    Name of Operation: EASTWAY SAC CLUB Facility ID: 32001388 Consultant: TONI FULLER Operation Type: Center Case Number: Visit Date: 4/16/2025 Number Present: 16 Completed Date: 4/16/2025 Age: From 5 To 12 Total Minutes: 100 Time In: 02:00 PM Time Out: 03:40 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced Upon arrival I was greeted by the site manager. The purpose of today’s unannounced visit was to monitor applicable child care requirements for a Routine Unannounced visit. The last Annual Compliance visit was conducted on 10/8/24. This facility operates with a five-star license, which was issued on 11/25/19. This facility operates on a “year-round” school schedule. The compliance history for the previous eighteen-month timeframe was 96%. OBSERVATIONS: During today’s visit the indoor primary space (cafeteria and gymnasium) and the designated outdoor space were monitored. This facility operated with one group of children. I observed the children arriving for afterschool care. After attendance was taken, the children went with the group leader outside to play, while the car-riding children were waiting in the cafeteria for pick-up. The group leader finished setting up the activity areas while the children ate their afternoon snack. Snack today was Cheetos and 100% juice. The following activity areas were set-up for the children: music, books, arts/crafts, manipulatives, board games, blocks, math, science, dramatic play, and woodworking. The required information was posted on the Parent Information Board to include current activity plans and current snack menus. The staff/child ratio chart was posted. OUTDOOR ENVIRONMENT: During today’s visit I observed that the stationary climber was still in disrepair. I observed that the climber was wrapped in caution tape. The site manager stated that the climber was off-limits for the afterschool children and during the operating school day. The site manager stated that the afterschool children used the open field for gross motor play. During today’s visit I observed a bin of hoola hoops, footballs, soccer balls, basketballs, bats, and jump ropes for the children to use outside. This facility has the following restriction on their license: PLAYGROUND AREA DOES NOT MEET CHILD CARE STANDARDS. Based on this restriction the outdoor play area was monitored for health and safety concerns only. CHILDREN’S RECORDS: Children’s records were not reviewed today. STAFF FILES: Staff files were not reviewed today. There were no new staff since the 10/8/24 Annual Compliance visit. INSPECTIONS: The last annual fire inspection was conducted on 2/28/25. The last sanitation inspection was conducted on 3/11/25. The last fire drill was conducted on 4/7/25. The last documented lockdown drill was conducted on 2/3/25. The last documented outdoor inspection was completed on 4/15/25. The condition of the climbing structure was included in the inspection report. The following violation was cited today: Violation Number Comment Rule 1756 Enhanced staff/child ratios and group sizes were not met. There were 16 children (ages 5 and above) with one group leader. 10A NCAC 09 .2818 The violation was corrected during the visit; the site manager worked with the group leader until some of the children left for the day. Since a violation of staff/child ratios was cited today, a follow-up visit will be conducted to monitor staff/child ratios. TECHNICAL ASSISTANCE was provided in the following areas: STAFF/CHILD RATIOS: During today’s visit I discussed the staff/child ratio violation with the program administrator by phone. (1) With 7 points in programming on the facility’s five-star license, they must maintain the highest voluntary enhanced staff/child ratio requirements. The restrictions on the license read “enhanced staff/child ratios” instead of “highest voluntary enhanced staff/child ratios” in error. The program administrator stated that he was aware that Eastway SAC Club needed to follow the highest voluntary enhanced staff/child ratios. I explained that the group was out of ratio today, because there were 16 children present (ages 5 and above) with one group leader. When children aged 5 are present in a group, the staff/child ratio cannot exceed 14 children with one group leader. During today’s visit I gave the site manager a copy of the staff/child ratio chart, because the information on the posted personalized staff/child chart was not correct. RATED LICENSE: Rated License reassessments are not required at this time. Hold harmless has been extended until the new QRIS process has been developed and ready for implementation (SB 425). The purpose of today’s unannounced visit was to monitor applicable child care requirements during the Annual Compliance visit. The last Annual Compliance visit was conducted on 10/4/23. This facility operates with a five-star license, which was issued on 11/25/19. This facility operates on a year-round school schedule. The compliance history for the previous eighteen-month timeframe was 98%. During today’s visit the indoor primary space (cafeteria and gymnasium) and the designated outdoor space were monitored. OUTDOOR ENVIRONMENT: This facility has the following restriction on their license: PLAYGROUND AREA DOES NOT MEET CHILD CARE STANDARDS. Based on this restriction the outdoor play area was monitored for health and safety concerns only. OBSERVATIONS: This facility operated as one group of children. Upon arrival for afterschool care, the children went to the bathroom and then played outside until 3:00 pm. The “car-riding” children operated the cafeteria and gymnasium until 3:00 pm. At 3:00 pm the tables were cleaned. The children ate afternoon snack, while the manager set out the activity areas. The following activity areas were available today: music, books, privacy space, arts/crafts, geography, manipulatives, board games, blocks, and housekeeping. for the afterschool children. The site manager stated that the children were allowed to eat food from home to supplement the provided after snack. The required information was posted on the Parent Information Board to include current activity plans and current snack menus. CHILDREN’S RECORDS: Five children’s records were reviewed for health and safety requirements. STAFF FILES: Staff files were reviewed, and yearly in-service training hours were tallied for the manager. INSPECTIONS: The last annual fire inspection was conducted on 11/13/23. The last sanitation inspection was conducted on 4/15/24. The last fire drill was conducted on 10/7/24. The last documented lockdown drill was conducted on 8/13/24. The last documented outdoor inspection was completed on 10/7/24. The following violations were cited today: Violation Number Comment Rule 508 Special diet or food allergy information was not posted where they can be seen in food preparation and eating areas. One child was allergic to eggs, milk, and peanuts. The information was not posted in the cafeteria (eating area). .0901(g) 807 A safe indoor and outdoor environment was not provided for the children. (1) The rubberized coating for the steps on the climber were torn and loose on multiple steps. (2) The blinds on the low windows in the cafteria were broken in numberous places. 10A NCAC 09 .0601(a) 1308 Application did not include the names of individuals to whom the center may release the child, as authorized by the person who signs the application. One child did not have the information on the application. .0801(a)(7) Violations must be corrected immediately. Please send me a signed Corrective Action letter by 10/22/24, stating how the violation cited today has been corrected. TECHNICAL ASSISTANCE was provided in the following topic areas: SAFE ENVIRONMENT: Technical Assistance was provided on temporary solutions for the cafeteria blinds. I observed that a work order had been submitted for the climber on 3/14/24. I asked the site manager to talk with the administrator about the climber. CRIMINAL RECORD CHECK: The site manager was aware that her Criminal Record Check clearance would expire on 10/11/24. She stated that she had begun the requalification process. PLAYGROUND SAFETY TRAINING: The site manager stated that she had completed the Playground Safety training a few years ago under Lisa Wilson. The site manager will locate the training certificate and place it on file for review during the next visit. RATED LICENSE: Rated License reassessments are not required at this time.
