I arrived prior to the end of the school operating day. The purpose of today’s unannounced visit was to monitor applicable child care requirements during the Annual Compliance visit. The last Annual Compliance visit was conducted on 5/17/23. This facility operates with a four-star license, which was issued on 6/21/19. This facility operates on a traditional school schedule. The compliance history for the previous eighteen-month timeframe was 99%. During today’s visit the indoor primary space (cafeteria) was monitored along with the designated outdoor play area. OBSERVATIONS: The children were operating as four groups. I observed the children arriving for afterschool care. Upon arrival for afterschool care, the children were greeted, and attendance was taken. I observed the children eating afternoon snack and engaged in the following activity areas in the cafeteria: science, arts/crafts, woodworking, manipulatives, music, housekeeping, math, reading with cozy area, board games, and privacy area. Each of the activity areas was well supplied with multiple play opportunities. Current activity plans were posted. The theme for the week was “Spring has Sprung”. The allergy list was posted in the cafeteria. OUTDOOR ENVIRONMENT: The site manager stated that the afterschool children used the following sections of the large outdoor space: both multi-purpose climbers, the swings, and the open field. I observed that one swing was broken, but it had been wrapped around the top bar of the swing frame, making it inaccessible to the children. The site manager stated that she routinely checked the field for any signs of fire ants, prior to letting the children play in the area. The required information was posted on the Parent Information Board. INCDENT REPORTS: The incident log was monitored during the visit. CHILDREN’S RECORDS: Twelve children’s files were reviewed for the required health and safety requirements. STAFF FILES: During the visit I reviewed staff files for on-site requirements. It was determined that the site manager had printed the wrong staff/training worksheet for my use. Multiple dates on the worksheet were changed. The site manager stated that she would send me the correct worksheet with all the current dates. Yearly in-service training requirements were tallied, and I determined that the staff had met their yearly training requirement. INSPECTIONS: The last annual fire inspection was conducted on 3/15/24. The last sanitation inspection was conducted on 2/26/24. The last fire drill was conducted on 4/14/24. The last documented lockdown/shelter-in-place drill was conducted on 3/6/24. The last documented outdoor inspection was completed on 4/10/24. The following violations were cited today: Violation Number Comment Rule 501 Meals/snacks did not comply with the Meal Patterns for Children in Child Care Programs. For afternoon snack the children had educational crackers and water. 10A NCAC 09 .0901(a) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. The staff orientation form was not complete for the group leader, who began employment on 2/14/24. .1101(a) Violations must be corrected immediately. Please send me a signed Corrective Action letter by 5/21/24, stating how the violation cited today has been corrected. TECHNICAL ASSISTANCE was provided in the following topic areas: OUTDOOR ENVIRONMENT: This facility has the following restriction on their license: PLAYGROUND AREA DOES NOT MEET CHILD CARE STANDARDS. RATED LICENSE: The Rated License three-year reassessment process has resumed. This facility is in Cohort II for reassessment. The assessment prep year for Club Blvd SAC Club will begin on 7/1/24. The assessment year will begin on 7/1/25. The site manager stated that she was started training new group leaders on the SACERS-U assessment tool. The purpose of today’s unannounced visit was to monitor applicable child care requirements during a Routine Unannounced visit. Monitoring of child care requirements was limited today, because the site manager was a group leader today to maintain staff/child ratios, leaving no one available to pull information. The last Annual Compliance visit was conducted on 5/17/23. This facility operates with a five-star license, which was issued on 6/21/19. The compliance history for the previous eighteen-month timeframe was 100%. Indoor primary space and the designed outdoor playground were monitored today. OBSERVATIONS: There were four groups operating today. I observed the children playing outside in the courtyard and on the playground. The required information was posted on the Parent Information Board. Current activity plans and current snack menus were posted. The following activity areas were set up in the cafeteria (primary space): Music, Legos, manipulatives, woodworking, arts/crafts, science, reading, board games, and language. Some of the tables in the primary space (cafeteria) were set up for a cooking decorating project today. The site manager stated that the facility did not have any new staff. By reviewing the staff/training worksheets from the 5/17/23 Annual Compliance visit, I verified that staff had current Criminal Record Check clearance and current CPR/First Aid training. CHILDREN’S FILES: Children’s records were not reviewed today. INSPECTIONS: The last annual fire inspection was conducted on 2/13/23. The last sanitation inspection was conducted 5/17/23. The last fire drill was conducted on 12/5/23. The last documented lockdown/shelter-in-place drill was conducted on 9/26/23. The last documented playground inspection was completed n 11/14/23. Please remember to conduct a playground inspection and lockdown/shelter-in-place drill in December, before closing for the Christmas holidays. The following violation was cited today: Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. There was an ouncovered outlet in primary space (cafeteria). 10A NCAC 09 .0604(c) Since the violation cited today was corrected during the visit, a Corrective Action letter was not requested. OUTDOOR ENVIRONMENT: This facility has the following restriction on their license: PLAYGROUND AREA DOES NOT MEET CHILD CARE STANDARDS. The outdoor environment was monitored today for safety concerns only. TECHNICAL ASSISTANCE was provided in the following topic areas: OUTDOOR ENVIRONMENT: The green caution tape around the rock area on the playground needs to be re-secured. RATED LICENSE: The Rated License three-year reassessment process has resumed. This center is in Cohort II for reassessment. The preparation year for the assessment will begin on 7/1/24.
