You, Ada Terry, assisted me with the visit. Upon arrival, I was greeted by Monica Thornton, Assistant Director. I completed a general walk-through of seven (7) classrooms with Ms. Terry. There were fifteen (15) staff, and eighty-eight (88) children present during the visit. Children throughout the facility were participating in circle time, center activities, transitions, personal care routines, and eating lunch. Infants were observed in free play activities with their teacher and having tummy time. Limited monitoring of child care requirements was conducted during the visit. Supervision of children, staff/child ratios, sanitation, use of adequate/approved space and permit restrictions were monitored. The License and emergency care plan were posted. During today’s visit, I discussed the allegation with the director and staff. They were each given an opportunity to provide information surrounding the allegation. I asked the director if there were insects in the center. According to the information provided there have been roaches seen in the building. They come in from backpacks and car seats that are left at the center. Monthly services are received from Clegg’s Termite and Pest Control. Car seats are checked for insects before being left at the center. There was a concern reported to Durham County Health Department, Children’s Environmental Health, concerning roaches in the center. On 10/6/25 an environmental health specialist made a visit to the center, and the report indicated there were 2-3 live roaches in the infant classroom only. The correction was to have wild life pro pest control come out for service on 10/17/25 and 10/31/25. The last sanitation inspection was completed on 2/4/26 and the center received a “Superior” classification with 7 demerits. None of the demerits received were due to insects. Ms. Terry stated that Clegg’s Termite and pest control provides service monthly to the center. I received the invoices from January-May 2026. The invoices for 2/23/26 and 3/25/26 indicated that roaches were caught on glue boards. A visit was made on 5/13/26 and the invoice did not indicate any sighting of roaches. During my walk through of the facility, I did observe a few dead insects on the floor of the infant room that is not open, and one dead roach under the diaper changing table. No diaper bags were observed in the infant classroom. Staff in classrooms were observed cleaning as needed following sanitation procedures. No violations were observed. Violation Number Comment Rule 9999 A violation was found for which there is no item number. Insects were not excluded from the child care center. There have been sightings of roaches in the facility. This is a violation of a rule in NC Sanitation Rule/Requirement 15A NCAC 18A .2831(b). Your compliance history prior to today’s visit was 96%. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. INVESTIGATION STATUS Based on my interviews and invoices from pest control this allegation has been determined to be SUBSTANTIATED. TECHNICAL ASSISTANCE Providing a safe and sanitary environment that is free of insects is crucial to ensure that children are not at risk of illness or harm. You may need to evaluate your environment and adopt practices and protocols that ensure long-term, environmentally sound pest suppression through a range of practices including pest exclusion, sanitation and clutter control, and elimination of conditions that are conducive to pest infestations. Require items to be sealed if insects are coming in through personal belongings. Limit unnecessary items brought from home. Ask families to regularly check and clean diaper bags, backpacks, blankets and extra clothing before bringing them to the center. Conduct routine inspections of classrooms, kitchens, storage areas, and children’s belongings. You, Ada Terry, Director, assisted me with the visit. Your program currently operates with a five-star license, issued November 2, 2024, earning 7 points in the education component, 6 points in the program standards component (meeting all enhanced standards) and 1 quality point for having a staff benefits package and infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four-and five-star licensed facilities where four-year-old children are enrolled. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-eight percent as of 11/3/25. The NC Secretary of State website was reviewed on 11/3/25, and Child Development Schools North Carolina LLC was listed as current- active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions and personal care routines. Infants were observed being fed baby food and bottles. Sleep checks were completed. Two violations were observed today. The violations are as follows: Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. First Aid training expired on 7/12/25 for one staff. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. CPR training expired on 7/12/25 for one staff. .1102(d) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violations documented must be corrected immediately. On or before 11/19/25, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware that any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. STAFF RECORDS Staff files were monitored for criminal background qualifying letters, current CPR/First Aid, medical, TB test and other specialized training. On your next annual compliance visit on-going training hours will be monitored. RATED LICENSE INFORMATION Your next Rated License is due in November 2027; however, I encourage you to visit the DCDEE website to review the new QRIS REQUIREMENTS, Pathways to the Stars. We are going away from a point-based system allowing you to have three different pathways to achieve stars. CHILD CARE RULES Child Care Rules have been updated as of 7/1/25.
