There was no one present at the facility. There was not a notice posted on the front door entrance. I did not observe any vehicles in the driveway and parking lot Violation Number Comment Rule 303 Children were not adequately supervised at all times. On December 19, 2025, on more than one occasion, a staff member is observed on video footage going out of the classroom and leaving eight(8) three-year-old children with no supervision. In addition, on two separate occasions, two staff members are observed scrolling on their cellphones. .1801(a)(1-5) 402 Each child's diapers were not changed at areas designated exclusively for diapering, on a surface that was smooth, nonabsorbent, easily cleanable and of tight construction. Two three-year-old children were observed removing and changing their pull-up in the classroom while exposed, rather than in the restroom or designated diapering area. 15A NCAC 18A .2819(a) & (b) 405 A child's hands were not washed after each diaper change. Two three-year-old children's hands were not washed after diaper change. 15A NCAC 18A .2803(c)(2) 417 Children three years old and older were in care and the schedule did not include blocks of time assigned to types of activities, including active play, quiet play or rest. On December 19, 2025, the schedule did not include blocks of time for a nap, or a structural lesson plan. .0508(d)(1) 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. On December 19, 2025, a staff member was observed pulling a chair from underneath a three-year-old child as the child attempted to sit down, causing the child to fall to the floor. The staff member then hit the same child on the left side of the face with a piece of bread before placing the bread on the child's plate. Additionally, the staff member sprayed an unknown liquid from a spray bottle on the side of a separate three-year-old child's face and pushed the child in the back. G.S. 110-91(10) 904 A child was handled in a rough way, including shaking, pushing, shoving, pinching, slapping, biting, kicking, or spanking. On December 19, 2025, a staff member is observed hitting three-year-old child with a open hand and pulling the child's hair after the child spilled milk. The staff member also grabbed the child by the back of the neck and forced the child to their seat. On two additional occasions, the staff member pulled the child toward the corner of the classroom, threw the child's shoe towards their body, and kicked the child's chair while the child was seated. Additionally, the staff member grabbed another three-year old child by the upper arms on two occasions. No visible marks or bruises were observed at the time. In addition, the staff grabs another three-year-old forcefully on two different occasions by the upper arms. No marks or bruises were observed. Secondly, the staff thumps a three year old child on their right ear and hits the child in the face which resulted into a lip injury. .1803(a)(1) 1810 There was a substantiation of child maltreatment. On March 31, 2026, the Division substantiated child maltreatment based on the physical injury to a child.     Name of Operation: A NEW BEGINNING II Facility ID: 32001805 Consultant: SHAMEQUA WILKERSON-HARRIS Operation Type: Center Case Number: Visit Date: 7/29/2025 Number Present: 19 Completed Date: 7/29/2025 Age: From 1 To 4 Total Minutes: 210 Time In: 09:00 AM Time Out: 12:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to assess applicable child-care requirements during your annual compliance visit. A checklist was used to note the requirements monitored today. Upon arrival I was greeted by Joyce Peterson, the Director. The owner, Darnella Warthen arrived within an hour of the visit. Darnella Warthen assisted me with today’s visit. There was a total of Nineteen (19) children present, aging in range from one (1) to six (4) years old. The children were observed playing in centers, eating breakfast, playing outside, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a five (5) star license issued March 15, 2022, earning seven (7) points in education, six (6) points in program standards and one (1) quality point. I reviewed the NC Secretary of State’s website and observed the owner of the facility as ANB Early Care & Education, LLC listed as current/active. INSPECTIONS *The last fire drill was conducted on 7/18/2025. *The last emergency drill was conducted 6/27/25. *The last playground inspection was completed 7/9/25. *The last sanitation was completed on 6/6/25 with a ‘Superior’ classification. *The last fire inspection was conducted on 10/31/25. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. New staff CPR/FA, qualification letters and newly enrolled children files were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. Transportation is not provided; therefore, these requirements were not monitored. This facility does not have children under 12 months enrolled, therefore safe sleep requirements were not monitored. The following violations were observed and documented during today’s visit Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. In space #2 I observed 1 child age one and 6 children two years of age with one staff member. GS 110-91(7);.0713(a-d) 428 A current activity plan was not posted for each group of children for reference. The activity plan posted was dated 7/21-7/25. GS 110-91(12); .0508(a) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. Albuterol was observed not stored in it's original box or with the prescription label. .0803(2)(a) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In space #1 an EpiPen was observed with an expiration date of 6/30/25. In space #1 Benadryl was observed with an expiration date of 9/2024. .0803(12) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In space #2 plastic ziplock bags were observed in two cubbies in reach of children one and two years of age. .0604(q) 1898 Staff did not complete the health and safety training within one year of employment. During today's visit I observed four staff members Health and Safety training not completed. .1102(a) COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 91%. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. STAFF/CHILD RATIO Upon arrival I observed space 2 with six two-year-olds, and one child that was one year with one teacher. Lower ratios are most critical for infants and young toddlers (birth to twenty-four months) as infant development and caregiving quality improves when group size and ratios are smaller. In addition, the children’s physical safety and sanitation routines require a staff that is not fragmented by excessive demands. Child-staff ratios in child care settings should be sufficiently low to keep staff stress below levels that might result in anger with children. Caring for too many young children, in particular, increases the possibility of stress to the caregiver/teacher, and may result in loss of the caregiver/teacher’s self-control. Please be reminded that you must follow the ratio of the youngest child. The ratio with a child one year of age is 1 staff member to 6 children. HEALTH AND SAFETY TRAINING During the visit I observed Health and Safety training not completed by four staff members. I would highly recommend using your outlook calendar or a reminder to make sure all staff have completed the Health and Safety Training to remain in compliance. ACTIVITY PLAN The activity plan in space #2 was dated 7/21-7/25. The teacher stated that she was working on the lesson plan. I would highly recommend having the administrator check all lesson plans on Friday to make sure they are completed weekly and posted. PLASTIC BAGS (SAFE ENVIRONMENT) During the visit I observed zip lock bags in two cubbies. In one cubby the plastic ziplock bag contained cups and spoons, the other plastic ziplock back contained white out, scissors, and an expo marker. Please remember that plastic bags are hazardous to children two and under. I would highly recommend reviewing Chapter 3 of Caring For Our Children regarding Health Promotion and Protection. MEDICATION In space #1 I observed an EpiPen with an expiration date of 6/30/25, Benadryl with an expiration date of 9/2024. The albuterol was not in the original box or with the prescription RX. I would highly recommend checking all medications monthly for the expiration date and making sure all prescription medications are in their original box or prescription RX. I would also recommend that you view the website for Caring For Our Children regarding Children with Special Health Care Needs in Chapter 8 at https://nrckids.org.CFOC. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. HEALTH CONSULTANT Ellie Morrise is the new Child Care Health Consultant. If you should have any health related questions, please reach out to her at morrise@email.unc.edu and/or 919.809.9645. HEALTH AND SAFETY TRAINING Health and Safety training is now available through Moodle on the DCDEE website. This training is provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety training must be renewed at least every five (5) years. If you completed the training in 2020, you should begin to retake the training now. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. ONGOING TRAINING HOURS I observed training hours from 8/2/24 to 7/29/25. Please have F. Bowser complete the Health and Safety Training on or before 11/1/2025. The program records were reviewed for current inspections. Transportation is not provided; therefore, these requirements were not monitored. This facility does not have children under 12 months enrolled, therefore safe sleep requirements were not monitored. The following violations were observed and documented during today’s visit Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In space #3 the electrical outlet did not have a safety plug while the outlet was not in use. 10A NCAC 09 .0604(c) 885 A first aid kit was not available on site at all times. A first aid kit was not filled with first aid supplies in space #3 during today's visit. .0604(s) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. During today's visit the EPR plan was observed not updated as information changed. .0607(e) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before August 15, 2024, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. OUTLET COVERS During today’s visit I observed an outlet without an outlet cover in space #3. The teacher stated that during morning drop off she plays music for the children. She stated that she did not put the outlet cover back in the socket. I discussed with the teacher and the director the importance of making sure outlet covers is always used. I would recommend checking throughout the day to make sure all outlets are covered. FIRST AID KIT In space #3 a first aid kit was not available. The teacher stated that she has a list to fill the first aid kit. I would recommend having a staff member check the first aid kits weekly to make sure that appropriate materials are stored in the first aid kit. I would also suggest visiting the website Caring for Our Children website concerning First Aid and Emergency Supplies. (https://eclkc.ohs.acf.hhs.gov/health-services-management/caring-our-children-basics/first-aid-emergency-supplies) EMERGENCY PREPAREDNESS AND RESPONSE PLAN During today’s visit I observed the EPR plan not updated as contact information changed. To complete the Emergency Response Plan please log on to the website rmp.nc.gov/portal. You will need to use your NC ID to log in. Once you have logged in you will need to click on the link that says, ‘planning tools.’ Then click on the link that says ‘Childcare Emergency Preparedness and Response plan. Remember to continue to update your EPR plan every year and print out the cover page showing the current date. Also, print the pages you made changes to. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. HEALTH CONSULTANT Ellie Morrise is the new Child Care Health Consultant. CLEAN CLASSROOMS FOR CAROLINA KIDS I encourage you to enroll in the Clean Classrooms for Carolina Kids program to identify and eliminate exposure to lead and asbestos hazards in building infrastructure. This program is an expansion of Clean Water for Carolina Kids. Facilities are now required to meet rule requirements NC 10A NCAC 41C. 1001-1007, NC 154A NCAC 18A .2816. Clean Classrooms for Carolina Kids is designed to ensure facilities meet all requirements, receive direct support throughout the participation process, and have access to communication resources.
