Upon arrival, I was greeted by the provider, Cynthia Yarborough, who assisted me during the visit. LICENSE STATUS The family child care home currently operates with a Four Star License issued on March 7, 2019. The program received seven points for Program Standards, three points for Staff Education, and one Quality Point. INSPECTIONS/DRILLS STATUS -The last documented outdoor play area inspection was completed on 1/7/26. -The last documented fire drill was conducted 3/2/26. -The last documented lockdown/shelter-in-place drill was conducted on 3/4/26. -The last annual compliance visit was conducted on 9/25/25. MONITORING I completed a walk-through of the home, visiting the caregiving space, bathroom, and outdoor play area. Four children were present, ranging in age from seven months to three years old. The children were observed eating snack, getting diapers changed, dancing to music, and playing freely with materials. A partial assessment of child care requirements was conducted. I monitored all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records. Two newly enrolled children’s records also were reviewed. One child requires emergency medications; therefore, this requirement was monitored and violations were cited. A child 12 months of age or younger is enrolled; therefore, safe sleep requirements were monitored and a violation was cited. INDOOR/OUTDOOR SPACE Children are cared for in a converted garage and a variety of materials are available for play, including blocks, manipulatives, books, and art materials. The bathroom is located in the hallway just outside the caregiving space, and the changing table is within the caregiving space. Cots and a portable crib are used for rest time. The outdoor learning environment is a fenced backyard with a small bike path and a variety of riding toys, playhouses, and other equipment for gross motor play. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. No attendance records were available for review. .1721(e)(6) 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). The provider's qualification letter expired on 2/3/26. G.S. 110-90.2 & .2703(n) & (o) 1404 Medication remaining after the course of treatment is completed, after authorization is withdrawn. or after authorization has expired, was not returned to the parent/guardian. Any medication not retrieved by parent/guardian within 72 hours of completion of treatment, or withdrawal of authorization, was not discarded. A child's medication expired in September 2025. 10A NCAC 09 .1720(c)(5) 1406 Authorization for up to 6 months to administer prescription or over-the-counter medication, when needed, for chronic medical conditions and for allergic reactions was not in writing and did not contain information required by rule 10 NCAC 09 .1720(c)(6)(A-I). The medical authorization form did not contain the valid dates of the authorization. 10A NCAC 09 .1720(b)(4) 1810 Operator did not retain the safe sleep acknowledgement in the child's records as long as the child was enrolled. A newly enrolled infant was missing a signed safe sleep policy. 10A NCAC 09 .1724(c) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by April 7, 2026. The letter must address each violation and explain how it has been corrected. The FCCH was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact the provider’s overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your facility. TECHNICAL ASSISTANCE -Criminal Backgrounds Checks for all providers should be completed every five years. Your last qualifying letter expired 2/3/26 and must be renewed immediately. You have 15 days to obtain a new qualifying letter; after that, you will not be able to keep children in child care until you receive your new qualifying letter. You said you had them but couldn't find them. Accurate attendance records show that you remain in compliance with child care ratios as well as provide a list of children present in the event of an emergency. I recommend placing the attendance log next to your entrance door for parents to sign children in and out each day. CONSULTATION -We reviewed your progress preparing for your upcoming rated license assessment. If you have completed additional coursework since your last submission to WORKS, please submit your education transcripts to DCDEE WORKS as soon as possible to allow sufficient time for processing and plan to send your application to me by April 30, 2026. The rated license application can be found on the DCDEE website under “Provider Documents and Forms,” and links to all required forms are on that document. Upon arrival, I was greeted by the provider, Cynthia Yarborough, who assisted me during today’s visit. LICENSE STATUS The family child care home currently operates with a Four Star license issued on March 7, 2019. The program received three points for Staff Education, seven points for Program Standards, and one Quality Point. INSPECTIONS/DRILLS STATUS -The last annual compliance visit was conducted on 10/9/24. -The last documented monthly outdoor play area inspection was completed on 9/1/25. -The last documented monthly fire drill was conducted on 9/18/25. -The last documented quarterly lockdown/shelter-in-place drill was conducted on 7/3/25. MONITORING I completed a walk-through of the home, including the children’s space, bathroom, and outdoor play area. At the time of the visit, there was one child present, aged one year old. There are three children enrolled. The children was observed being held by the provider, playing freely with materials, coloring, and eating lunch. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, capacity, health and safety requirements, equipment and furnishings, and program records. The provider plans to start transporting children; therefore, this requirement was monitored and found in compliance. No enrolled children have medications or medical care plans, so these requirements were not monitored. Children 12 months or younger are enrolled; therefore, safe sleep requirements were monitored and found in compliance. The provider verified that all the information on the FCCH Verification of Required Information for Operator and Additional Caregivers form was correct. INDOOR/OUTDOOR ENVIRONMENT Inside the home, children are cared for in a converted garage. A variety of items for learning through play are available, including manipulatives, blocks, books, art materials, and home living props. The changing table is in the caregiving space, and the bathroom is located just down the hall within sight of the caregiving space. The outdoor play area is a fenced backyard with a variety of gross motor equipment and a small bike path. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 702 Potentially hazardous items, including but not limited to, power tools, nails, chemicals, propane stoves, lawn mowers, and gasoline or kerosene, whether or not intended for use by children were not stored in locked areas, removed from the premises, or otherwise inaccessible to children. A propane tank was in an unlocked storage shed in the outdoor play area. .1719 (a) (1) 925 Signed and dated statement which attests that a copy of the discipline policy was given to and discussed with a parent/guardian of each child prior to enrollment was not on file in the home. A signed copy of the discipline policy was not on file for a child enrolled on 5/6/25. G.S.110-91(10); .1727(a)&(b) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by October 9, 2025. The letter must address each violation and explain specifically when and how it was corrected, as well as how you will prevent the same violations from happening in the future. Please send this to mandy.annunziata@dhhs.nc.gov and include the following information: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today’s visit, the facility’s compliance history score was 96%. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. Failure to maintain compliance may result in an administrative action. The facility was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact the center’s overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your program. TECHNICAL ASSISTANCE -Safety is a top priority when providing care for young children. Today I observed an unlocked storage shed, which contained a propane tank, in the outdoor play area. You said that your husband must have left it unlocked because that is his shed. You stated that you will tell him to keep it locked at all times. Please be sure to double-check the lock any time children are in the backyard. -It is important to keep accurate records for all children in your care. One child who enrolled on 5/6/25 did not have a signed discipline policy on file. Please make sure the child’s parent receives a copy of the discipline policy and signs it. To ensure each child’s file is complete, you should use the children’s file checklist available on the DCDEE website in “Provider Documents and Forms” under the “Provider” tab. CONSULTATION -We discussed the new licensure pathway options and I told you how to access the QRIS information on the DCDEE website. You are still considering whether to pursue Pathway 1 or Pathway 2. -You said that you want to begin transporting children, so we discussed the requirements. Be sure that you have the proper child safety seats, a first aid kit and a fire extinguisher in your vehicle as well as files with children’s photos, emergency information, and permission to transport forms. -The Clean Classrooms for Carolina Kids Program provides free facility-wide testing for lead in water and inspections for asbestos and lead-based paint hazards without any out-of-pocket costs. Your water was tested on 11/14/24 and voluntary mitigation efforts were recommended, including faucet fixture replacement and/or certified filter installation. -You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please review and update your EPR plan yearly. -Criminal Backgrounds Checks for the provider and all household members 16 years of age and older should be completed every five years. Your background check will expire 2/3/26, and you may start the renewal process up to six months in advance of the expiration date. TRAINING REMINDERS -Training from 10/9/24 to 9/25/25 was reviewed today and documented on the Verification of Required Information for Operator and Additional Caregivers form. You completed 3.5 extra hours which will carry over to next year. -CPR and First Aid must be renewed every two years. Your CPR and First Aid certification expires 8/2/27. -ITS-SIDS training must be renewed every three years. Your training expires on 11/13/26. -Health and safety training must be renewed every five years. Your training expires on 11/2/28. RATED LICENSE STATUS The NC State Legislature has extended “hold harmless” status until new rated licensing guidelines are in place, and you may opt to keep your current license and delay your rated license assessment until the new guidelines are adopted. Please use this time to update your WORKS account with your education information. You can visit the North Carolina Rated License Project website (https://www.ncrlap.org) for resources regarding the rated license assessment process and the FCCH self-study. AGENCY UPDATES Please check the Division of Child Development and Early Education’s website frequently for any new policies or rules. Go to https://ncchildcare.ncdhhs.gov/, and click on the “What’s New” tab for all agency updates, including QRIS Modernization. I recommend exploring the new pathway options to obtain a rated license. New QRIS rules are in effect, and the Division is preparing for a gradual transition to the new requirements. Resources, forms, and training will be provided to support you in preparing for new options to earn your Two- through Five-Star Rated License.
