The front desk administrator, Nida Abbasi, was present and assisted me during today’s visit. Upon arrival, you greeted me and welcomed me into your program. I completed a general walk through of one (1) classroom and their outdoor learning environment. The children were observed exploring their outdoor learning environment. RATED LICENSE STATUS The program operates with a Notice of Compliance reissued on November 24, 2023. INSPECTIONS The last fire drill was conducted on 4/21/26. The last emergency drill was conducted on 3/24/26. The last playground inspection was conducted on 4/21/26. The last fire inspection was conducted on 10/21/25. The last sanitation inspection was conducted on 4/8/26. During today’s visit, a full assessment of the Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. You stated that you do not administer medication, therefore, this requirement was not monitored. The following violations were observed or recorded during today’s visit. Violation Number Comment Rule 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. One staff member did not have a completed emergency information form on file for review during today's visit. .0701(a) 1041 Prior to employment a Criminal Background Check was not completed. Two employees did not have an acceptable criminal background check on file for review during today's visit. G.S. 110-90.2(b) 1867 The depth of the loose surfacing was not based on critical height of the equipment. There was less than 2 inches of mulch around the climbing structure fall zones on the playground for the children ages 3 to 5 years. .0605(k)(1-4) 1896 At least one staff member counted to comply with the rule did not complete the training in playground safety within six months of employment date. No staff member had a playground safety training certificate available for review during today's visit. .1102(e ) 9999 Designated emergency medication was not stored out of reach of children at least five feet high, but not required to be in locked storage. A child's epi-pen was stored in the EPR bag which was hanging less than five feet high. This a violation of a requirement in 10A NCAC 09.1719(a)(5). In order to comply with the NC Laws & Rules, any violations cited today must be corrected immediately. A compliance letter should be sent to me by May 13, 2026. The letter should address each violation, and explain how and when it has been corrected. This will allow you to track when you should order mulch to stay in compliance. We are encouraging all staff to begin their renewal process early to avoid lapses in their qualification. HEALTH AND SAFETY TRAININGS Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings dare provided free of charge. You should use your NCID username and password to access the Moodle portal. As per Child Care Rule .1103(b) the Health and Safety trainings must be renewed at least every 5 years. If you completed the trainings in 2017, you should begin to retake the training. Zainab Qaabidh, Director was present and assisted me during the visit today. The visit summary was reviewed with the director and a copy was emailed to her at the conclusion of the visit. Upon arrival, I was greeted by the director. We completed a general walk through of one (1) classroom and the outdoor play area. The children were observed eating a morning snack, circle time and preparing for outdoor play. CURRENT LICENSE STATUS Currently this center operates with a Notice of Compliance issued September 12, 2022. INSPECTIONS *The last annual compliance visit was conducted on September 8, 2022. *The fire inspection was completed on August 11, 2023, and the facility is approved for day time care only. *The last fire drill and playground inspections were both completed on August 22, 2023. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered.
