Upon arrival, I was greeted by the business manager, Shay Perez, and director, Mary Ann Whitley, who both assisted me during today’s visit. LICENSE STATUS The center currently operates with a Five Star License issued on October 18, 2024. The program achieved their star rating through NAEYC accreditation. The center is licensed to serve 149 children on first shift. Prior to today’s visit, I reviewed the NC Secretary of State’s website and noted that the owner of this facility, Bright Horizons Children’s Centers, LLC, is listed as current/active. INSPECTIONS/DRILLS STATUS -The last sanitation inspection was completed on 6/13/25, and the center scored a superior rating with six demerits. -The last fire inspection was completed on 9/18/25. -The last documented outdoor play area inspection was completed on 9/8/25. -The last documented fire drill was conducted on 9/8/25. -The last documented lockdown/shelter-in-place drill was conducted on 8/11/25. -The last annual compliance visit was conducted on 10/17/24. MONITORING I completed a walk-through of the center, visiting twelve classrooms as well as the outdoor learning environment. The infants were being fed bottles, napping, and playing with toys on the floor. The toddlers were observed getting their diapers changed, washing hands, playing on the playground, singing with their teachers, reading books, and eating lunch. Preschoolers were observed engaging in STEM activities, painting with spray bottles of watercolors, creating self-portraits, playing on the playground, participating in circle time, and playing freely in centers. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records. Seven staff and fifteen children’s files were reviewed. This program does not provide transportation; therefore, this requirement was not monitored. The storage of medications as well as corresponding medication permission forms were reviewed and found in compliance. The center serves children under one year of age, thus, safe sleep requirements were monitored and found in compliance. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. More than 12 months lapsed between the last two fire inspections (5/28/24 and 9/18/25). 10A NCAC 09 .0304(a) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. A staff member hired on 9/15/25 had a medical report on file that was more than 12 months old, dated 11/21/23. 10A NCAC 09 .0701(a) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by October 22, 2025. The letter must address each violation and explain specifically when and how it was corrected, as well as how you will prevent the same violations from happening in the future. Please send this to mandy.annunziata@dhhs.nc.gov and include the following information: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today’s visit, the center’s compliance history score was 98%. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. Failure to maintain compliance may result in an administrative action. The center was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact the center’s overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your center. TECHNICAL ASSISTANCE -Regularly inspecting the building for potential fire hazards should be a priority when caring for young children. Fire inspections are required yearly with no more than 12 months between inspection dates. Your last documented fire inspection was almost four months overdue. I suggest adding a reminder note on your calendar a month before the next inspection is due so you don’t miss this deadline. -When new staff are hired, they are required to submit a medical report and Tb test that were completed no more than 12 months prior to the first day of employment. One staff member who was recently hired (D.I.) submitted a medical report that was completed almost two years prior to her date of employment. Moving forward, please be sure staff members do not begin work in classrooms until the medical report and Tb test are on file and are no more than 12 months old. CONSULTATION -Per sanitation inspectors, centers must discontinue the use of baby gates to make kitchens inaccessible to children. If found at an inspection, the center will be placed into provisional status (which will be longer than 7 days since it will be a construction issue), and it will remain in that status until they have addressed the issue. -Criminal Backgrounds Checks for all staff members should be completed every five years. One staff member’s provisional qualification letter expires 10/10/25, and she may not work with children after that date unless she receives a new qualification letter. You may contact the CBC Unit via telephone (919) 814-6401 or by email DHHS.CBC.Unit@dhhs.nc.gov if you have questions or need assistance. -You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please review and update your EPR plan yearly. -The Clean Classrooms for Carolina Kids Program provides free facility-wide testing for lead in water and inspections for asbestos and lead-based paint hazards without any out-of-pocket costs. You are required to test for lead in your water every three years. TRAINING REMINDERS -Training from 10/17/24 to 10/7/25 was reviewed today and results can be found on the attached Staff/Training Worksheet. All required staff ongoing training hours for the current year should be completed by 10/17/25. -CPR and First Aid must be renewed every two years. New staff members must complete training within 90 days of hire. You stated that approximately 25 staff members will participate in CPR/First Aid renewal on 10/13/25. -Health and Safety Training must be renewed every five years. You can find this training in Moodle on the DCDEE website. New staff members must complete training within one year of hire. You stated that Bright Horizons requires monthly Health and Safety training that fulfills this requirement yearly. -“Recognizing and Responding to Suspicions of Child Maltreatment” (part of the Health and Safety Training) should be completed within 90 days of hire and renewed every five years. This training can be found on the Positive Child Alliance NC website: https://preventchildabusenc.org/online-trainings/ -ITS-SIDS training must be renewed every three years. Staff members who completed training in 2022 are due to renew in 2025. New staff members working with infants must complete training within two months of hire, and must not be left alone with infants during that time period. Upon arrival I was greeted by Shay, the Business Manager. Mary Ann Whitley, the Director assisted with the visit. I completed a general walk-through of the center. At the time of the visit, there were a total of 133 children present, ranging in age from infant to five (5) years old. The children were observed participating in outdoor activities on the playground, transitioning to lunch and completing routine tasks. LICENSE STATUS The center currently operates with a Five-star license issued on August 5, 2019, earning six (6) points in Staff Education, six (6) points in Program Standards and one (1) Quality point. INSPECTIONS *The last fire drill was conducted 4/28/24. *The last emergency drill was conducted 3/13/24. *The last playground inspection was completed 5/14/24. *The last sanitation was completed on 1/24/23 with a 'Superior' classification. *The fire inspection was completed on 5/28/24, and approved for daytime care only. MONITORING During today’s visit, a partial assessment of Child Care Requirements was conducted for all classrooms. The monitoring included CPR/First Aid, criminal background checks and special training requirements of newly hired staff. The monitoring also included License and permit restrictions, staff/child ratios, supervision, storage of hazardous items, medication, materials/equipment, and all space occupied by the children including outdoor play environment. The following violation was observed and documented. Violation Number Comment Rule 843 A drug or medicine was administered after its expiration date. An Epi-Pen located in the Toddler 4 space was observed with an expiration date of 4/2024. 10A NCAC 09 .0803(1)(d) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before June 13, 2024, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE During today’s visit, the information below was shared with you to maintain quality within your program and to prevent further violations. MEDICATION During today’s visit an Epi-Pen was observed with an expiration date of 4/2024. The director removed the Epi-Pen and contacted the parents during the visit requesting for a new Epi-Pen. I would recommend having a member of management to use a checklist monthly to ensure that all medications are up to date. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. RATED LICENSE STATUS On Monday June 12, 2023, the hold harmless legislation was extended and signed into law. All facilities are allowed to remain at their current star level without reassessment until June 30, 2024. Your facility has been determined to be in Cohort 2. Your reassessment year will be between July 1, 2025, and June 30, 2024. I will reach out to you prior to the beginning of your preparation year. Please use this time to make sure all your staff have a WORKS account with all education information updated. Also review updated resources on the North Carolina Rated License Project (NCRLAP) website (www.ncrlap.org). HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety trainings must be renewed at least every five (5) years. If you completed the trainings in 2017, you should begin to retake the training now. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. AGENCY UPDATES Please remember to check the Division of Child Development and Early Education website (ncchildcare.gov) on a weekly basis to stay up to date on all new policies and rules.
