    Name of Operation: EASLEY SAC CLUB Facility ID: 32001474 Consultant: KIA REID Operation Type: Center Case Number: Visit Date: 10/28/2024 Number Present: 25 Completed Date: 10/28/2024 Age: From 5 To 10 Total Minutes: 150 Time In: 01:15 PM Time Out: 03:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today's visit was to monitor child care requirements during your routine unannounced visit. Latasha Holmes, Site Manager, was present and assisted me when needed. The visit summary was reviewed with Ms. Holmes, and a copy was given to her at the conclusion of the visit. Upon arrival, I checked into the main office then proceeded to the gym to meet Ms. Holmes. I completed a general walk through the spaces used by the children and outdoor play areas. Although this DPI program does not want their playground to be monitored for child care requirements, I did check for safety concerns. The children were arriving to the program, transitioning to activities, and eating afternoon snack. CURRENT LICENSE STATUS Currently this program operates with a five-star license issued March 3, 2020, earning 5 points in the Staff Education Component, 7 points in the Program Standards Component and 1 Quality Point. The facility operates after school care from 2:15pm-6:00pm. Currently the children have access to the gym, cafeteria, music and art rooms. The bathrooms are in the hallway outside of the cafeteria. INSPECTIONS *The last annual compliance visit was conducted on May 2, 2024. *The last sanitation inspection was completed on April 15, 2024. *The last documented lockdown drill was completed on August 6, 2024. *The last documented fire drill was conducted on October 7, 2024. During this visit, a partial assessment of Child Care Requirements was conducted. The items monitored were Supervision, Staff/Child Ratio, CPR/FA, Special training, Criminal Background Checks, Emergency Medical Care Plan, Storage of Hazardous Substances, Storage of Medication, General Safety, Discipline, Adequate/Approved Space, Program Records, License Posted, and Permit Restrictions. The following violation was cited. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. Outside on the playground a long cord is attached to the school building going across to the modular trailer. The cord is hanging low enough to cause an injury to the children. 10A NCAC 09 .0601(a) In order to comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be sent to me by November 11, 2024. The letter must address each violation and explain how it has been corrected. Please send this to Kia.Reid@dhhs.nc.gov and include the following: -Facility name -Facility ID number -Each item number TECHNICAL ASSISTANCE On the playground used by the children a long cord is attached to the school building across to the modular trailer that is near the play structure. This is a safety hazard for the children because the cord is hanging low enough to cause an injury to a child. The site manager stated she has reported the safety hazard to the office and they put a work order in to maintenance. STAFF RECORDS Staff records were monitored since the last annual compliance visit in May 2024 for criminal background qualifying letters, current CPR/First Aid and BSAC training. On your next annual compliance visit training hours will be monitored.
