    Name of Operation: ART_BEE HOME DAY CARE Facility ID: 32002164 Consultant: SHANNEL HALL Operation Type: Family CC Home Case Number: Visit Date: 11/5/2024 Number Present: 5 Completed Date: 11/5/2024 Age: From 0 To 2 Total Minutes: 225 Time In: 11:00 AM Time Out: 02:45 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to assess applicable child-care requirements during your annual compliance visit. The family childcare provider, Beatriz Arteaga, assisted me with today’s visit. Upon arrival, you greeted me and welcomed me into your program. Five children were present today. I conducted a general walk-through of the childcare spaces. I observed the children exploring their outdoor learning environment, eating lunch, and resting their bodies during nap time. RATED LICENSE STATUS The program holds a 2-star-rated license issued on 10/9/2019. The program is in the “held harmless” status and points from the previous assessment are carried forward. The program received 2 points in Program Standards, 2 points in Staff Education, and 1 quality point by Infant capacity restricted to no more than two children under one year of age. INSPECTIONS The last fire drill was conducted on 11/4/24. The last emergency drill was conducted on 10/1/24. The last playground inspection was conducted on 11/4/24. During this visit, a full assessment of Child Care Requirements was conducted with the following exceptions. You reported that you do not transport children. Therefore, that requirement was not monitored. The following violations were observed and recorded during today's visit. Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. One child did not have a completed medical form on file for review during today's visit. GS 110-91(1); 10A NCAC 09.1721(a)(1) In order to comply with the NC Laws & Rules, any violations cited today must be corrected immediately. A compliance letter should be sent to me by November 19, 2024. The letter should address each violation, and explain how and when it has been corrected. Please send the letter to shannel.hall@dhhs.nc.gov and include the following: -Facility name -Facility ID number -Each item number TECHNICAL ASSISTANCE The following items were discussed to help you prevent a reoccurrence of the violations observed during today's visit. I would suggest creating a calendar to chart when forms are due back to remain in compliance at all times. COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 99%. It will be updated after today's visit, and you will be informed of the new score. CONSULTATION Some providers are experiencing delays in scheduling their fingerprinting appointments. Criminal record checks can be completed up to six months prior to the expiration date. We are encouraging all staff to begin their renewal process early to avoid lapses in their qualification. ANNUAL IN-SERVICE TRAINING The in-service hours were monitored for the annual year November 8, 2023 – November 5, 2024. HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. You should use your NCID username and password to access the Moodle portal. As per Child Care Rule .1103(b) the Health and Safety trainings must be renewed at least every 5 years. Anyone who completed the trainings in 2017, should begin to retake the training now. AGENCY UPDATES Please remember to check the Division of Child Development and Early Education website: nchildcare.ncdhhs.gov frequently to stay updated on rule requirements. This documentation was reviewed with the provider, Beatriz Arteaga, and a copy of the visit summary documentation was given to her at the conclusion of the visit today. Upon arrival, I was greeted by Ms. Arteaga. I completed a general walk through of the child care space and outdoor play area. There were five (5) preschool children present during the visit. The children were observed in outdoor play, circle time, having their diapers changed and preparing for lunch. CURRENT LICENSE STATUS Currently this family child care home (fcch) operates with a two-star license issued October 9, 2019, earning two (2) points in staff education, two (2) points in program standards and one (1) quality point. The fcch is approved for daytime and overnight care. There are currently no infants enrolled and no children enrolled on second and third shift. INSPECTIONS *The last annual compliance visit was completed on November 14, 2022. *The last documented outdoor inspection and fire drill were completed on October 3, 2023. *The last documented shelter-in-place drill was completed on August 5, 2023. DESIGNATED CHILDREN’S SPACE The children are cared for in a space just past the entrance of the two-story home. In the children’s space, there are a variety of manipulatives, pretend play toys, books, and art materials for the children to explore throughout the day. The bathroom is located just past the entrance on the right and is accessible to those that are potty trained. The children eat their meals in the kitchen. *The children are prevented access to the upstairs level of the home by a gate. *The outdoor play area is in the backyard of the home and is fenced with one shed that is kept locked. *The provider informed me of the following: -Transportation is not provided at this time. -There have not been any incident/accident reports written since my last visit. -There are three pets in the home. -There are no children with allergies. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 1893 Application did not include all required information including, but not limited to: child's full name, named to be called, child’s date of birth, any allergies, including symptoms and the type of response required, and any fears or behavior characteristics a child has. One child's application did not include the hospital preference. 10A NCAC 09 .1721 (a)(3)(A)(B)(C)(E) 1894 Application did not include names and contact information of individuals to whom the operator may release the child as authorized by the person who signs the application. One child's application did not include this information. .1721(a)(3)(F) 2025 Operator and/or staff members did not complete a professional development plan within one year of employment. The provider did not complete a professional development plan. .1703(i) 2041 Application did not include the names and the phone numbers of the child's physician and the persons to be contacted in an emergency situation. One child's application did not include a physician and one child's file did not include emergency contact information. 10A NCAC 09 .1721(a)(3)(G)(H) In order to comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be sent to me by November 22, 2023. The letter must address each violation and explain how it has been corrected. According to NC General Statute 110-90 (4) (d) all facilities must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months. ANNUAL COMPLIANCE RECORDS The provider’s file, children’s records and program records were reviewed. You have not completed the required number of on-going training hours for the year. Please submit 3.5 hours to me by 11/15/23 of a violation of not completing training hours will be added to the visit. RATED LICENSE INFORMATION DCDEE has decided that Rated License assessments will resume, and each program will complete the process based on Cohort assignments (1-3). Your program is in Cohort 2. The planning period will start on 7/1/24 and the assessment period begins 7/1/25. -Use this time to create WORKS account. To create your account, go to the DCDEE website, click the provider tab, then click DCDEE WORKS. You can use your NCID to create your account. -Review updated resources on the North Carolina Rated License Project (NCRLAP) website (www.ncrlap.org). -Request technical assistance from CCSA -Participate in trainings related to the Family Child Care Environment Rating Scale CONSULTATION The U.S. Consumer Product Safety Commission has recalled numerous products. You can find more information about each item by accessing www.cpsc.gov. *The U.S. Food and Drug Administration (FDA) has issued an alert. FDA advises parents and caregivers not to buy or feed WanaBana Apple Cinnamon Fruit Puree Pouches to toddlers and young children because of elevated lead levels.
