    Name of Operation: CRESSET CHRISTIAN ACADEMY PRE-SCHOOL Facility ID: 32000027 Consultant: SHAMEQUA WILKERSON-HARRIS Operation Type: Center Case Number: Visit Date: 6/25/2026 Number Present: 39 Completed Date: 6/25/2026 Age: From 0 To 5 Total Minutes: 165 Time In: 09:30 AM Time Out: 12:15 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to complete your Routine Unannounced visit. Upon arrival I was greeted by Shelia Hill, the Director. I completed a general walk-through of the center. At the time of the visit, there were a total of thirty-nine (30) children present, ranging in age from infant to five (5) years old. The children were doing artwork, reading books, participating in group time, playing with materials, interacting with the teachers, playing outside and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a four (4) star license issued October 8, 2021, earning four (4) points in education, six (6) points in program standards and one (1) quality point. I reviewed the NC Secretary of State’s website and observed the owner of the facility Cresset Baptist Church Life Spring Church listed as current/active. MONITORING During today’s visit, a partial assessment of Child Care Requirements was conducted for all classrooms. The monitoring included CPR/First Aid, criminal background checks and special training requirements for one newly hired staff. The monitoring also included License and permit restrictions, staff/child ratios, supervision, storage of hazardous items, medication, materials/equipment, and all space occupied by the children. The outdoor play environment was during the time of the visit. INSPECTIONS *The last fire drill was conducted on 6/9/26. *The last emergency drill was conducted on 3/4/26. *The last playground inspection was completed 5/27/26. *The last sanitation was completed on 1/12/26 with a “Superior‘’ classification. *The last fire inspection was completed on 11/19/25. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 538 Baby bottles were not stored to protect from contamination. In space #301, two bottles were observed without a covering or top to avoid contamination. 15A NCAC 18A .2804(d) 620 All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. It was observed in space #304 chipping paint in the music area near the baseboard. Children one to two years of age are cared for in that classroom. In the hallway near the two-year-old classroom hole about four inches long and wide. 15A NCAC 18A .2825(a) 871 Center staff did not comply with the safe sleep policy. In space #301 an infant was observed sleeping in a bouncy seat. 10A NCAC 09 .0606(a) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before July 9, 2026, the letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. Please make sure to include: -Facility name -Facility ID number -Each item number The letter can be emailed to Shamequa.Wilkerson-Harris@dhhs.nc.gov COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 83%. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. INFANT SLEEP POLICY In space #301 an infant was sleeping in a bouncy seat. It is important that a child is placed in his or her crib or approved sleeping device. The teacher was unaware that the child was not allowed to sleep in the bouncy seat. I would recommend that you review your safe sleep policy with all staff that work in the infant classroom. I would also have the staff member review ITS-SIDS for a refresher. You may also review resources from the American Academy of Pediatrics regarding safe sleep. INFANT BOTTLES Two infant bottles in space #301 did not have top (covering) to prevent contamination. I discussed with the teacher and the director the importance of a covering or top. The teacher stated the parents bring the bottles with no caps. I would suggest sending a memo reminder to the parents regarding your effort to prevent contamination. WALLS In space #2 the wall in the music area near the baseboard has chipping paint. Chipping paint can, if ingested to cause choking hazards and other medical issues. In the hallway near the two-year-old classroom the wall was observed with a hole in the wall. I would recommend covering or repairing the area while your center is closed over the next week. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. HEALTH AND SAFETY TRAINING Health and Safety training courses are now available through Moodle on the DCDEE website. This training courses are provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety training must be renewed at least every five (5) years. If you complete the trainings in 2021, you should begin to retake the training now. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. AGENCY UPDATES Please remember to check the Division of Child Development and Early Education website (ncchildcare.gov) on a weekly basis to stay up to date on all new policies and rules. There was a total of forty-eight (48) children present, aging in range from infants to four (4) years old. The children were observed playing in centers, interacting in circle time, preparing for lunch and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a four (4) star license issued October 8, 2021, earning four (4) points in education, six (6) points in program standards and one (1) quality point. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. New staff CPR/FA, qualification letters, a percentage of old staff and children files were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. Transportation is not provided; therefore, these requirements were not monitored. This facility does have children under 12 months enrolled, therefore safe sleep requirements were monitored and found in compliance. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. A current activity/lesson plan was not posted in space #304, #305, and #307. GS 110-91(12); .0508(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. The closet door in space #305 was observed not locked that stored Febreze Air Mist, Wet Jet swiffer solution and Wet Jet Swiffer. .2820(b) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. In space #302 diaper cream was not stored in a locked cabinet or locked container. In space #305 Benadryl and diaper cream was not stored in a locked cabinet or locked container. 15A NCAC 18A .2820(d) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. Diaper Rash permission to administer the cream expired 8/2025 and Desitin did not have a form in space #305. 10A NCAC 09 .0803(1)(a & b) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. NON-PRESCRIPTION/PRESCRIPTION I observed an expired parent authorization form in space for two’s (room 305) with an expiration date of 8/2025. I observed Mineral Sunscreen SPF 50 with an expiration date of 8/2023. A parent authorization form was observed with dates to administer from 6/2024 to 1/2026. I would recommend having parents drop all prescription and non-prescription medications at the office to be reviewed and then placed in the classrooms. I would also have a member of management to check all medication monthly to ensure compliance. SANITATION During the visit it was observed that the feeding apparatus was not properly cleaned after a child completed a bottle. The administrator and teachers were unaware that the apparatus needed to be cleaned after each feeding. I would recommend having all feeding completed in a highchair or the child being held during feeding. ACTIVITY PLAN It was observed in the toddler room (304) lesson plan was not posted. In room 305 the lesson plan posted was dated October 27th – 31st. In room 307 the lesson plan posted was dated September 15th. Having an updated lesson plan plays a key role in preparing for children’s care and learning. Lesson plans help the classroom stay organized, assist in the learning process of young children and give guidance and structure to the class. I would recommend having the staff to complete lessons plans on or before Thursday of each week and submit them to management to be posted by close of business on Friday for the following week. MEDICATION/HAZARDOUS STORAGE In space #302 diaper cream was not stored in a locked storage or a locked cabinet or locked storage. In space #305 the closet door was observed unlocked. The closet stored an aerosol can of Febreze Air Mist, Wet Jet Swiffer mop, the cleaning solution for the Wet Jet, diaper cream, and Benadryl. The teacher stated that the closet normally stays locked and the children do not go in the closet. I would recommend placing a key in an area near the door to use to make sure the door remains locked. I would also recommend a member of management do a safety check randomly through out the day to ensure all closets are locked. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. QRIS MODERNIZATION I discussed with the administrator the different Pathways during today’s visit. Ms. Hill stated that she will be discussing the different Pathways with the Head of School to make a decision. HEALTH CONSULTANT Ellie Morrise is the new Child Care Health Consultant. The classroom is approved for care; however, it is not a safe environment. In order for the classroom to continue to be used the classroom will need to be free of storage and have adequate equipment and materials. I discussed with Ms. Hill using another classroom while working to bring space 301 in compliance. No children were being cared for in the space during the visit. Mats were stored in all the classrooms with the floor side touching the bedside during today's visit. 15A NCAC 18A .2821(a) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Plastic and plastic ziplock bags were stored in an unlocked cabinet and drawer in space #302 and #305. .0604(q) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. SAFE INDOOR ENVIORNMENT The cabinet that stored wipes was unlocked that contained plastic in space #302 with infant children, and #305. Plastic ziplock bags were stored in an unlocked drawer in space #305 with children one and two years old receiving care. Please be mindful to lock the cabinets after each use. I would recommend checking all classrooms to ensure that locks are working properly. I would also recommend viewing Caring for our Children in regards to Safe Indoor Environment at https://nrckids.org/CFOC. REFRIGERATOR The refrigerator in space #305 maintained 50.9-degree Fahrenheit. The teacher stated that she has adjusted the temperature, however the temperature will not hold 45 or below. I would recommend defrosting the refrigerator to see if the temperature will remain in compliance. BEDS, COTS, MATS, AND LINENS Mats were observed in all classrooms stored with the floor side touching the bedside. I discussed with the Director using dividers in between mats. I would recommend discussing with the teachers to sanitize mats before using until dividers are able to be used. PLAYGROUND MULCH The mulch on the playground only measured about 1 in a half inch at the fall zones. I observed on the playground inspection completed by the administrator that the playground inspection needed more mulch has been recorded each month. I would recommend viewing Caring For our Children regarding Chapter 6 play areas/playgrounds. STAFF FILES One staff member file who was hired on 6/2/25 did not have a medical on file. One staff member hired on 1/2/25 did not have a signed medical on file from a physician. The form was completed but with no signature or stamp from the physician. I would recommend requesting your annual fire inspections a month early to avoid delinquency. INCIDENT LOG During today’s visit I observed the incident log not being maintained. The last documentation in the incident log was documented on 11/23/21. I would recommend having a member of the management complete the incident log weekly to ensure that you remain in compliance. REFRIGERATOR TEMPERATURE The temperature in space #303 maintained a temperature of 48 degrees Fahrenheit. I would recommend defrosting and adjusting the cooling temperature. I would also recommend checking the temperature in all refrigerators to ensure that you remain in compliance. CRIMINAL BACKGROUND CHECK The Head of School did not have a criminal background check on file during today’s visit. I discussed with the Head of School and the Director the process of obtaining a criminal background check. The Head of School was unaware that a criminal background check was needed. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. ROOM TEMPERATURES I would recommend using a thermometer in your infant and toddler classroom. The infant and toddler classrooms operate from the same HVAC unit. The infant classroom felt significantly cooler than the toddler classroom. You will want to ensure that the infant classroom never goes below 65 degrees Fahrenheit and above 75 degrees Fahrenheit. A child in space #1 and #space #3, did not have the correct authorization forms to administer emergency medication on file during today's visit. .0803(6)(a-i); .0803(7)(a-g); .0803(8)(a-d) 9995 A violation was found for which there is no item number. In space #6 a child's bottle that was previously used was stored back into the refrigerator. This is a violation of a requirement in 15A NCAC 18A .2804(h). 9999 Water bottles were not labeled with the name of the child to whom the water bottle belongs, individually stored in the child's cubby, and sent home with the child at the end of the day. In space #3 and space #4, individual water bottles were not labeled with the child's name. This is a violation of a rule in NC Sanitation Requirement 15A NCAC 18A .2804(i). In order to comply with the NC Laws & Rules, any violations cited today must be corrected immediately. A compliance letter should be sent to me by December 20, 2023. The letter should address each violation, and explain how and when it has been corrected. Please send the letter to shannel.hall@dhhs.nc.gov and include the following: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY: Prior to today's visit, the program's compliance history was 85%. It will be updated after today's visit, and you will be informed of the new score. TECHNICAL ASSISTANCE The following items were discussed to help you prevent a reoccurrence of the violations observed during today's visit. I would suggest putting a medication check on you monthly check list for either you or your staff. I would suggest parents drop off all emergency medication to you before taking them to the classrooms to ensure all the pertinent information is completed by the parents. I would suggest doing a daily walk through to randomly do check for things to be in place. ANNUAL IN-SERVICE TRAINING The in-service hours were monitored for the annual year December 7, 2022 - December 6, 2023. CONSULTATION Some providers are experiencing delays in scheduling their fingerprinting appointments. Criminal record checks can be completed up to six months prior to the expiration date. We are encouraging all staff to begin their renewal process early to avoid lapses in their qualification. RATED LICENSE REASSESSMENT This facility prep year is from July 1, 2023-June 30, 2024. The facility reassessment year will not begin until July 1, 2024. During this time, the provider will decide whether they wish to have the Environment Rating Scale Assessment conducted. We discussed making sure the provider WORKS account reflects her current degree. We discussed reviewing the NCRLAP website ERS resources (www.NCRLAP.org) for updated information. Ms. Hill is aware that she can request an ERS assessment free of charge during the preparation, and depending on the scores, they can be used as guidance to enhance program quality. Technical assistance can be requested anytime during this process. HEALTH AND SAFETY TRAININGS Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings dare provided free of charge. You should use your NCID username and password to access the Moodle portal. As per Child Care Rule .1103(b) the Health and Safety trainings must be renewed at least every 5 years. Some of your staff completed the trainings in 2017, they should begin to retake the training. AGENCY UPDATES Please remember to check the Division of Child Development and Early Education website: nchildcare.ncdhhs.gov frequently to stay updated on rule requirements. The North Carolina General Statute 110-90.2(b) requires that all childcare providers must have a criminal background check prior to employment and every three years thereafter. As of July 11th, instead of renewing your criminal background check every three years, you will renew them every five (5) years. Please note that the criminal background unit will not be resending letters that were previously done before July 11, 2022.
