Upon arrival, I was greeted by the provider, Deborah Royster, who assisted me during the visit. LICENSE STATUS The family child care home currently operates with a Three Star License issued on April 10, 2020. The program received two points for Program Standards, four points for Staff Education, and one Quality Point. INSPECTIONS/DRILLS STATUS -The last documented outdoor play area inspection was completed on 4/2/26. -The last documented fire drill was conducted on 3/20/26. -The last documented lockdown/shelter-in-place drill was conducted on 4/1/26. -The last annual compliance visit was conducted on 10/10/25. MONITORING I completed a walk-through of the home, visiting the caregiving space, bathroom, and outdoor play area. Two children were present, both eight months of age. The children were observed playing on the floor with the caregiver and sitting in swings and a exersaucer. A partial assessment of child care requirements was conducted. I monitored all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records. Newly enrolled children’s records also were reviewed. No enrolled children have medications or medical care plans, so these requirements were not monitored. Children 12 months of age or younger are enrolled; therefore, safe sleep requirements were monitored and a violation was cited. INDOOR/OUTDOOR SPACE Children are cared for in a room between the living room and kitchen, and a variety of materials are available for play, including blocks, manipulatives, books, and dramatic play items. The bathroom and changing table are located down the hall from the caregiving space. The unfenced outdoor play area is located in the front of the house. There are a variety of toddler slides and climbers, playhouses, a basketball goals, balls, and other equipment for gross motor play. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. An unlocked bathroom cabinet within children's reach contained hydrogen peroxide, rubbing alcohol, and an aerosol product. .1719 (a)(7) 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. To ensure you have all required forms, I recommend collecting them from the parent before you begin caring for the child. You can also use the children’s file checklist available on the DCDEE website under the “Provider” tab. -Record keeping is an important piece of a FCCH operation. Today, an infant who enrolled on 9/22/25 was missing a medical report that had been completed by a doctor, which is required within 30 days of enrollment. Upon arrival, I was greeted by the provider, Deborah Royster, who assisted me during today’s visit. LICENSE STATUS The family child care home currently operates with a Three Star license issued on April 10, 2020. The program received four points for Staff Education, two points for Program Standards, and one Quality Point. INSPECTIONS/DRILLS STATUS -The last annual compliance visit was conducted on 10/29/24. -The last documented monthly outdoor play area inspection was completed on 10/2/25. -The last documented monthly fire drill was conducted on 10/2/25. -The last documented quarterly lockdown/shelter-in-place drill was conducted on 9/8/25. MONITORING I completed a walk-through of the home, including the children’s space, kitchen, bathroom, and outdoor play area. At the time of the visit, there were two children present ranging in age from 7 weeks to one year old. There are two children enrolled. The infant was observing drinking a bottle, being held by the provide, and sleeping. The toddler was observed playing with vehicles and interacting in the provider while playing with pretend vegetables, and being read a book. The provider kept up a running dialogue with the toddler, naming objects and asking questions to extend his learning. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, capacity, health and safety requirements, equipment and furnishings, and program records. The provider does not transport children; therefore, this requirement was not monitored. Storage of diaper cream and corresponding permission forms were monitored and found in compliance. Children 12 months or younger are enrolled; therefore, safe sleep requirements were monitored and found in compliance. The provider verified that all the information on the FCCH Verification of Required Information for Operator and Additional Caregivers form was correct. INDOOR/OUTDOOR ENVIRONMENT Inside the home, children are cared for in a room between the living room and kitchen. A variety of items for learning through play are available, including manipulatives, blocks, books, and home living props. The bathroom and changing table are located down the hall from the children’s play area. Mats and a portable crib are used for rest time. The outdoor learning environment is an unfenced front yard with a variety of gross motor equipment. The child was observed playing with puzzles, singing, and reading a book with the provider. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, capacity, health and safety requirements, equipment and furnishings, and program records. The provider does transport children; however, the car used for transportation was at the auto repair shop today and unavailable for monitoring. I will return at a later date to inspect the car. Until then, you should not provide transportation. One enrolled children uses diaper cream. Medication requirements were monitored and found in compliance. Children 12 months or younger are enrolled; therefore, safe sleep requirements were monitored. The provider verified that all the information on the FCCH Verification of Required Information for Operator and Additional Caregivers form was correct. INDOOR/OUTDOOR ENVIRONMENT Inside the home, children are cared for in a space between the kitchen and living room. A variety of items for learning through play are available, including puzzles, books, art materials, home living props, and infant toys. The children eat their meals in this area. The bathroom and changing table are located within sight of the children’s play area. The outdoor play area is in the front yard of the home and is not fenced. There are a variety of gross motor play items, including a small slide. There is an enclosed play area for the infant. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 907 Written record of up-to-date pet vaccinations as required by NC law and local ordinances, including rabies vaccination for dogs and cats, was not available. No vet records or animal acknowledgment form was on file for the dog that was present today. .1719(b)(1) 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. One child's health assessment was incomplete. The parent part was filled out, but there was no physician signature or record of a health exam. GS 110-91(1); 10A NCAC 09.1721(a)(1) 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. The last documentation of attendance was on October 17, 2024. .1721(e)(6) 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). The provider's qualification letter expired on April 4, 2024. G.S. 110-90.2 & .2703(n) & (o) 1821 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of 30 days. You have 15 days to obtain a new qualifying letter; after that, you will not be able to keep children in child care until you receive your new qualifying letter. -When caring for infants, it’s important to follow safe sleep practices because the consequences for failing to follow procedures can be life-threatening. One required component of safe sleep policy is visually checking infants and documenting these sleep checks on a sleep log, which must be retained for 30 days. Today, the logs revealed that the sleep checks have not been regularly documented. There was also no signed safe sleep policy in the infant’s file. If you are having difficulty adhering to the safe sleep rules, you should consider only providing care for children over one year of age. -It is important to keep children’s medical records on file to ensure children are healthy and able to participate in activities. Today, one child’s health assessment on file had not been completed and signed by a physician. I recommend using the children’s file checklist in “Provider Documents and Forms” under the “Provider” tab on the DCDEE website and checking files regularly to help you keep track of required records. Requiring all completed forms at the time of enrollment will help you stay in compliance. -Providers are required to keep daily attendance records for all children in their care, including drop-off and pick-up times. No attendance had been documented since October 17, 2024. Accurate attendance records show that you remain in compliance with child care ratios as well as provide a list of children present in the event of an emergency. I recommend placing the attendance log next to the front door for parents (or you) to sign children in and out each day. -Fire drills are required to be conducted monthly. Your last documented drill was in August 2024. I suggest conducting all your drills and inspections on the same day of each month (for example, the 15th of every month) to help you easily remember when everything is due. -A small dog was present in the home today, but none of the children’s files contained the animal acknowledgement form. You stated that the dog belongs to your daughter and was just visiting today. You also said all parents were verbally informed. For the safety of the children at any future visits, please have all parents sign the animal acknowledgement form and also keep a copy of the dog’s shot records on hand to be sure the dog is up-to-date on all vaccines. -It is important to keep hazardous products out of reach of children and in locked storage. Today I noted cleaning products stored in reach of the changing table as well as aerosol products and sunscreen under the unlocked bathroom sink. I recommend placing all cleaning products in the bathroom in a cabinet with a metal lock (not a plastic childproof lock). Upon arrival, I was greeted by the provider, Deborah Royster, who assisted me during today’s visit. LICENSE STATUS The family child care home currently operates with a three star license issued on April 10, 2017. The program received four points for staff education, two points for program standards, and one quality point. INSPECTIONS/DRILLS STATUS -The last annual compliance visit was conducted on 12/5/23. -The last documented monthly outdoor play area inspection was completed on 10/1/24. -The last documented monthly fire drill was conducted on 8/9/24. -The last documented quarterly lockdown/shelter-in-place drill was conducted on 8/13/24. MONITORING I completed a walk-through of the home, including the children’s space, kitchen, bathroom, and outdoor play area. At the time of the visit, there was one child present who was two years old. There are three children enrolled. The child was observed playing with puzzles, singing, and reading a book with the provider. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, capacity, health and safety requirements, equipment and furnishings, and program records. The provider does transport children; however, the car used for transportation was at the auto repair shop today and unavailable for monitoring. I will return at a later date to inspect the car. Until then, you should not provide transportation. One enrolled children uses diaper cream. Medication requirements were monitored and found in compliance. Children 12 months or younger are enrolled; therefore, safe sleep requirements were monitored. The provider verified that all the information on the FCCH Verification of Required Information for Operator and Additional Caregivers form was correct. INDOOR/OUTDOOR ENVIRONMENT Inside the home, children are cared for in a space between the kitchen and living room. A variety of items for learning through play are available, including puzzles, books, art materials, home living props, and infant toys. The children eat their meals in this area. The bathroom and changing table are located within sight of the children’s play area. The outdoor play area is in the front yard of the home and is not fenced. There are a variety of gross motor play items, including a small slide. There is an enclosed play area for the infant. The facility was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact the center’s overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your program. TECHNICAL ASSISTANCE -Criminal Backgrounds Checks for all providers should be completed every five years. Your last qualifying letter expired 4/4/24 and must be renewed immediately. You have 15 days to obtain a new qualifying letter; after that, you will not be able to keep children in child care until you receive your new qualifying letter. -When caring for infants, it’s important to follow safe sleep practices because the consequences for failing to follow procedures can be life-threatening. One required component of safe sleep policy is visually checking infants and documenting these sleep checks on a sleep log, which must be retained for 30 days. Today, the logs revealed that the sleep checks have not been regularly documented. There was also no signed safe sleep policy in the infant’s file. If you are having difficulty adhering to the safe sleep rules, you should consider only providing care for children over one year of age. -It is important to keep children’s medical records on file to ensure children are healthy and able to participate in activities. Today, one child’s health assessment on file had not been completed and signed by a physician. I recommend using the children’s file checklist in “Provider Documents and Forms” under the “Provider” tab on the DCDEE website and checking files regularly to help you keep track of required records. Requiring all completed forms at the time of enrollment will help you stay in compliance. -Providers are required to keep daily attendance records for all children in their care, including drop-off and pick-up times. A checklist was used to note the requirements monitored today. The Family Child Care Home Provider, Deborah Royster, was present and assisted me during today’s visit. I completed a general walk-through of the children’s space, outside play area, kitchen area and bathroom. At the time of the visit, there were three (3) children present aging in range one (1) to two (2) years old. The children were observed playing, interacting with the provider and routine care. LICENSE STATUS The Family Child Care Home (FCCH) operates with a Three (3) star license issued April 10, 2020. The facility is approved for first, second shift with a maximum of five (5) preschool children at any time and the Fireplace/Woodstove not used during operating hours. MONITORING During today’s visit, a full assessment was conducted monitoring all spaces occupied by the children, including the outdoor play environment. The provider’s and children files were reviewed. All spaces were monitored for supervision, child capacity, materials and equipment. The provider does not provide transportation; therefore, this requirement was monitored. The provider does not have any children 12 months and younger enrolled, therefore safe sleep requirements were not monitored. No children enrolled have medications or medical care plans, so these requirements were not monitored. The FCCH provider verified the information on the Verification of required information for operator and additional caregiver’s form to be accurate. --PROGRAM RECORDS *The last documented fire drill was conducted 11/1/23. *The last documented shelter-in-place or lockdown drill was conducted 11/1/23. *The last documented playground inspection was conducted 11/2/23. CHILDREN FILES I discussed with the provider the importance of receiving the application for each child on or before the first day of attendance. The provider stated to me that today was the first day of attendance for the child that did not have an application on file. I recommend using the Children File Checklist to make sure that all forms are on file for each child. ITS-SIDS The provider asked for assistance to register for an ITS-SIDS class to renew the training. The Provider was able to register for the training scheduled for January 4, 2024. I recommend that you utilize your outlook calendar to send a reminder to renew all necessary trainings before they expire. A checklist was used to note the requirements monitored today. The Family Child Care Home Provider, Deborah Royster, was present and assisted me during today’s visit. I completed a general walk-through of the children’s space, outside play area, kitchen area and bathroom. At the time of the visit, there were three (3) children present aging in range one (1) to two (2) years old. The children were observed playing, interacting with the provider and routine care. LICENSE STATUS The Family Child Care Home (FCCH) operates with a Three (3) star license issued April 10, 2020. The facility is approved for first, second shift with a maximum of five (5) preschool children at any time and the Fireplace/Woodstove not used during operating hours. MONITORING During today’s visit, a full assessment was conducted monitoring all spaces occupied by the children, including the outdoor play environment. The provider’s and children files were reviewed. All spaces were monitored for supervision, child capacity, materials and equipment. The provider does not provide transportation; therefore, this requirement was monitored. The provider does not have any children 12 months and younger enrolled, therefore safe sleep requirements were not monitored. No children enrolled have medications or medical care plans, so these requirements were not monitored. The FCCH provider verified the information on the Verification of required information for operator and additional caregiver’s form to be accurate. --PROGRAM RECORDS *The last documented fire drill was conducted 11/1/23. *The last documented shelter-in-place or lockdown drill was conducted 11/1/23. *The last documented playground inspection was conducted 11/2/23. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. A child enrolled on 5/9/22 did not have a copy of the health assessment on file. GS 110-91(1); 10A NCAC 09.1721(a)(1) 1306 Operator has not completed ITS-SIDS training every three years from the completion date of the previous ITS-SIDS training. The provider did not renew ITS-SIDS training on or before the expiration date of 10/29/23. .1703(a)(4) 1892 Application was not on file for each enrolled child, including his/her own preschool child(ren) who are not school-age. One child enrolled that started today, 12/5/23, did not have an application on file. .1721(a)(3) To comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be received on or before December 19, 2023, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. CHILDREN FILES I discussed with the provider the importance of receiving the application for each child on or before the first day of attendance. The provider stated to me that today was the first day of attendance for the child that did not have an application on file. I recommend using the Children File Checklist to make sure that all forms are on file for each child. ITS-SIDS The provider asked for assistance to register for an ITS-SIDS class to renew the training. The Provider was able to register for the training scheduled for January 4, 2024. I recommend that you utilize your outlook calendar to send a reminder to renew all necessary trainings before they expire. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. IN-SERVICE TRAINING You are required to receive 10 training hours yearly. I reviewed your training hours from 12/19/22 to 12/5/23. Please send me five (5) additional training hours on or before 12/19/23 to remain in compliance. RATED LICENSE STATUS On Monday June 12, 2023, the hold harmless legislation was extended and signed into law. All facilities are allowed to remain at their current star level without reassessment until June 30, 2024. Your facility has been determined to be in Cohort 2.
