The annual compliance monitoring checklist for FCCH was used to note requirements monitored today. Upon my arrival, I was greeted by the owner/operator, F. Geddie. I was unaccompanied as I completed a general walk-through of the indoor and outdoor environments. I observed three (3) children during today’s visit. The children were observed during free choice play, completing routine care tasks, and preparing for lunch. Interactions with the children were both positive and nurturing. There were a variety of age-appropriate books, toys, and materials in the indoor and outdoor environments. LICENSE STATUS: This FCCH currently operates with a three-star rated license issued on April 12, 2023. REQUIRED DRILLS/INSPECTIONS: The last annual compliance visit was conducted on March 3, 2025. The last documented monthly fire drill, the last documented quarterly lock down drill and/or shelter-in-place drill, and the last monthly playground inspection were completed on January 7, 2026. During today’s visit, a full assessment of the child care requirements was conducted. I observed all required postings and attendance logs. Currently, there are no infants enrolled, so safe sleep logs were not monitored. I monitored program/space requirements, equipment and furnishings, capacity & supervision, storage and/or administering of medication, outdoor area & equipment, the operator’s file, and children’s records. The operator was in compliance with the required training including CPR, first aid, and SIDS. The qualification letter for the operator and her household member was on file and available for review. The operator reported that she has one (1) dog, a Yorkshires Terrier named Chase. She also stated that she does not have an additional caregiver and does not provide transportation. The Verification of Required Information for Operator and Additional Caregivers form and the Operator’s Statements of Responsibility form were completed during today’s visit. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 716 Electrical outlets not in use were not covered. An electrical outlet accessible to children was uncovered. 10A NCAC .1719(a)(27) 908 Health questionnaire was not completed annually. The operator completed the last annual health questionnaire on January 7, 2025, which was not updated annually as required by the rule. .1703(a)(1) 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). The operator qualification letter expired on December 10, 2025. The operator did not complete the criminal background check and obtain a current qualification letter until January 30, 2026. G.S. 110-90.2 & .2703(n) & (o) 1951 Animal cages were not kept free of animal waste. During today’s visit, animal waste was observed in the indoor dog cage. .1719(b)(2) COMPLIANCE HISTORY: Prior to today’s visit, the 18-month compliance history for this FCCH was 83%. A compliance verification letter was not required because the violations cited were corrected during today’s visit. TECHNICAL ASSISTANCE/CONSULTATION: During today’s visit, technical assistance was provided regarding the requirement to maintain animal cages free of waste in child care facilities. This practice is essential to protect the health and safety of children, as animal waste can harbor harmful bacteria, viruses, parasites, and fungi that may cause illness, particularly in young children whose immune systems are still developing. Unsanitary cages may also attract insects and rodents, increasing the risk of disease transmission. Maintaining clean animal cages or kennels supports a safe, sanitary, and healthy learning environment for children. As a reminder, all child care providers are required to complete a Criminal Background Check (CBC) and receive a valid qualification letter before the expiration date listed on their current qualification letter. Additionally, the qualification letter is valid for a maximum of five years from the date of issuance. Please review the North Carolina General Statute 110-90.2 and Section .2703 of the Child Care Rules, which cover requirements on Criminal Background Checks. During today’s visit, Ms. Geddie stated that she was not prepared to complete the rated license assessment. She stated that she has not completed the child observations as required in Pathway 2: Classroom & Instructional Quality Pathway to maintain a three star-rated license. Additionally, Ms. Geddie stated she has not completed the coaching and training options for the operator as required to meet the program standards for this pathway. Ms. Geddie stated that she is implementing Gee Whiz education family child care curriculum and will move forward with completing the child observations before her next compliance visit. I reminded Ms. Geddie if she experiences any challenges or barriers, to reach out to her child care consultant for support. Thank you for your time today. Upon arrival, I was greeted by the owner/operator, F. Geddie. I observed a total of six (6) children present during today’s visit which consisted of three (3) infants aged 12 months and under, two (2) three-year old children and one (1) school-aged child getting up from a nap. I observed the caregiver engaging in positive and nurturing interactions with all the children. LICENSE STATUS: This FCCH currently operates with a three-star rated license issued on April 12, 2023. REQUIRED DRILLS/INSPECTIONS: The last annual compliance visit was completed on March 14, 2024. The last documented monthly fire drill was conducted on July 10, 2024. The last documented quarterly lockdown/shelter-in-place drill was conducted on August 23, 2024, and the last documented monthly playground inspection was completed on July 10, 2024. During today’s visit, a partial assessment of the child care requirements was conducted. I monitored all required postings, storage of medication and hazardous products, attendance logs, and safe sleep logs, toys, and materials. Ms. Geddie is in compliance with the required training including CPR, First Aid, and SIDS. The qualification letters for Ms. Geddie and her household members were in compliance. The operator stated she does not have any pets, nor does she provide transportation. She also stated she does not have an additional caregiver. The Verification of Required Information for Operator and Additional Caregivers form was completed. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. Under the sink in the bathroom used by enrolled children, I observed a bottle of all-purpose glass cleaner and two (2) bottles of Isopropyl alcohol. In an unlocked laundry closet, I observed moth balls, a bottle of Clorox bleach, and a bottle of laundry detergent. .1719 (a)(7) 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. The last documented monthly playground check was completed on July 10, 2024. 10A NCAC .1721(e)(5)(A-F) 716 Electrical outlets not in use were not covered. Electrical outlets were observed uncovered in the space designated for child care, the hallway, and the kitchen. 10A NCAC .1719(a)(27) 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. The provider did not complete the attendance form documenting the arrival/departure of the children. .1721(e)(6) 929 Developmentally appropriate activities for the ages of children in care, were not provided as planned on the written schedule and activity plan. The operator did not have a written schedule and/or an activity plan G.S.110-91(12) & .1718(a)(6) 1804 Safe sleep policy and poster about safe sleep practices was not posted in a prominent place in the infant sleeping room or area where parents and caregivers are able to view daily. The operator did not have a copy of the safe sleep poster and policy posted as required by the rule. .1724(b) 1821 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of 30 days. The caregiver did not complete visual checks on the sleeping infants aged 12 months and documentation was not maintained on file for the last 30 days . .1724(a)(8)&(f) 1853 The operator did not conduct a monthly fire drill. The last documented monthly fire drill was conducted on July 10, 2024. .1719(a)(15) & .1721( e)(2) 1948 Sleeping infants, ages 12 months and younger were not visually checked every 15 minutes. The caregiver did not check on the sleeping infant, age 12 months and younger every 15 minutes and document .1724(a)(7) COMPLIANCE HISTORY: Prior to today’s visit, the 18-month compliance history for this facility was 100%. COMPLIANCE LETTER: All violations must be corrected immediately. A compliance letter is due to me within two (2) weeks of today’s visit. By October 22, 2024, please send me a letter addressing each violation, how the violations were corrected, and how compliance will be maintained in the future. In your letter, be sure to include your facility name, facility ID number and each item number for the violation cited. You may submit the letter to me electronically at tanya.herring@dhhs.nc.gov. If you elect to mail your letter, please ensure that you allow sufficient time so that the letter reaches me on time. My mailing address is: Tanya Herring 2201 Mail Service Center Raleigh, NC 27699-2200 TECHNICAL ASSISTANCE/CONSULTATION: During today’s visit, the safe sleep poster, safe sleep policy, and safe sleep logs were not in compliance with child care rules and requirements. These rules are extremely important and have been established to help reduce the risk of sudden infant death syndrome (SIDS) while infants are in child care facilities. Caregivers must check on sleeping infants at least every 15 minutes as required by Child Care Rule .1724 (a)(7), The rule goes on to discuss safe sleep practices for infants under the age of 12 months. Pacifiers must not to be clipped to the infant’s clothing and there should be no items in the crib or bassinet. These items include, but are not limited to, pillows, blankets, bumper pads, wedges, stuffed animals, toys, and bottles. SAFE INDOOR/OUTDOOR ENVIRONMENTS: All spaces accessible to children during operating hours must be checked prior to care on a daily basis to ensure that all electrical outlets are covered with safety plugs, all storages that contains hazardous products and aerosol cans are locked with an approved lock as defined in the child care rules, and both indoor/outdoor areas are free of debris, broken toys/equipment, and all other potential hazardous that can cause serious injuries or death.
