Upon arrival I was greeted by Dr. Quincy Farmer, the Assistant Principal. I was directed to NC Pre-K classroom 406. Ms. Pfouts, the Lead Teacher, assisted me with today’s visit. At the time of the visit, there were a total of nine (9) children present, ranging in age from four (4) to five (5) years old. The children were observed in group time, completing a writing assignment, working on math, interacting with the teachers, transitioning outside, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a Five (5) star license issued February 8, 2019, earning seven (7) points in education, seven (7) points in program standards and one (1) quality point. INSPECTIONS *The last fire drill was conducted 3/28/25. *The last emergency drill was conducted 3/14/25. *The last playground inspection was completed 4/7/25. *The last sanitation was completed on 3/17/25 with a “Superior ‘’ classification. *The last fire inspection was unable to be monitored. The principal was unable to access the report. Please submit the report on or before Monday, April 28, 2025, to remain in compliance. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. The PreK teachers CPR/FA, qualification letters and newly enrolled children files were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. The following violations were observed and documented during today’s visit. Violation Number Comment Rule 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. A child enrolled on 10/21/24 did not have a medical exam on file. GS110-91(1) 1323 Each child was not immunized as per Article 6 of Chapter 130A and an immunization record was not on file before or within 30 days after admission. A child enrolled on 10/21/24d did not have an immunization record on file. 10A NCAC 09 .0302(d)(2) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before May 6, 2025, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. CHILDREN FILES During today’s visit a child enrolled on 10/21/24 did not have a medical exam and immunization records on file. The teacher stated that she has tried to obtain the records from the parent. She also stated the coordinator has also attempted, but the parent will not comply. It is best practice to receive all necessary paperwork on the first day of enrollment. I would also doing a record review monthly to ensure that you have all necessary paperwork. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety trainings must be renewed at least every five (5) years. If you completed the trainings in 2020, you should begin to retake the training now. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. Upon arrival I was greeted by Dr. Quincy Farmer, the Assistant Principal. I was directed to NC Pre-K classroom 406. Ms. Pfoust, the Lead Teacher, assisted me with today’s visit. At the time of the visit, there were a total of eight (8) children present, ranging in age from four (4) to five (5) years old. The children were observed transitioning from the cafeteria, completing a writing assignment, working on math, interacting with the teachers, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a Five (5) star license issued February 8, 2019, earning seven (7) points in education, seven (7) points in program standards and one (1) quality point. INSPECTIONS *The last fire drill was conducted 12/5/24. *The last emergency drill was conducted 12/9/24. *The last playground inspection was completed 12/2/24. *The last sanitation was completed on 11/12/24 with a “Superior ‘’ classification. MONITORING During today’s visit, a partial assessment of Child Care Requirements was conducted for all classrooms. The monitoring included CPR/First Aid, criminal background checks and special training requirements of newly hired staff. The monitoring also included License and permit restrictions, staff/child ratios, supervision, storage of hazardous items, medication, materials/equipment, and all space occupied by the children including outdoor play environment. The following violation was observed and documented during today’s visit. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Ms. Bailey assisted me today. Upon my arrival, I was greeted and welcomed into your program. I completed a general walk-through of the spaces used by the children and the outdoor play area. I observed the children participating in free play, exploring centers, exploring their outdoor learning environment, and resting their bodies. RATED LICENSE STATUS The facility currently operates with a five-star license issued on February 8, 2019. The program is in the "held harmless" status, and points from the previous assessment are carried forward. The program received 7 points in Program Standards, 7 points in Staff Education, and 1 Quality point through the educational option of 75% of lead teachers having BA/BS in early childhood education or child development. INSPECTIONS The last sanitation inspection was conducted on 2/16/24. The last fire inspection was conducted on 10/18/23. The last fire drill was conducted on 4/29/24. The last emergency drill was conducted on 3/7/24. The last playground inspection was conducted on 4/29/24. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. I did not assess transportation requirements because the program does not transport children. The following violations were observed and recorded during today's visit. Violation Number Comment Rule 804 There was not a sufficient supply of clean linen on hand. A child in space #2 did not have a sheet on their mat when napping. 10A NCAC 09 .0806(c) 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. An employee did not have a current health questionnaire on file for review during today's visit. .0701(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of Aid training from an approved training organization was not in the staff file. An employee did not have a current first aid certificate on file for review during today's visit. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. An employee did not have a current CPR certificate on file for review during today's visit. .1102(d) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. An employee in space #1, space #2 and the administrator did not have the required training hours available for review during today's visit. .1103(a) 1314 Emergency information did not name childs health care professional. I would suggest coming up with a plan to make sure everyone completes the required annual paperwork, training hours and printing them by a certain date. I would suggest reviewing children’s files when parents are turning paperwork in to ensure all pertinent information is completed. I would suggest making sure extra cot sheets are available in case a child doesn’t have one for the day. CONSULTATION Criminal background checks must be renewed every five (5) years. Criminal background checks can be completed up to six months prior to the expiration date. We are encouraging all staff to begin their renewal process early to avoid lapses in their qualification. HEALTH AND SAFETY TRAININGS Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge.
