Upon arrival I was greeted by Mrs. Bryson. You assisted me with the visit. I completed a general walk-through of the center. At the time of the visit, there were a total of sixteen (16) children present, ranging in age from one (1) to five (5) years old. The children were working on art projects, playing with materials, interacting with the teachers, eating lunch, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a Five (5) star license issued December 1, 2017, earning seven (7) points in education, seven (7) points in program standards. I reviewed the NC Secretary of State’s website and observed the owner of the facility, John Bryson and Deborah Bryson listed as current/active. INSPECTIONS *The last fire drill was conducted on 4/29/25. *The last emergency drill was conducted 5/12/25. *The last playground inspection was completed 4/11/25. *The last sanitation was completed on 2/27/25 with a “Superior ‘’ classification. *The fire inspection was completed on 5/29/24. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. 10% of old staff, all new staff CPR/FA, qualification letters, and 10% of enrolled children’s files were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. The following violations were observed and documented during today’s visit. Violation Number Comment Rule 604 Lavatories were not kept clean, in good repair and kept free of storage. Lavatories were used for storage. I observed, toys, a carpet, chairs, bulletin board, Swiffer jet mops, and a vacuum stored in each bathroom. 15A NCAC 18A .2818(a) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. The written authorization for an EpiPen expired on 3/23/25 for a child in space #1. 10A NCAC 09 .0803(1)(a & b) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. Health and Safety training for staff was observed last completed in 2019. .1103(b) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before May 27, 2025, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. SANITATION The bathrooms in the three classrooms were also used for storage. I observed Swiffer jet mops, Swiffer jet pads, a rug, chairs, bulletin board, toys, books, vacuum and brooms in the bathrooms. Please remember that bathrooms are not used for storage. I would recommend storing those items in the closet for each room. I would also recommend viewing the website Caring For Our Children at https://nrckids.org/CFOC in regard to proper storage. MEDICATION The permission authorization for an EpiPen in space #1 was valid through 9/23/24 – 3/23/25. I would recommend setting up a reminder to update authorization forms for all medications monthly. HEALTH AND SAFETY TRAINING Health and Safety training was observed last completed for staff in 2019. Every five years Health and Safety training must be completed. I would recommend using an outlook reminder to make sure that you remain in compliance. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety trainings must be renewed at least every five (5) years. If you completed the trainings in 2020, you should begin to retake the training now. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. I would suggest monitoring for all forms for medications (prescription/non-prescription) monthly. HAZARDOUS MATERIALS In space #3 I observed Swiffer jet pads stored on the back of the toilet in the restroom that children use. All cleaning supplies should be stored in locked storage. I would suggest checking weekly to make sure all cleaning supplies are locked in a locked storage out of reach of children. The director, Mrs. Bryson, assisted me on today's visit. Upon arrival, you greeted me and welcomed me into your program. Three (3) classrooms were opened today. I conducted a general walk-through of the indoor and outdoor learning environments. I observed the children participating in free play while exploring their outdoor environment. In their indoor learning environment, the children were observed having free play, practicing for graduation, and exploring different centers, and having lunch. RATED LICENSE STATUS The program holds a 5-star-rated license issued on 12/1/17. The program is in the "held harmless" status and points from the previous assessment are carried forward. The program received 7 points in Program Standards, and 7 points in Staff Education. INSPECTIONS The last sanitation inspection was conducted on 3/18/24. The last fire inspection was conducted on 3/17/23. The last fire drill was conducted on 4/11/24. The last emergency drill was conducted on 4/11/24. The last playground inspection was conducted on 4/3/24. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. I did not assess transportation requirements because the program does not transport children. The following violations were observed and recorded during today's visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. A current fire inspection was not on file or available for review during today's visit. The last fire inspection was conducted on 3/17/23. 10A NCAC 09 .0304(a) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. A child in space #2 epi-pen was not stored in it's original container. .0803(2)(a) 846 Over-the-counter medicines were not in their original containers or administered as authorized in writing by parent, physician or authorized health professional. A child in space #2 had Benadryl that was not in it's original container. 10A NCAC 09 .0803(4) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Five staff members including the director did not have current First Aid certifications on file and available for review during today's visit. Each staff member First Aid certification expired on 5/3/23. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Five staff member including the director did not have a current CPR certification on file and available for review during today's visit. Each employees certification expired on 5/3/23. .1102(d) In order to comply with the NC Laws & Rules, any violations cited today must be corrected immediately. A compliance letter should be sent to me by June 4, 2024. The letter should address each violation, and explain how and when it has been corrected. TECHNICAL ASSISTANCE The following items were discussed to help you prevent a reoccurrence of the violations observed during today's visit. I would suggest contacting the fire marshal two to three months in advance to schedule your inspection before your inspection expires. I would put on the calendar to schedule staff to get in a CPR & 1st Aid training at least two months before their certification expires. I would suggest adding medication to your monthly checklist. CONSULTATION Some providers are experiencing delays in scheduling their fingerprinting appointments. Criminal record checks can be completed up to six months prior to the expiration date. We are encouraging all staff to begin their renewal process early to avoid lapses in their qualification. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Five staff members including the director did not have current First Aid certifications on file and available for review during today's visit. Each staff member First Aid certification expired on 5/3/23. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Each employees certification expired on 5/3/23. .1102(d) In order to comply with the NC Laws & Rules, any violations cited today must be corrected immediately. TECHNICAL ASSISTANCE The following items were discussed to help you prevent a reoccurrence of the violations observed during today's visit. I would suggest contacting the fire marshal two to three months in advance to schedule your inspection before your inspection expires. I would put on the calendar to schedule staff to get in a CPR & 1st Aid training at least two months before their certification expires. I would suggest adding medication to your monthly checklist. CONSULTATION Some providers are experiencing delays in scheduling their fingerprinting appointments. Criminal record checks can be completed up to six months prior to the expiration date. We are encouraging all staff to begin their renewal process early to avoid lapses in their qualification. HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. You should use your NCID username and password to access the Moodle portal. As per Child Care Rule .1103(b) the Health and Safety trainings must renew at least every 5 years. Some of your staff completed the trainings in 2017, they should begin to retake the training now. The director, Ms. T. Bryson, assisted me on today's visit. Upon arrival, you greeted me and welcomed me into your program. Two (2) classrooms were opened today. I conducted a general walk-through of the indoor and outdoor learning environments. I observed some children participating in a teacher directed activity in their indoor learning environment. Some of the older children were observed having free play while exploring their outdoor environment. RATED LICENSE STATUS The program holds a 5-star-rated license issued on 12/1/17. The program is in the "held harmless" status and points from the previous assessment are carried forward. The program received 7 points in Program Standards, and 7 points in Staff Education. INSPECTIONS The last sanitation inspection was conducted on 9/14/23. The last fire inspection was conducted on 3/17/23. The last fire drill was conducted on 10/17/23. The last emergency drill was conducted on 9/23/23. The last playground inspection was conducted on 11/23/23. The following items were monitored during today's routine unannounced visit: indoor and outdoor area, supervision, capacity, CPR & 1st Aid certifications, CBC’s completed, storage of hazardous substances, storage of medication, adequate and approved space, provider records, license posted and permit restrictions. The following violation was observed and recorded during today’s visit. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. We discussed making sure the provider WORKS account reflects her current degree. We discussed reviewing the NCRLAP website ERS resources (www.NCRLAP.org) for updated information. Ms. Bryson is aware that she can request an ERS assessment free of charge during the preparation, and depending on the scores, they can be used as guidance to enhance program quality.
