I was accompanied by Child Care Consultant Amanda Annunziata. Upon arrival, we were greeted by the provider, Angela Northern, who assisted us during today’s visit. LICENSE STATUS Currently this program operates with a Four-Star License issued December 5, 2019. The program earned seven (7) points in education standards, two (2) points in program standards, and one (1) quality point. INSPECTIONS/DRILLS STATUS -The last documented monthly outdoor play area inspection was completed on 06/10/2024 -The last documented monthly fire drill was conducted on 06/10/2024. -The last documented quarterly lockdown/shelter-in-place drill was conducted on 04/06/2024. -The last annual compliance visit was conducted on July 19, 2023. MONITORING I completed a walk-through of the home, including the children’s space, kitchen, bathroom, and outdoor play area. At the time of the visit, there were two children present ranging in age from one year to three years old. There are four children enrolled. The children were observed engaging in block play and pretend play. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, capacity, health and safety requirements, equipment and furnishings, and program records. The provider does not transport children; therefore, this requirement was not monitored. No enrolled children have medications or medical care plans, so these requirements were not monitored. There is one child 12 months or younger enrolled; however, the child has not attended since May 2024. Therefore, safe sleep records were not available for review because they had been discarded after 30 days. The provider verified that all the information on the FCCH Verification of Required Information for Operator and Additional Caregivers form was correct. VIOLATION The following violation was observed and documented during today’s visit: Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. A child enrolled on 1/4/21 did not have a health assessment on file during today's visit. GS 110-91(1); 10A NCAC 09.1721(a)(1) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by July 29, 2024. The letter must address each violation and explain how it has been corrected. Please send this to mandy.annunziata@dhhs.nc.gov and include the following information: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today’s visit, the facility’s compliance history score was 99%. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. Failure to maintain compliance may result in an administrative action. The facility was not in compliance with all applicable minimum licensing requirements during today’s visit and a violation was documented that could impact the center’s overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your program. TECHNICAL ASSISTANCE During today’s visit, the information below was shared with you to maintain quality within your program and to prevent further violations. -It is important to keep children’s medical records on file to ensure children are healthy and able to participate in activities. Today, one child was missing a health assessment. I recommend using the children’s file checklist under “Provider Documents” on the DCDEE website and checking files regularly to help you keep track of required records. CONSULTATION The following items were discussed during today’s visit to improve the quality of your program. During today’s visit we observed 4 ongoing training hours completed. You are required to complete 5 ongoing training hours annually. Please submit one additional training hour on or before 07/19/2024 to remain in compliance. In addition, please find documentation supporting health and safety training completed in 2022 so that your records may be updated. TRAINING REMINDERS -Training from 7/20/23 to 7/14/2024 was reviewed today and documented on the Verification of Required Information for Operator and Additional Caregivers form. All required training hours should be completed by July 19, 2024. -CPR and First Aid must be renewed every two years. Your CPR and First Aid expires on 07/15/2025. -Health and safety training must be renewed every five years. Your training expires in 2027. -ITS-SIDS training must be renewed every three years. Your training expires on 10/11/2024. RATED LICENSE STATUS The NC State Legislature has extended “hold harmless” status until new rated licensing guidelines are in place, and you may opt to keep your current license and delay your rated license assessment until the new guidelines are adopted. Please use this extra time to update your WORKS account with your education information. You can visit the North Carolina Rated License Project website (https://www.ncrlap.org) for resources regarding the rated license assessment process and the FCCH self-study.
