Tracy Stanley, Lead Consultant, was also present during the visit. Davisha McDonald, Provider, accompanied me during a walk-through of the home. During the visit, I discussed the allegations with Ms. McDonald. Limited monitoring of child care requirements occurred during today’s visit. Violations must be corrected immediately. Within one week (March 2, 2025), you must submit documentation of the corrections you made and your plan to maintain compliance with the identified child care requirements to me at ( Cusarah Riddick, Investigation consultant, 919-208-0546, cusarah.riddick@dhhs.nc.gov and fax-919-715-1013). Violation Number Comment Rule 705 All medicines, including refrigerated and unrefrigerated, were not kept in locked storage. During the visit, medication was not stored in a locked box. .1719(a)(5) 1102 Parent/guardian was not notified of when and where the child was to be transported, and the name of the transportation provider. On October 24,2024, a parent did not know the whereabouts of their four year-old child who had been transported by the provider to a doctor's appointment. .1723(5) 1845 The child care provider and/or household member who incurred pending charges, indictments, or convictions since their last qualification letter was issued by the Division, did not notify the operator of these charges within five business days or before returning to work. The operator did not notify the Division of these pending charges, indictments, or convictions within one business day of being notified by the child care provider or household member. On June 18, 2024, a household member received charges that were not reported to the division. G.S. 110-90.2 & .2703(m) 1846 An individual over the age of 15, who resides in a family child care home or a center located in a residence on a permanent or temporary basis, or those who had their 16th birthday after the initial licensing of a family child care home; did not complete and submit required forms within five business days. On February 24, 2025, an individual was found to be living at the residence without a completed criminal background check. The provider, Davisha McDonald was present and assisted me when needed. The visit summary was reviewed with Ms. McDonald and a copy was given to her at the conclusion of the visit. Upon arrival, I was greeted by Ms. McDonald. I completed a general walk through of the indoor and outdoor environments. The four (4) preschool children present were observed in center activities, eating a morning snack and playing outside. CURRENT LICENSE STATUS Currently this fcch operates with a four-star license issued Marcy 13, 2023 earning seven points in Staff Education, two points in Program Standards and one Quality Point. INSPECTIONS *The last annual compliance visit was conducted on September 8, 2023. *The last fire drill, outdoor inspection and shelter-in-place were all conducted on August 2, 2024. INDOOR ENVIRONMENT In the children’s space, there are a variety of materials for the children to explore throughout the day. The bathroom is located near the childcare space and is accessible to those that are potty trained. *The outdoor play area is in the backyard of the home and is fenced. The provider informed me of the following: -Medication is not administered at this time. -There have not been any accident/incident reports written. -There are no children with allergies. -Transportation is not provided. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 908 Health questionnaire was not completed annually. The health questionnaire was completed on 2/3/23. .1703(a)(1) 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. Two children currently enrolled did not have a health assessment on file. GS 110-91(1); 10A NCAC 09.1721(a)(1) 1718 The written plan of care was not given and explained to parents of children in care on or before the first day the child attended the home. Parents did not sign a statement acknowledging the receipt and explanation of the plan. Parents did not give written permission for their child to be transported by the operator for specific routine tasks that are included on the written schedule. The eleven children enrolled did not have this information on file. .1712(e )(6) 1821 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of 30 days. The on-going training requirements were monitored from September 2023-August 2024 and was found to be in compliance. TECHNICAL ASSISTANCE -You must observe and record sleep positions for all infants who are sleeping. You might find it helpful to set a timer for 15 minutes when infants are sleeping and reset the timer until all infants are awake. This gives an auditory reminder to check on the infants while they are sleeping. You can also make sure the safe sleep charts are kept near where the infant is sleeping to remind yourself to document sleep positions. A safe sleep environment is essential in reducing the risk of SIDS. -A hospital must be documented on the children's application in the preferred hospital section. Nearest or closet is not acceptable. -I suggested Ms. McDonald require the parents bring in the health assessment on the child’s first day instead of waiting the 30 days. -I suggested Ms. McDonald to use the children’s file checklist to ensure all paperwork is in the file. Best practice would be to check the files twice per year to make sure all forms are present. The purpose of today’s visit was to monitor applicable childcare requirements during a Routine Unannounced visit. The last Annual Compliance visit was conducted on 9/8/23. This Family Child Care Home operates with a four-star license, which was issued on 3/13/23. The compliance history for this facility for the previous eighteen-month timeframe is 91%. During today’s visit the operator stated that the current shift operating hours were working well: First Shift: 7:30 am – 3:45 pm Second Shift: 3:45 pm – 12:30 am Currently there are children enrolled on both shifts. The operator stated that she was not having an issue with shift-overlap. During today’s visit the indoor child care space and the outdoor environment were monitored. Current activity plans and menus were posted. RESIDENTS IN THE HOME: The operator stated that the residents in the home were the operator and her four daughters. OBSERVATIONS: There were four children present today. I observed the children lying down for naptime. NEW CHILDREN: I reviewed the file for the child, who began attending on Tuesday 1/23/24. OPERATOR REQUIREMENTS: By reviewing the documentation from the 9/8/23 Annual Compliance visit, I verified that the operator had current CPR, First Aid, and IT’S SIDS training, along with current Criminal Record Check status. The household members also have current Criminal Record Check status. I reminded the operator that she needed to complete an updated CPR and First Aid training by 4/20/24. SMOKE DETECTORS: The electrically operated smoke detector with battery back-in was tested during the visit. TRANSPORTATION: The operator stated that she was not transporting children. IN-SERVICE TRAINING: I reminded the operator that in-service training hours would be tallied during the Annual Compliance visit. INSPECTIONS: The last fire drill was conducted on 1/3/24. The last shelter-in-place drill was conducted on 1/4/24. The last outdoor inspection was completed on 1/4/24. The following violation was cited today: Violation Number Comment Rule 102 Number of preschool children exceeded five children. The attendance records showed that there were six preschool age children present and in care on Tuesday 1/23/24 and 1/24/24. GS 110-91(7)(b) All violations must be corrected immediately. Please send me a signed Corrective Action letter by 2/8/24 explaining how this violation has been corrected. I stated that since a violation of over enrollment was cited, a return visit in the near future to monitor the number of children in care. OVER ENROLLMENT: According to the attendance record, six preschool age children were in attendance on 1/23/24 and 1/24/24. The sixth child, who began attending on 1/23/24 was present during today’s visit but left during the visit. Technical Assistance was provided on the violation cited today. It was determined that the operator only had a space for the new child on Thursdays and Fridays. The operator stated that she would discuss the child’s schedule and the availability for his care with the parent to determine how to correct the violation cited today. DAILY SCHEDULE: I observed that the posted daily schedule reflected a naptime of 12:30 pm to 2:30 pm. I arrived at 11:10 am and the children were lying down for naptime. I discussed this matter with the operator. She stated that naptime was early on Thursdays only, due to a course she was taking online. The operator stated that naptime would end in two hours from the time they laid down to rest. OUTDOOR ENVIRONMENT: Please keep the garden hose wound or inaccessible to the children to eliminate a potential tripping hazard. Since this was the second attempted visit, I telephoned the operator. She stated that she was on the way to the Family Child Care Home. There were no children present today. For injury reasons, the Family Child Care Home was closed this week. The operator stated that she planned to re-open on Monday, 9/11/23. The purpose of today’s unannounced visit was to monitor applicable childcare requirements during your first Annual Compliance visit. This Family Child Care Home began operation on 9/12/22. This facility was issued a four-star license on 3/13/23. The compliance history for the previous eighteen-month time frame is 87%. During today’s visit the indoor child care space and the outdoor environment were monitored. The required information was posted on the Parent Information Board. During the visit the verification form, enrollment worksheet, and signed statements of responsibility were completed and reviewed. CURRICULUM: This Family Child Care Home was implementing The Creative Curriculum. SMOKE DETECTORS: The smoke detectors were tested during the visit. TRANSPORTATION: The operator stated that she was not conducting any transportation services, even though some children’s files contained signed transportation permission slips. INSPECTIONS: The last outdoor inspection was completed on 9/1/23. The last documented lockdown drill was conducted on 8/13/23. The last documented fire drill was conducted on 9/1/23. IN-SERVICE TRAINING: Yearly in-service training hours for the operator were tallied during today’s visit. HEALTH AND SAFETY TRAINING: I verified that the operator had completed the required Health and Safety trainings with the exception of “Administering Medication”. The operator will complete the final training by 9/11/23 and send me verification of completion. The following violations were cited today. Violation Number Comment Rule 539 The written schedule did not include a minimum of one hour of outdoor play throughout the day, if weather conditions permit. The operator needed a written schedule for second shift care. .1718(a )(7)(D) 908 Health questionnaire was not completed annually. The operator's health questionnaire expired on 5/12/23. .1703(a)(1) 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. Seven children did not have a medical assessment on file. GS 110-91(1); 10A NCAC 09.1721(a)(1) 912 Written feeding schedule was not on file for each child under 15 months of age and/or was not updated regularly to reflect changes in the child's needs. The infant did not have a feeding schedule on file. .1706(i) 1892 Application was not on file for each enrolled child, including his/her own preschool child(ren) who are not school-age. One child did not have an application on file. .1721(a)(3) 1894 Application did not include names and contact information of individuals to whom the operator may release the child as authorized by the person who signs the application. Five children did not have emergency contact information on file, in case the child's parents could not be reached. .1721(a)(3)(F) All violations must be corrected immediately. Please send me a signed Corrective Action letter by 9/21/23, stating how each violation has been corrected. TECHNICAL ASSISTANCE WAS PROVIDED TODAY IN THE FOLLOWING AREAS: SHIFTS: Upon arrival the operator stated that her hours were: First Shift from 7:30 am – 5:00 pm, and Second Shift from 3:45 pm to 11:30 pm. After the complexity of managing these shifts were discussed, Technical Assistance was provided, and the operator stated that changing the shift hours to the following would work better to avoid any shift overlap situations: First Shift 7:30 am to 3:30 pm Second Shift: 3:30 pm to 11:30 pm OUTDOOR ENVIRONMENT: During today’s visit I observed three chickens walking around the fenced-in play area. The operator stated that they had hatched the chickens from eggs. The chicken had a coup, but the operator stated that the chickens walked around the fenced-in space when the children were not outside playing. I stated that I would inquire if additional steps were needed, regarding the chickens. RETURN: I stated that I would make a follow-up visit in the near future to see the children.
