Name of Operation: BIG WORLD LITTLE PEOPLE CHILD CARE Facility ID: 32001811 Consultant: KIA REID Operation Type: Family CC Home Case Number: Visit Date: 12/18/2025 Number Present: 3 Completed Date: 12/18/2025 Age: From 1 To 3 Total Minutes: 105 Time In: 10:00 AM Time Out: 11:45 AM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for a routine unannounced visit. You, Tabatha Allen, Owner/Operator, assisted me with the visit. Your program operates with a four-star license issued May 15, 2024, earning seven (7) points in Staff Education, two (2) points in Program Standards and one (1) Quality Point (serves no more than two infants under age one). The facility is approved for daytime and overnight, no children are enrolled on 2nd or 3rd shift, no infants are enrolled and there are no four-year-old children enrolled. The last annual compliance visit was conducted on May 8, 2025. The license was posted, and the restrictions were in compliance. A walk-through of the fcch was completed today, monitoring the indoor and outdoor areas. I observed children in both the indoor and outdoor learning environments and found supervision and space capacity to be in compliance. Children throughout the fcch were participating in group time, free play in activity areas, transitions, personal care routines, and eating lunch. Two violations were observed today and must be corrected immediately. The violations are as follows: Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. The last documented outdoor inspection was completed on 10/20/25. 10A NCAC .1721(e)(5)(A-F) 1853 The operator did not conduct a monthly fire drill. The last documented fire drill was conducted on 10/2/25. .1719(a)(15) & .1721( e)(2) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The program’s compliance history was ninety seven percent as of 12/8/25. The violations documented must be corrected immediately. On or before January 1, 2026. I must receive a written, dated, and signed compliance letter that describes accurately and in detail how the violations were corrected. Email the letter to: Kia.Reid@dhhs.nc.gov The compliance letter must include: -Facility name -Facility ID -Your name and signature TECHNICAL ASSISTANCE Setting a reminder on your phone or computer would help to keep track of monthly inspections. OPERATOR RECORDS Your file was monitored for criminal background qualifying letters, current CPR/First Aid, and other specialized training. On your next annual compliance visit, on-going training hours will be monitored. RATED LICENSE INFORMATION Your next Rated License is due May 2027; however, I encourage you to visit the DCDEE website to review the new QRIS REQUIREMENTS, Pathways to the Stars. We are going away from a point-based system allowing you to have three different pathways to achieve stars. REMINDERS The following requirements will come due in the near future; please make arrangements to complete the items before the deadline. *Your health questionnaire will expire on 1/3/26. *Your ITS-SIDS training will expire on 3/9/26. *Please check the Division of Child Development and Early Education website: ncchildcare.ncdhhs.gov frequently and click the “what’s new” tab to stay informed of all updates available to you. The provider was present and assisted me when needed. This documentation was reviewed with the provider, and a copy was given to her at the conclusion of the visit. Upon arrival, I was greeted by the provider. I completed a general walk through of the areas used by the children inside and outside. There were four (4) preschool children present, and they were observed building with manipulatives then preparing for nap. CURRENT LICENSE STATUS Currently this family child care home operates with a four-star license issued May 15, 2024, earning seven (7) points in Staff Education, two (2) points in Program Standards and one (1) Quality Point. INSPECTIONS *The last annual compliance visit was conducted on May 15, 2024. *The last fire drill was conducted on September 4, 2024. *The last documented outdoor inspection was conducted on October 17, 2024 *The last shelter-in-place drill was conducted on October 17, 2024. DESIGNATED CHILDREN’S SPACE In the children’s space, there are a variety of materials for the children to explore throughout the day. The bathroom is accessible to those that are potty trained. Activities take place in the child care space and the children eat in the kitchen. *The outdoor play area is in the backyard of the home and is fenced. There is one shed on the property. *The provider informed me of the following: -Medication is not being administered to any of the children. -Transportation is not provided at this time. -No infants are enrolled at this time. During this visit, a partial assessment of Child Care Requirements was conducted. The items monitored were Supervision, Space Capacity, Discipline, License Posted, Permit Restrictions, Adequate & Approved space, Program Records, Staff Records, CPR, First Aid, ITS-SIDS, Criminal Background Checks, Storage of Hazardous Substances, and Storage of Medication. The following violation was cited today. Violation Number Comment Rule 1831 A valid qualification letter was not on file and available for review at the facility. The qualification letters for the provider and household member was not on file for review. G.S. 110-90.2 (b) & (d) & .2703(e) .1702(b)(1) 1853 The operator did not conduct a monthly fire drill. The last documented fire drill was conducted on 9/4/24. .1719(a)(15) & .1721( e)(2) To comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be sent to me by November 20, 2024. The letter must address each violation and explain how it has been corrected. Please send this to Kia.Reid@dhhs.nc.gov and include the following: -Facility name -Facility ID number -Each item number TECHNICAL ASSISTANCE I explained to Ms. Allen that although she and the household member are qualified, and the letters don't expire until 2026, the child care rule requires the qualification letters to be kept on file. Ms. Allen stated that she must have thrown the paperwork away while cleaning out her file. Ms. Allen contacted the CBC unit during the visit and was informed she needed to reset her NCID. ON-GOING TRAINING HOURS On your next annual compliance visit training hours will be monitored.
