The purpose of today’s unannounced visit was to monitor applicable child care requirements for your Annual Compliance visit. The last Annual Compliance visit was conducted on 3/6/25. This center operates with a three-star license, which was issued 8/19/19. CORPORATION STATUS: According to the NC Secretary of State web site, the current corporation status of Broad Street Children’s Cottage LLC is “current-active”. COMPLIANCE HISTORY: The compliance history for this center for the previous eighteen-month timeframe was 98%. During today’s visit all indoor and outdoor licensed space was monitored. OBSERVATIONS: The children enrolled at this facility operate as one group. During today’s visit I observed the children enjoying the outdoor environment, eating morning snack, and having circle reading time with the teachers. Morning snack consisted of Cherrios, cottage cheese, and water. The center provides daily snacks, and the children bring their lunches from home. I observed the lunches stored in the refrigerator. All the children have a signed “opted out” form on file. The center had additional food items available. The required information was posted. CHILDREN’S RECORDS: Six children’s records were reviewed today. STAFF RECORDS: Staff files were reviewed today. Yearly in-service training hour requirements were tallied during the visit for the timeframe of 3/6/25 to 3/3/26. TRANSPORTATION: This program does not offer transportation services. INSPECTIONS: The last documented annual fire inspection was completed on 8/4/25. The last documented sanitation inspection was conducted on 9/22/25. The last documented fire drill was conducted on 2/23/26. The last documented shelter-in-place drill was conducted on 11/10/25. The last documented playground inspection was completed on 2/24/26. The last documented water testing for Clean Water for NC Kids was completed on 9/23/25. The center completed Clean Classrooms for Carolina Kids on 8/26/25. INCIDENT REPORT LOG: The log was reviewed during the visit. The following violations were cited today: Violation Number Comment Rule 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. One staff needed an updated health questionnaire on file. .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. One staff needed updated emergency information on file. .0701(a) 1232 Each employee's personnel file did not contain an annual staff evaluation and a staff development plan. One staff needed an updated Annual Evaluation. 10A NCAC 09 .0514(f) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last documented shelter-in-place drill was conducted on 11/10/25. .0604(u);.0302(d)(8) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. One child had medication for asthma. The child needed a medical authorization form signed by a parent to administer the medication. .0803(6)(a-i); .0803(7)(a-g); .0803(8)(a-d) All violations must be corrected immediately. Please send me a signed Corrective Action letter by 3/17/26, stating how each violation has been corrected. TECHNICAL ASSISTANCE was provided today in the following areas: CRIMINAL RECORD CHECK: I reminded the director that one staff needed to complete the Criminal Record Check requalification process by 3/18/26. HEALTH AND SAFETY TRAINING: I reminded the director that one staff needed to complete the Health and Safety trainings again in 2026, in accordance with NC Child Care Rule 10 NCAC 09. 1106 (b). ON GOING TRAINING DOCUMENTATION: Please complete the form for each staff, using the training certificates in staff files. RATED LICENSE: Technical Assistance had been provided on the new QRIS rated license assessment system during the 10/9/25 visit and by phone on 12/9/25. The director selected pathway I (program assessment) for completing the star-rated license reassessment. Technical Assistance was provided today on the timing for assessment. The director stated that she would begin the self-study immediately and apply for the Environment Rating Scales assessment, hoping to have the ECERS-3 conducted before the end of the summer. The director stated that she had participated in the workshops offered by the NC Rated License Assessment project. Upon arrival, I was greeted by the provider, Sandy Kosik. RATED LICENSE STATUS The facility currently operates with a Three-star license issued on August 19, 2019. The program received 2 points in Program Standards, 4 points in Staff Education, and 1 Quality point for having a 20% or less staff turnover. INSPECTION/SAFTEY DRILL STATUS: The last sanitation inspection was completed on 6/15/23. The last fire inspection was completed on 8/11/23. The last outdoor play area inspection was completed on 3/18/24. The last fire drill was conducted on 3/5/24. The last documented lockdown/shelter-in-place was conducted on 2/14/24. The last annual compliance visit was conducted on 4/20/23. Prior to today's visit, the status of the owners, Broad Street Children’s Cottage, LLC, is listed as current and active on the NC Secretary of State website. MONITORING: During today’s visit, a full assessment of Child Care Requirements was conducted. A checklist was used to monitor the requirements. This program does not transport children; therefore, these requirements were not checked monitored. A general walkthrough of the facility indoor and outdoor spaces. The children were observed participating in art activities, morning snacks and outdoor play. The caregivers were interacting with the children, meeting their developmental needs. The following violations were observed and cited during today’s visit: Violation Number Comment Rule 415 A current schedule was not posted for each group of children for reference. A schedule was not posted for space "A" and "B". GS 110-91(12);.0508(a) 428 A current activity plan was not posted for each group of children for reference. Space "B" did not have a current activity plan posted. GS 110-91(12); .0508(a) 1821 The EPR Plan did not include the date of the last revision of the plan. The last documented EPR plan was in 2017. .0607(d)(8) Please submit a corrective action letter for the above violations. While the Division allows you time to explain how you have corrected violations when they are documented, it is expected that you correct all violations immediately. Your corrective action letter must include the following: Name of your program, Id number of your program, Date of Letter, Violation(s) number(s), and How and When each violation was corrected. Your letter is due by no later than 4/2/24 and should be emailed to: corrine.fulp@dhhs.nc.gov. COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history score was 99%. The center was not in compliance with all applicable minimum licensing requirements during today's visit and violations were cited that could impact the center’s overall Compliance History Score. An updated Compliance History Score will be generated after today’s visit, and you will be informed of the results on the next visit to your center. TECHNICAL ASSISTANCE During the visit today we reviewed the staff training worksheet and found that Sandy Kosik, Mary Mrozkowski, and Gwen Pelo have not obtained their training requirement for the year. All completed training hours will be due April 20. SCHEDULE/ACTIVITY PLAN A current schedule and activity plan for each group of children should be posted for reference by parents and by caregivers. The schedule and activity plan may be combined into a single document. REMINDER * E. Kirk medication will expire on April 4, 2024. * The summary of NC Law was updated in September 2023, your posted copy will need to be reprinted. RATED LICENSE You are in Cohort #2 and your planning period starts on 7/1/24. You can do an ERS assessment during the prep year and use your score for the rated license. Make sure that staff have updated their WORKS accounts to reflect all current coursework. You can also do the following to help staff prepare for the visit: •Review previous assessments. •Look for assessment related resources – ncrlap.org. •Complete self-assessment to become familiar with ERS. •Visit www.ncrlap.org for updated trainings and videos. The NC Early Childhood Equivalency Exam is no longer an option for applicants until further notice. Here are other options to consider: ·Enroll in EDU 119 at your local community college ·Pursue the CDA option: Child Development Associate (CDA) Credential ·Other Pathways to the Professional Development (enroll in 4 year program) CURRENT UPDATES/DCDEE You may check the Division of Child Development and Early Education website: https://ncchildcare.ncdhhs.gov and click on “What’s New” tab for all updates.
