The purpose of today’s visit was to monitor applicable child care requirements during the Annual Compliance visit. This Family Child Care Home operates with a one-star license, which was issued on 3/11/13. The last Annual Compliance visit was conducted on 1/7/25. The compliance history for the previous eighteen-month timeframe was 86%. OBSERVATIONS: There were 5 preschool age children and one school age child present today. I observed the children preparing for naptime and resting during naptime. The operator had a daily schedule posted, along with current menus, and activity plans. CHILDREN’S RECORDS: The Children’s records were reviewed today. OPERATOR’S FILE: Upon arrival the operator gave me the completed enrollment worksheet, signed statements of understanding, and operator verification form. The three documents were reviewed with the operator. The operator will complete 1.5 hours of in-service by 1/7/26 to complete her training requirement for this year. TRANSPORTATION: This Family Child Care Home does not offer transportation services. SMOKE DETECTORS: Since it was naptime, the smoke detectors were not tested during today’s visit. INCIDENT REPORTS: The operator stated there had been no injuries to record in the incident log. INSPECTIONS: The last documented fire drill was conducted on 12/4/25. The last documented lockdown drill was conducted on 10/6/25. The last outdoor inspection was conducted on 11/8/25. The operator completed the Clean Water for NC Kids on 3/13/25. The operator stated that she had completed and submitted her answers to Clean Classrooms for Carolina Kids. She is waiting for a response from them regarding asbestos and lead based paint. The following violation was cited today: Violation Number Comment Rule 1409 Operator did not provide a physically safe and healthy indoor and outdoor environment that meets the developmental needs of the children in care. A wooden panel had been attached to a fence gate to cover fencing wire ends. The wooden panel had deteriorated and the wood was splintering. 10A NCAC 09 .1719 (a) All violations must be corrected immediately. Please send me a signed Corrective Action letter by 12/30/25, stating how the violation has been corrected. TECHINCAL ASSISTANCE WAS PROVIDED DURING THE VISIT ON THE FOLLOWING TOPIC AREAS: OUTDOOR ENVIRONMENT: We discussed that the best way to deal with the deteriorating wood that covered a damaged area on an interior fence gate in the backyard was to remove the gate. ADMINISTERING MEDICATION: One child had medication and paperwork on file for asthma. The parent had completed the paperwork on 12/1/25 for a timeframe of 12/1/25 to March 2027. I explained that a parent could complete the paperwork for a timeframe not to exceed 6 months. After six months, the parent must complete the paperwork again, in accordance with NC Child Care Rule 10 NCAC 09. 1720. This Family Child Care Home operates with a one-star license, which was issued on 3/11/13. The last Annual Compliance visit was conducted on 1/7/25. The compliance history for the previous eighteen-month timeframe was 90%. Designed indoor and outdoor child care space was monitored today. OBSERVATIONS: There were three preschool children present today. I observed the children engaged in a variety of indoor activities and preparing for lunch. Lunch today consisted of fish sticks, crackers, peas, fruit, and milk. I observed the daily attendance sheet and current activity plans. CHILDREN’S RECORDS: Children records were not reviewed today, since no new children had enrolled since the 1/7/25 Annual Compliance visit. I did verify that the operator had permission for walking trips to the neighborhood park. The operator reviewed the items that she carried in the cart for all walking trips to the park. OPERATOR’S FILE: I verified that the operator had current Criminal Record Check clearance and CPR/First Aid training. The operator stated that she had an appointment to begin the Criminal Record Check re-qualification process next week, while the Family Child Care Home was closed. I stated that I would monitor for yearly in-service training during the next Annual Compliance visit. INSPECTIONS: The last documented fire drill was conducted on 5/6/25. The last documented shelter-in-place drill was conducted on 5/30/25. The last outdoor inspection was conducted on 6/2/25. The operator stated that upstairs were scheduled for the playground next week, while the Family Child Care Home was closed for her vacation week. The following violation was cited today. Violation Number Comment Rule 903 When medication is administered, authorization for the operator to administer the specific medication according to the parent's or health care professional's instructions was not on file and available. MEDICATION: A violation was cited during the 1/7/25 visit, regarding required paperwork to administer Albuterol to a child. To correct the violation, the operator had the parent complete a Medical Action Plan. During today’s visit I explained that the parents also needed to complete and date the medical authorization form. The form has space at the bottom for the operator to document when treatments are given to the child. Clean Water for Carolina Kids: The operator stated that she received the water bottles, completed the steps, and returned the bottles. She further stated that she had not received any response, even after emailing them and trying unsuccessfully to call them for test results. I suggested that the operator email Clean Water for Carolina Classrooms consistently, requesting a reply. Clean Classrooms for Carolina Kids: The operator stated that she had not completed the steps, because she thought she had to wait until after clearance was received from lean Water for Carolina Kids. I stated that the two projects were separate. The operator stated that she would watch the video and answer the questions for Clean Classrooms this weekend. GS 110-102 1714 The written plan of care did not specify the names of individuals who will be responsible for the care of children when the operator attends to routine tasks. One child did not have a Written Plan of Care on file. .1712(e )(2) 2030 Operator did not provide a written statement to parents regarding the smoking and tobacco restriction. One child needed verification of the no-smoking policy on file. .1719(a)(11) All violations must be corrected immediately. Please send me a signed Corrective Action letter by 1/21/24, explaining how each violation has been corrected. AGES OF PRESCHOOL AGE CHILDREN: One of the children present today turned five years old on 1/2/25. The operator stated that she had been told that as soon as a child had his/her fifth birthday, the child was considered a school age child. I explained that a child became school age when the child could begin attending Kindergarten. The child, age 5, present today will be eligible to attend Kindergarten in August 2025. AGENCY UPDATES: Child Care facilities are required to test their drinking and cooking water for lead every three years. For information you should contact Clean Water for Carolina Kids www.cleanwaterforcarolinakids.org. The operator completed the video on 3/6/24 and then experienced difficulty in continuing with the steps. I verified today that the FCCH is in the database for the project. The operator will contact the organization again to request the water testing kit. Child care facilities are now required to test for lead and asbestos hazards in their buildings. The Clean Classrooms for Carolina Kids program is designed to identify and eliminate exposure to lead and asbestos hazards in building infrastructure, while providing child care facilities with the support needed to meet all rule requirements. The program is free, and the first step is to watch the pre-enrollment webinar, which are available at www.cleanwaterforUSkids.org/carolina. The operator stated she would begin the steps to complete the project. Since a violation of over-enrollment was cited today, an unannounced follow-up visit will be conducted soon to monitor the number of children present. This documentation was completed on the computer, reviewed the owner, and a copy of the visit summary documentation was given to her during the visit Upon arrival, I were greeted by the provider. I completed a general walk through of the children's space, kitchen, bathroom, and outdoor play area. There were four (4) preschool children present, and they were observed in routine care, eating breakfast, and outdoor play. CURRENT LICENSE STATUS Currently this family child care home operates with a one-star license issued March 11, 2013. INSPECTIONS The last fire drill was completed on 7/10/23. The last shelter-in-place drill was completed on 4/6/23. The outdoor inspection was completed on8/5/23. The last annual compliance visit was conducted virtually on 2/21/23. INDOOR ENVIRONMENT In the children’s space, there are a variety of manipulative, books, pretend play toys, and art materials for the children to be creative with throughout the day. The bathroom is accessible to those that are potty trained. *The outdoor play area is in the backyard of the home and is fenced. MONITORING During this visit, a partial assessment of Child Care Requirements was conducted. Monitoring included Supervision, CPR/FA, and ITS-SIDS training, General Licensing requirements, Criminal Background Checks, Administering of Medication, Storage of Hazardous Substances, Storage of Medication, Discipline, Program Records, License Posted, Fire Safety, Sanitation, and Capacity. The following violation was observed, documented and technical assistance was provided in maintaining compliance. No violations of child care requirements were cited on the visit. Violation Number Comment Rule 923 Operator did not keep records of monthly fire drills, quarterly lockdown or shelter-in-place drills, incident reports, incident logs, accurate attendance, emergency numbers, emergency preparedness and response plan, field trip/transportation permission, pet vaccinations records and/or playground inspections monthly outdoor safety checks for a minimum of one year. The last shelter-in-place drill was conducted on 4/6/23. .1721 (e)(7) 1401 Prescribed medications were not stored in their original containers in which they are dispensed with the pharmacy label. A container of Albuterol Sulfate was not in the original container. 10A NCAC 09 .1720(c)(1)&(2) 1403 Prescription or over-the-counter medication and topical, non-medical ointment, repellant, lotion, cream, fluoridated toothpaste, or powder was administered to any child without the required information. A container of Desitin cream approved to administer expired on 1/2023. 10A NCAC 09 .1720(c)(1&2) Please submit a corrective action letter for the above violations. While the Division allows you time to explain how you have corrected violations when they are documented, it is expected that you correct all violations immediately. TECHNICAL ASSISTANCE The information below was shared with you to maintain quality within your program and to prevent future violations. SHELTER IN PLACE DRILL Please conduct a shelter-in-place/lock down drill by the end of this month. Please note, shelter-n-place/lockdown drills are to be conducted every three (3) months. PLAYGROUND The outdoor play area needs to have the mulch redistributed around the swing/climbing structure fall zones. Mulch should be at least 6 inches deep around the fall zones.
