You, Shanita Herring, Assistant Director, assisted me during the visit. Your program currently operates with a five-star license, issued January 18, 2018, earning five (5) points in staff education, seven (7) points in program standards and one (1) quality point. The last annual compliance visit was conducted on 3/10/25. The sanitation inspection was completed 10/6/25 with a “Superior” classification. The last fire inspection was conducted on 10/9/25, and your facility was approved for daytime care only. *The last documented fire drill and playground inspection were completed on 1/30/26. *The last documented shelter-in-place drill was completed on June 25, 2025. Prior to today's visit I reviewed the NC Secretary of State's website and observed that the owner of this facility, Bright Horizons Children’s Center, LLC is listed as current/active. You visited each indoor and outdoor space with me. Children throughout the facility were participating in group time, free play in activity areas, transitions, personal care routines, in outdoor play and eating lunch. Infants received care according to individual needs including diapering and bottle feeding. Three violations were observed today. One was corrected during the visit, and the others must be corrected immediately. The violations are as follows: Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. Three bottles in space #1 were not dated. 15A NCAC 18A .2804(d) 601 Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. The refrigerator did not have a thermometer in space #1 (infant classroom). 15A NCAC 18A .2806(j)(2) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last lockdown drill was conducted on 6/25/25. .0604(u);.0302(d)(8) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The program’s compliance history was 92% as of 2/20/26. The violations documented must be corrected immediately. On or before March 9, 2026. I must receive a written, dated, and signed compliance letter that describes accurately and in detail how the violations were corrected. Email the letter to: Kia.Reid@dhhs.nc.gov and include the following information. -Name, position -Facility name -Facility ID number -Your signature TECHNICAL ASSISTANCE -Emergency drills, such as lockdown/shelter-in-place drills, must be conducted in a timely manner to allow staff and children to practice and prepare for an actual emergency. I suggest setting a reminder on your calendar to help you maintain compliance with this rule. ANNUAL COMPLIANCE RECORDS Children’s records and staff files were monitored. The on-going training requirements were monitored from March 2025-February 2026 and was in compliance. RATED LICENSE INFORMATION The facility is interested in Pathway #2. During today’s visit we reviewed the requirements to achieve a five-star license through Pathway #2. I explained the timeline for completing the rated license reassessment. A QRIS Conversation Template was completed, and the facility plans to apply for a Rated License by June 2026. The director, Alex Roberts, was present and assisted me during the visit. The visit summary was reviewed with Ms. Roberts and a copy was given to her at the conclusion of the visit. Upon arrival I was greeted by Ms. Roberts. We completed a general walk through of the indoor and outdoor areas. The infants were being fed, changed and having tummy time. The toddlers were observed in activities with their teachers, in routine care, and eating lunch. The preschool children were observed in outdoor play and preparing for lunch. CURRENT LICENSE STATUS Currently this center operates with a five-star license issued January 8, 2018, earning five (5) points in staff education, seven (7) points in program standards and one (1) quality point. INSPECTIONS *The last annual compliance visit was conducted on March 10, 2025. *The sanitation inspection was completed June 11, 2025, with a "Superior" classification. *The fire inspection was completed on September 9, 2024, and is approved for day time care only. *The last documented fire drill was conducted on August 26, 2025. *The last documented playground inspection was completed on August 29, 2025. *The last documented lockdown drill was conducted on June 25, 2025. During this visit, a partial assessment of Child Care Requirements was conducted monitoring all classrooms. The monitoring included License and Permit Restrictions, Staff/Child Ratios, Supervision, CPR/First Aid coverage for the center, Special Training Requirements, Storage of Hazardous Items and Medication, Use of Approved Space, Criminal Background Checks, Program Records and Materials/Equipment for the Indoor Environment. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was completed on 9/4/24. 10A NCAC 09 .0304(a) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. In space #5 a tube of Desitin was on a cart within the children's reach, and a jar of Vaseline was on the diaper changing table. Both of the non-prescription creams were not made inaccessible to the children. In space #6 a purse with prescribed medication was on the cabinet within the children's reach not in locked storage. 15A NCAC 18A .2820(d) In order to comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be sent to me by September 29, 2025. The letter must address each violation and how it will be maintained in the future. Non-prescription diaper creams/sunscreens must be kept out of the reach of children at least 5 feet high. *You must notify the fire inspector to complete the annual fire inspection. The fire inspection is due within 12 months of the center's previous fire inspection. STAFF RECORDS I monitored eight new staff files since the last annual compliance visit in March 2025 for criminal background qualifying letters, current CPR/First Aid, medical, TB test and other specialized training. On your next annual compliance visit training hours will be monitored. CONSULTATION Automated Background Check Monitoring System North Carolina child care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE’s criminal background check system. Provider Access to ABCMS allows administrators to: •See the real-time background check status of staff members. •Run a printable report of the staff roster to assist with compliance visits. •See new background check applicants and add to staff roster. To get started, complete the ABCMS Child Care Provider Portal Training in Moodle—a video tutorial followed by a few questions. QRIS REQUIREMENTS *I encourage you to visit the DCDEE website in preparation for the transition to the new Quality Rating Information System (QRIS). Facilities will be offered a choice of three pathways to demonstrate quality and earn a 2-5 Star Rated License. The infants and toddlers were observed in activities with their teachers, water play outside and eating lunch. The preschool children were observed engaged in circle time and preparing for lunch. CURRENT LICENSE STATUS Currently this center operates with a five-star license issued January 8, 2018, earning five (5) points in staff education, seven (7) points in program standards and one (1) quality point. CONSULTATION STAFF RECORDS I monitored six new staff files since the last annual compliance visit in March 2023 for criminal background qualifying letters, current CPR/First Aid, medical, TB test and other specialized training. They were wearing raincoats with hoods. Lunch today would consist of chicken nuggets, green beans, peaches, and milk. Catherine Finley, Director, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Finley, Shanita Bellfield, Health and Safety Director, and several additional staff members. Limited monitoring of child care requirements occurred during today’s visit. Violation Number Comment Rule 303 Children were not adequately supervised at all times. On more than one occasion, staff members failed to provide adequate supervision which led to multiple biting incidents between two to three-year-old children.
