    Name of Operation: A NEW BEGINNING III Facility ID: 32001908 Consultant: SHAMEQUA WILKERSON-HARRIS Operation Type: Center Case Number: Visit Date: 3/11/2026 Number Present: 17 Completed Date: 3/11/2026 Age: From 0 To 4 Total Minutes: 110 Time In: 09:15 AM Time Out: 11:05 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to complete your Routine Unannounced visit. Upon arrival I was greeted by Ms. Wilkerson, the Preschool Teacher. Mrs. Peterson, the Director of A New Beginning II arrived to check on both centers. The Director, Carnilla Smith, arrived within 10 minutes and assisted me with the visit. I completed a general walk-through of the center. At the time of the visit, there were a total of seventeen (17) children present, ranging in age from infant to four (4) years old. The children were playing with materials, interacting with the teachers, eating lunch, participating in circle time, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a Five (5) star license issued January 12, 2018, earning seven (7) points in education, six (6) points in program standards and one (1) quality point. MONITORING During today’s visit, a partial assessment of Child Care Requirements was conducted for all classrooms. The monitoring included CPR/First Aid, criminal background checks and special training requirements for newly hired staff. The monitoring also included License and permit restrictions, staff/child ratios, supervision, storage of hazardous items, medication, materials/equipment, and all space occupied by the children. The outdoor play environment was not monitored due to rain during the time of the visit. INSPECTIONS *The last fire drill was conducted on 2/9/2026. *The last emergency drill was conducted 2/9/2026. *The last playground inspection was completed 3/10/2026. *The last sanitation was completed on 10/21/2025 with a “Superior ‘’ classification. *The fire inspection was conducted on 5/22/2025. The following violations were observed and documented during today’s visit. Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. In space #3 two bottles were observed stored in the refrigerator with the date of 2/16/26. 15A NCAC 18A .2804(d) 608 Children did not wash their hands upon arrival at the center, after each visit to the toilet, before eating, before and after water activity play, after outside play, and after handling animals or animal cages. A child in space #2 did not have her hands washed after sneezing causing body fluids to spread on the table. 15A NCAC 18A .2803(c) 609 Staff did not wash their hands thoroughly before beginning work, before/after handling food, before bottle feeding or serving to other children, after toileting or handling body fluids, after diaper changing and after handling soiled items. A staff member in space #2 did not wash her hands after cleaning up body fluids. 15A NCAC 18A .2803(a) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. Albuterol medication was not kept in the original labeled container or a prescription label. .0803(2)(a) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. A child that was transported from home to school did not have emergency and identifying information on the bus. 10A NCAC 09 .1003(d) 1128 For routine transport of children to and from the center, staff did not use the list to document attendance as children boarded and departed the vehicle. A child transported from home to school and from school to home did not have documented attendance as child boarded and departed the vehicle. 10A NCAC 09 .1003(l) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before March 25, 2026, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. Please make sure to include: -Facility name -Facility ID number -Each item number The letter can be emailed to Shamequa.Wilkerson-Harris@dhhs.nc.gov COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 100%. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. NUTRITION FOR INFANT/TODDLERS It was observed in the infant room two bottles stored in the refrigerator not labeled with the current date. The date observed on the bottles was 2/16/26. The teacher discussed with me that they reminded the parents to label the bottles, however, they do not. It is important to make sure all bottles are labeled with a current date and name to avoid serving bottles that may be spoiled. I would recommend discussing with the staff to check the bottles before they are placed in the refrigerator. HANDWASHING In space #2 a child was observed sneezing causing body fluids to get on the table. The teacher used gloves and cleaned the table. The teacher did not wash the child or her hands afterwards. I discussed with the teacher that she would need to wash her hands and the child’s hands. being dropped off in space #4. The child did not wash their hands when entering the classroom. Proper handwashing can lower the spread of sickness in the center. MEDICATION A breathing machine was observed in space #1 without the medication being in a prescription box or the prescription label. The teacher stated the box may have fallen out and been thrown away. A prescription label or box is needed for each prescription medication. I would recommend having the parents drop off all medications with the Administrator to ensure all items are collected to remain in compliance. TRANSPORTATION A child was observed not having any documentation or emergency and identifying information on the bus. The child also did not have documentation of attendance as the child boarded and departed the the bus. The bus driver stated that the child normally rides in the afternoon. It is important that attendance documentation, emergency and identifying information is on the bus. I would recommend re-training all staff on transportation policies. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. QRIS Modernization Mrs. Warthen was not available at the center during the visit. The Director stated that Mrs. Warthen will contact me with the Pathway she would like to consider. HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety trainings must be renewed at least every five (5) years. If you completed the trainings in 2021, you should begin to retake the training now. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. AGENCY UPDATES Please remember to check the Division of Child Development and Early Education website (ncchildcare.gov) on a weekly basis to stay up to date on all new policies and rules. The owner, Mrs. Darnella Warthen, assisted me in today’s visit. Upon my arrival, I was greeted and welcomed into your program. Two (2) classrooms were opened today. I conducted a general walk-through of the indoor and outdoor learning environments. I observed the children resting their bodies during nap time. RATED LICENSE STATUS The program holds a 5-star-rated license issued on 5/3/18. The program is in the “held harmless” status and points from the previous assessment are carried forward. The program received 6 points in Program Standards, 7 points in Staff Education, and 1 Quality point through the programmatic option of having a staff benefits package and an infrastructure of parent involvement. INSPECTIONS The last sanitation inspection was conducted on 10/4/23. The last fire inspection was conducted on 6/16/23. The last fire drill was conducted on 12/5/23. The last emergency drill was conducted on 10/9/23. The last playground inspection was conducted on 12/6/23. During this visit, a partial assessment of the child care requirements was conducted. The following items were monitored and found in compliance during this visit: nurturing interactions, daily attendance, license posted & permit restrictions followed, safe indoor environment, fire drills & inspections, CSC qualifications, CPR/First Aid certification, and storage of medications & hazardous materials. The following violations were observed and recorded during today's visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. The playground for the older children had broken wood, bricks, drop cords, and other hazardous items. 10A NCAC 09 .0601(a) In order to comply with the NC Laws & Rules, any violations cited today must be corrected immediately. A compliance letter should be sent to me by December 27, 2023. The letter should address each violation, and explain how and when it has been corrected. Please send the letter to shannel.hall@dhhs.nc.gov and include the following: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 96%. It will be updated after today's visit, and you will be informed of the new score. TECHNICAL ASSISTANCE The following items were discussed to help you prevent a reoccurrence of the violations observed during today's visit. I would suggest making sure that you are making sure that all items are properly stored in one area and in placed out of the areas that are designated for the children during school hours. CONSULTATION Criminal background checks must be renewed every five (5) years. Criminal background checks can be completed up to six months prior to the expiration date. We are encouraging all staff to begin their renewal process early to avoid lapses in their qualification. RATED LICENSE REASSESSMENT This facility prep year is from July 1, 2023-June 30, 2024. The facility reassessment year will not begin until July 1, 2024. During this time, the provider will decide whether they wish to have the Environment Rating Scale Assessment conducted. We discussed making sure the provider WORKS account reflects her current degree. We discussed reviewing the NCRLAP website ERS resources (www.NCRLAP.org) for updated information. The director is aware that she can request an ERS assessment free of charge during the preparation, and depending on the scores, they can be used as guidance to enhance program quality. Technical assistance can be requested anytime during this process. HEALTH AND SAFETY TRAININGS Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. You should use your NCID username and password to access the Moodle portal. As per Child Care Rule .1103(b) the Health and Safety trainings must be renewed at least every 5 years. Some of your staff completed the trainings in 2017, they should begin to retake the training.
