Name of Operation: WAGES STEPHEN & SUSAN PARR FAMILY LEARNING CENTER Facility ID: 96000616 Consultant: TAMEKA JOHNSON Operation Type: Center Case Number: Visit Date: 11/15/2023 Number Present: 26 Completed Date: 11/15/2023 Age: From 0 To 3 Total Minutes: 60 Time In: 10:20 AM Time Out: 11:20 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose for today's unannounced visit was to monitor applicable child care requirements during your routine unannounced visit. Upon my arrival, I was greeted by the administrator, Perry Miller. I was accompanied by Ms. Miller as I completed a general walk-through of the indoor and outdoor areas, which consisted of four (4) classrooms, the kitchen, and the outdoor play environment. The children were observed during free choice activities, outdoor play and completing routine care tasks. Meals were observed and in compliance with the Meal Patterns. **Current License Status Currently this center operates with a four-star rated license issued November 10, 2023. ** Annual Inspections: The last sanitation inspection was completed July q9, 2023, and received a ‘Superior’ classification with 0 demerits. The last fire inspection was completed on February 23, 2023. The last documented fire drill was conducted on November 8, 2023, the last documented lockdown drill was conducted on November 8, 2023, and the last documented playground inspection was completed on October 31, 2023. During today’s visit, a partial assessment of Child Care Requirements was conducted. The monitoring included: supervision of children, staff/child ratio, storage of hazardous substances, storage of medicine, adequate/ approved space, posted license, license restrictions, attendance logs and program records. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. One outlet cover was missing in space B and space D. 10A NCAC 09 .0604(c) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The Shelter-in-place or lockdown drill was not document on the emergency drill log. .0604(u);.0302(d)(8) The violations cited during today’s visit were corrected. Therefore, a compliance letter is not needed. Technical assistance/Consultation: During today’s visit, I observed that the locked down drill was not recorded on the emergency drill log. Ms. Miller stated that she forgot to document the drill. As a reminder, child care rule .0604(u) states you must keep a record that includes the date of each drill, time of day, the length of time to reach the designated location and the signature of the person conducting the drill.
