The last Annual Compliance visit was conducted on 9/12/24. This facility operates with a five-star license, which was issued on 1/17/19. This facility operates on a traditional school schedule; the 2025-2026 school year began on 8/28/25. The compliance history for the previous eighteen-month timeframe was 99%. OBSERVATIONS: During today’s visit the licensed indoor and outdoor space was monitored. There were currently 8 children (age 4) enrolled. Six of the eight children were present today. The assistant teacher stated that none of the children had a food allergy or required medication. Upon arrival I the children in the license classroom space #104 were walking down the hall, returning from cafeteria. The children walked to the classroom, washed their hands, and then went outside to play on their designated playground. A couple of the children were carrying lunchboxes. The teacher stated that all the children signed the nutrition form. I observed the signed forms while reviewing the children’s files. I observed the children enjoying the outdoor environment. The teacher stated that the stationary climber on the designated outdoor space was off-limits for the Pre-K children, based on the age range on the climber. I reviewed the information posted on the Parent Information Board. CHILDREN’S RECORDS: Six children’s records were reviewed today. The two remaining children enrolled had not started attending, until a translator for their spoken language could be set in place. STAFF FILES: There were no new staff at this center. Staff files were reviewed to include yearly in-service training completed since the 9/10/24 Annual Compliance visit. INSPECTIONS: The last annual fire inspection was conducted on 2/28/25. The last sanitation inspection was conducted on 3/12/25. The last fire drill was conducted on 8/29/25. The last documented lockdown drill was conducted on 3/7/25. The last documented outdoor inspection was completed on 8/27/25. The following violations were cited today: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The posted activity plan was for last week. GS 110-91(12); .0508(a) 853 Incident logs were not completed and maintained as required. The incident logs were not on the premises today. .0802(g)(1-6) Violations must be corrected immediately. Please send me a signed Corrective Action letter by 9/18/25, stating how the uncorrected violation cited today has been corrected. TECHNICAL ASSISTANCE was provided in the following areas: RATED LICENSE: RATED LICENSE: The Pathways to the Stars transition is beginning now. There will be a “rules roll out” module in Moodle soon. I stated that free webinars were offered on the ECERS-3 through the NC Rated License Assessment Project, www.ncrlap.org. I stated that the webinars could be used to meet yearly in-service training hour requirements. NEXT ANNUAL COMPLIANCE VISIT: During today's visit I talked with the assistant principal and the teacher about conducting the next Annual Compliance visit at the end of the 2025/2026 school year, instead of the first week of a new school year.
