Name of Operation: DUNN HEAD START LEARNING CENTER Facility ID: 43000498 Consultant: LEIGH MCKENZIE-LEE Operation Type: Center Case Number: Visit Date: 7/31/2023 Number Present: 8 Completed Date: 7/31/2023 Age: From 2 To 3 Total Minutes: 35 Time In: 10:35 AM Time Out: 11:10 AM Time In: Time Out: List to Use: Center Type Of Visit: Admin Action Follow-Up Lic Announced/Unannounced: Unannounced The purpose of today's unannounced visit was to monitor compliance with the Corrective Action Plan of the Administrative Action issued by the Division of Child Development and Early Education on 1-12-23. An extension of the provisional license was issued on June 6, 2023. Your provisional license is now effective through January 12, 2024. A copy of the original action, extended action and the extended provisional license were observed posted in the lobby of the facility. The administrator was not present during the visit; however, I spoke with her by phone during the visit. I monitored the indoor environments for compliance with child care requirements. Staff/child ratios, supervision, capacity and use of space were monitored for compliance. The children were observed playing outside. A meal or snack was not observed during the visit. Stipulations of the Corrective Action Plan were monitored during the visit: Stipulation # 1 - No violations of sanitation requirements were observed during the visit. This stipulation will be monitored during future administrative action follow up visits. Stipulation # 2 - The remediation plan was submitted to Harnett County Environmental Health prior to the issuance of the administrative action. Therefore, this stipulation has been met. A Modification Request, dated 2-28-23, was sent to the Executive Director for the facility from Gale Violette, Harnett County Department of Environmental Health requesting changes to the plan submitted for remediation. An additional Modification Request, dated 3-13-23, was sent to the Executive Director for the facility. The administrator sent me an email explaining that the plan would not be approved until a contractor for the remediation work has been selected by Johnston Lee Harnett Community Action. She informed me during a telephone call that as of today, she has not been notified that a contractor has been selected. Please continue to notify me of any changes or additional modification requests. Please email any additional documentation, including modification requests or revised remediation plans. In addition, please ensure that you notify me once the plan has been approved so that I can monitor the timeframe established in stipulation # 3. Stipulation # 3 - This stipulation requires written verification that potential lead hazards have been remediate from Harnett County Health Department be submitted. The plan for remediation has not yet been approved, thus work has not started in the facility. This stipulation will be monitored during future visits. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The activity plans posted in spaces 5 and 6 were dated for 7-24-23 to 7-28-23. GS 110-91(12); .0508(a) ****Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules.**** Please send me a written statement describing how and when the above violation was corrected. The written statement must be received no later than 8-14-23. Please note that if your letter is mailed, a hand written signature is required. As an alternative, compliance letters can now be submitted via email. I recommend emailing your letter to reduce the time needed to ensure my receipt of your information; however, if you do not have access to email, please ensure that your letter is mailed in a timely fashion and that you allow for any possible delays in the mail service.
