Upon arrival, I was greeted by the provider, Telita Thorpe-Heggins, who assisted me during the visit. LICENSE STATUS The family child care home currently operates with a Four Star License issued on May 10, 2024. The program received four points for Program Standards, six points for Staff Education, and one Quality Point. INSPECTIONS/DRILLS STATUS -The last documented outdoor play area inspection was completed on 10/15/25. -The last documented fire drill was conducted on 10/11/25. -The last documented lockdown/shelter-in-place drill was conducted on 9/15/25. -The last annual compliance visit was conducted on 3/5/25. MONITORING I completed a walk-through of the home, visiting the caregiving space and bathroom. The outdoor play area was not monitored due to inclement weather. Two children were present, ranging in age from eight months old to one year old. The children were freely playing with toys and getting a diaper change during the visit. A partial assessment of child care requirements was conducted. I monitored all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records. Newly enrolled children’s records also were reviewed. No enrolled children have medications or medical care plans, so these requirements were not monitored. Children 12 months of age or younger are enrolled; therefore, safe sleep requirements were monitored and violations were cited. INDOOR/OUTDOOR SPACE Children are cared for in the lower level of a split-level home, and a variety of materials are available for play, including home living materials, infant toys, cars and trucks, puzzles, and books. A bathroom is located within the children’s play area, and there are a small refrigerator and microwave within the space for food preparation. Cots are used for rest time. The outdoor learning environment is a fenced backyard with a variety of toys for gross motor play. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. Two children, enrolled on 7/8/25 and 9/8/25, were missing health assessments in their files. GS 110-102 1804 Safe sleep policy and poster about safe sleep practices was not posted in a prominent place in the infant sleeping room or area where parents and caregivers are able to view daily. No safe sleep policy was posted in the caregiving space. .1724(b) 1810 Operator did not retain the safe sleep acknowledgement in the child's records as long as the child was enrolled. TECHNICAL ASSISTANCE -Important information, including your safe sleep policy and Summary of NC Law, were not posted today. You stated that you had forgotten about the safe sleep policy since it had been some time since you had a baby enrolled, and you were not aware the Summary of Law needed to be posted. You printed the required copies and hung them during my visit today. -When caring for infants, it’s important to follow safe sleep practices because the consequences for failing to follow procedures can be life-threatening. One required component of safe sleep policy is visually checking infants and documenting these sleep checks on a sleep log, which must be retained for 30 days. Today, no logs were available for review. You said you had forgotten to complete them but you will print copies of the logs to use moving forward. Please also have the child's parents sign the safe sleep policy and place it in the child's file. -Record keeping is an important piece of a FCCH operation. Today, two children were missing medical reports, which is required within 30 days of enrollment. You stated that the toddler’s parent will bring the medical report today, and that you thought you have the infant’s medical report but might have misplaced it. The toddler was also missing an immunization record, which you said the parent will also bring today. To ensure each child’s file is complete, you may reference the children’s file checklist available on the DCDEE website under the “Provider Documents and Forms” tab. Requiring all forms at the time of enrollment will help you stay in compliance. CONSULTATION -We discussed the new licensure pathway options and I showed you how to access the QRIS information on the DCDEE website. Your rated license is due for renewal in 2027, and at that time you will choose whether to pursue Pathway 1, 2, or 3 to renew your license. -We discussed the requirement for having an accurate Written Plan of Care of file for all children that will allow you to still be an emergency caregiver for a family member. Please update the Written Plan of Care for all children and ask parents to sign the new copy. Children should only be in care at your licensed location unless participating in a field trip or routine activity identified on Written Plan of Care. If you need to be away from the childcare, your substitute caregiver should meet all the same training requirements as you. Parents must be made aware anytime you transport children away from the home, and you should take children’s files with you when you leave the home (include pictures of the children, emergency medical care information, and permission to transport forms). -Criminal Backgrounds Checks for all providers should be completed every five years. -The Clean Classrooms for Carolina Kids Program provides free facility-wide testing for lead in water and inspections for asbestos and lead-based paint hazards without any out-of-pocket costs. Visit www.cleanwaterforuskids.org/carolina to enroll. Providers are required to test for lead in drinking water per Child Care Rule .1725. The provider's water was tested on 5/31/24 and no mitigation was required. -You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please review and update your EPR plan yearly. TRAINING REMINDERS -Documentation of 5 hours of ongoing training will be needed by your next annual compliance visit. -CPR and First Aid must be renewed every two years. Your training expires 4/24/26. -ITS-SIDS training must be renewed every three years. Your training expires 4/24/28. -Health and Safety Training must be renewed every five years. LICENSE STATUS The family child care home currently operates with a Four Star License issued on May 10, 2024. The program received four points for Program Standards, six points for Staff Education, and one Quality Point. INSPECTIONS/DRILLS STATUS -The last documented outdoor play area inspection was completed on 10/15/25. -The last documented fire drill was conducted on 10/11/25. -The last documented lockdown/shelter-in-place drill was conducted on 9/15/25. -The last annual compliance visit was conducted on 3/5/25. MONITORING I completed a walk-through of the home, visiting the caregiving space and bathroom. The outdoor play area was not monitored due to inclement weather. Two children were present, ranging in age from eight months old to one year old. The children were freely playing with toys and getting a diaper change during the visit. A partial assessment of child care requirements was conducted. I monitored all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records. TECHNICAL ASSISTANCE -Important information, including your safe sleep policy and Summary of NC Law, were not posted today. You stated that you had forgotten about the safe sleep policy since it had been some time since you had a baby enrolled, and you were not aware the Summary of Law needed to be posted. You printed the required copies and hung them during my visit today. -When caring for infants, it’s important to follow safe sleep practices because the consequences for failing to follow procedures can be life-threatening. One required component of safe sleep policy is visually checking infants and documenting these sleep checks on a sleep log, which must be retained for 30 days. Today, no logs were available for review. You said you had forgotten to complete them but you will print copies of the logs to use moving forward. Please also have the child's parents sign the safe sleep policy and place it in the child's file. -Record keeping is an important piece of a FCCH operation. Today, two children were missing medical reports, which is required within 30 days of enrollment. You stated that the toddler’s parent will bring the medical report today, and that you thought you have the infant’s medical report but might have misplaced it. The toddler was also missing an immunization record, which you said the parent will also bring today. To ensure each child’s file is complete, you may reference the children’s file checklist available on the DCDEE website under the “Provider Documents and Forms” tab. Today, one child was missing a health assessment. I recommend using the children’s file checklist under “Provider Documents” on the DCDEE website and checking files regularly to help you keep track of required records. You might consider requiring all forms from parents before their child joins your program. -Providers who are licensed to care for infants must renew their ITS-SIDS training every three years. Your training expired on 8/30/24 and needs to be renewed immediately. You stated that you thought that class was required only if you have infants enrolled, which you do not. I clarified the rule. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. The facility was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact the center’s overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your program. TECHNICAL ASSISTANCE -Criminal Backgrounds Checks for all providers and household members ages 16 and older should be completed every five years. Your last qualifying letter expired 12/4/24, and your son’s qualifying letter expired 10/16/24. Both must be renewed immediately. You stated that you thought a prior consultant had told you that they were not due to be renewed until 2027. You have 15 days to obtain new qualifying letters; after that, you will not be able to keep children in child care until you receive your new qualifying letters. Today, one child was missing a health assessment. I recommend using the children’s file checklist under “Provider Documents” on the DCDEE website and checking files regularly to help you keep track of required records. You might consider requiring all forms from parents before their child joins your program. -Providers who are licensed to care for infants must renew their ITS-SIDS training every three years. Your training expired on 8/30/24 and needs to be renewed immediately. You stated that you thought that class was required only if you have infants enrolled, which you do not. I clarified the rule. Often these behaviors are developmentally appropriate, however, some behaviors can be difficult for a teacher to manage. Providing technical assistance, coaching and professional development for staff to prevent and address these challenging behaviors, as well as promote children’s social competence can be beneficial. If you would like assistance with dealing with challenging behaviors of children in your care, you can contact a Healthy Social Behavior Specialist at Child Care Services Association; they can be reached at (919) 403-6950. Upon arrival, I was greeted by the provider, Telita Thorpe-Heggins, who assisted me during today’s visit. LICENSE STATUS The family child care home currently operates with a four-star license issued on May 10, 2024. The program received seven points for staff education, two points for program standards, and one quality point. INSPECTIONS/DRILLS STATUS -The last annual compliance visit was conducted on 4/30/24. -The last documented monthly outdoor play area inspection was completed on 1/15/25. -The last documented monthly fire drill was conducted on 12/19/24. -The last documented quarterly lockdown/shelter-in-place drill was conducted on 11/22/25. MONITORING I completed a walk-through of the children’s space and bathroom. I could not monitor the outdoor play area due to inclement weather at the time of my visit. There were two children present ranging in age from two years to four years old. There are five children enrolled. The children were observed playing freely with materials, reading books, and dancing to music. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, capacity, health and safety requirements, equipment and furnishings, and program records. The provider does not transport children; therefore, this requirement was not monitored. No enrolled children have medications or medical care plans, so these requirements were not monitored. Children 12 months or younger are not enrolled; therefore, safe sleep requirements were not monitored. The provider verified that all the information on the FCCH Verification of Required Information for Operator and Additional Caregivers form was correct. INDOOR/OUTDOOR ENVIRONMENT Inside the home, children are cared for in the lower level of a split-level home. A variety of items for learning through play are available, including manipulatives, blocks, books, art materials, and home living props. The bathroom and changing table are located within the children’s play area, and there are a small refrigerator and a microwave within the space for food preparation. The outdoor play area is a fenced backyard with a variety of gross motor equipment, including slides, a sandbox, riding trucks, and a sand and water table.
