The purpose of today’s visit was to monitor applicable child care requirements during a Routine Unannounced visit. This center operates with a five-star license, which was issued on 2/1/21. The last Annual Compliance visit was conducted on 12/3/24. COMPLIANCE HISTORY: The compliance history for this center for the previous eighteen-month timeframe was 97%. During today’s visit all indoor and outdoor licensed space was monitored. OBSERVATIONS: I observed the children having circle time, enjoying the outdoor environment, washing their hands for lunch, and eating lunch. Lunch today consisted of meatballs, mashed potatoes, diced peaches, and milk. CHILDREN’S RECORDS: Children’s records were not reviewed today. STAFF RECORDS: During today’s visit I reviewed the file for the staff person, who began employment after the 12/3/24 Annual Compliance visit. I verified that the staff had current Criminal Record Check clearance and CPR/First Aid training. Both teachers in the infant age classroom had IT’S SIDS training, along with the director. I observed that the center had Criminal Record Check clearance letters on file for the therapists that work with children at this center. I observed that current activity plans, menus, staff/child ratio chart, and food allergies were posted in each classroom. TRANSPORTATION: This program does not offer transportation services. INSPECTIONS: The last documented annual fire inspection was completed on 6/5/24. The last documented sanitation inspection was conducted on 10/23/24. The last documented fire drill was conducted on 4/25/25. The last documented shelter-in-place drill was conducted on 2/25/25. The last documented playground inspection was completed on 4/7/25. The last documented water testing for Clean Water for NC Kids was completed on 4/23/24. The director stated that the center had registered with Clean Classrooms for Carolina Kids and was working to complete the process for lead paint/asbestos clearance. The following violations were cited today: Violation Number Comment Rule 501 Meals/snacks did not comply with the Meal Patterns for Children in Child Care Programs. The lunch today needed a grain (bread) to meet the NC Child Care nutrition requirements. Bread was listed on the menu for today. 10A NCAC 09 .0901(a) 540 An individual written feeding plan was not provided by child's parent or health care provider or was not followed and posted. (omit posting for centers located in a residence). Two children in classroom space #7 were under 15 months of age and needed a feeding schedule. 10A NCAC 09 .0902(a) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. Permission to administer an Epi-Pen to a child in classroom space #2 expired on 4/14/25. .0803(6)(a-i); .0803(7)(a-g); .0803(8)(a-d) All violations must be corrected immediately. Please send me a signed Corrective Action letter by 5/13/25, explaining that the three violations cited today have been corrected. TECHNICAL ASSISTANCE was provided today in the following areas: NUTRITION: Meals for this center was catered by JMS Caterers. NC Child Care nutrition requirements include a grain for lunch. The menu indicated that bread would be served with the meal today, but bread was not delivered with the meal today. I asked the director to talk with the caterer about this matter. RATED LICENSE: Please know that the “hold harmless” status for Rated License reassessment has been extended until the new QRIS guidelines are ready for implementation. The program manager, Selena Komegay, and I conducted a walkthrough of the indoors and outdoors spaces. All spaces used for children were monitored. There were 79 children, aged 1 year through 5 years of age present today. Children were napping, free play, and participating in gross motor activities in the classroom. CURRENT LICENSE STATUS: Your center currently operates with a Five Star License, issued on February 1, 2021, earning 6 points in Education, 6 points in Program Standards and 1 Quality point for benefit package. The program Families and Communities Rising, Inc. was current and active with the North Carolina Secretary of State. INSPECTION/SAFTEY DRILL STATUS: The last sanitation inspection was completed on 11/16/23. The last fire inspection was completed on 6/13/23. The last outdoor play area inspection was completed on 12/1/23. The last fire drill was conducted on 12/1/23. The last Shelter in Place/Lockdown was conducted on 10/2/23. The last update EPR plan was on 9/13/23. The last Annual Compliance was conducted on 1/25/23. PROGRAM RECORDS: Staff on-going training requirements were assessed from January 2023 – January 2024. Sixteen children’s records were viewed during today’s visit. ITEMS MONITORED: A full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. The Staff Training Worksheet and Fire Inspection were mailed to me prior to today’s visit. Your signature on these forms will serve as verification that the information provided is complete and accurate. THE FOLLOWING VIOLATIONS WERE OBSERVED DURING TODAY’S VISIT. Violation Number Comment Rule 1314 Emergency information did not name child's health care professional. One child's file did list a hospital preference. .0802(c)(2) While the Division allows you time to explain how you have corrected violations when violations are documented, it is expected that you correct all violations immediately. When you send your compliance to me, it is to verify that you have already corrected or addressed the violations from your visit summary. If I do not receive your letter by December 26, 2023, and/or if you do not address all violations, I will be making a return visit to your program to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. The letter must address each violation, explain how it has been corrected and how it will be maintained in the future. RATED LICENSE You are currently in your preparation year; it will end on June 30, 2024. Your assessment year will start on July 1, 2024. To take advantage of the benefits now available, you must decide when you want to have the assessment done. Remember you can decide to count your score toward your assessment year if you are pleased with the result. I am leaving a copy of the Rated License Assessment Request Review with you today. Please email the form when completed. During today’s visit you noted that you would like to request for the Environmental Rating Scale to be completed in February or March. When you are ready you can send the names of any new staff to get education points. TECHNICAL ASSISTANCE: The information below was shared with you to maintain quality within your program and to prevent future violations. INFANTS As babies’ eating habits change and are introduced to solid food staff should remind parents to make the adjustments on their feeding schedule. Employees that have only high school education may want to use this pathway to meet lead teacher status. The North Carolina Early Childhood Equivalency Exam is a new pathway option comparable to a NC Early Childhood Credential. Applicants who achieve a score of 80% or higher on the NC Early Childhood Equivalency Exam earn the new North Carolina Early Childhood Equivalency Certificate. This certificate is equivalent to a North Carolina Early Childhood Credential and will qualify the individual to be a lead teacher in an early childhood classroom. WORKS Now is the time to make sure all staff are enrolled in NC WORKS and their information is current. To enroll in training and view information in the DCDEE Moodle tool, you must use your NCID - the same NCID that is used for WORKS registration, and/or the criminal background check (CBC) portal. The summary of the North Carolina Child Care Law and Rules was revised in September 2023. Please make sure you have updated your posting on the parent board. ANNUAL LICENSING FEE The Division of Child Development and Early Education is required by law [North Carolina General Statute 110-90(1a)] to establish an annual licensing fee for child care facilities. Payment must be made online on the DCDEE website using a Mastercard/Visa debit or credit card, or by e-check. You will need your Facility ID number and invoice number to complete the payment process. The Annual Licensing Fee is due by December 31, 2023. Failure to pay the license fee is a violation of child care law (N.C.G.S. 110-90(1) (a) and will result in a penalty assessment, accrued interest, and issuance of an Administrative Action, up to and including revocation of the child care license. Ms. Selena Kornegay, center director was present and assisted me during the visit. CURRENT LICENSE STATUS: Your center currently operates with a Five Star License, issued on February 1, 2021, earning 6 points in Education, 6 points in Program Standards and 1 Quality point for benefit package. The last annual compliance visit was conducted 1/25/23. A partial monitoring of applicable child care requirements was conducted on today’s visit. I did not assess transportation requirements because the program does not transport children. The following violation was observed and documented: Violation Number Comment Rule 847 Parent's medication authorization did not include required information. An authorization form for a tube of hydrocortisone did not have date and the amount to apply. 10A NCAC 09 .0803(4)(6-9) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff hired 2/6/23 did not have verification of First aide on file. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff hired on 2/6/23 did not have verification CPR completion on file. .1102(d) While the Division allows you time to explain how you have corrected violations when violations are documented, it is expected that you correct all violations immediately.
