qualifications letters were monitored as well as their CPR/FA. All spaces were monitored for supervision, child capacity, materials, and equipment. The following violations were observed and documented. Violation Number Comment Rule 810 Refrigerator was not maintained at a temperature of 45 degrees Fahrenheit or below and/or was not monitored by a refrigerator thermometer. The refrigerator maintained a temperature of 48 degrees Fahrenheit during today's visit. 10A NCAC 09 .1725(a)(10) 1876 The operator did not review the EPR Plan annually or when information in the plan changed to ensure all information is current. The last annual update and review of the EPR plan was conducted on 5/2022. .1714(e ) 1889 Products that are labeled "keep out of reach of children" without any other warnings, were not stored on a shelf or in an unlocked cabinet that is five feet above the finished floor.
