A checklist was used to note the requirements I monitored today. The Operator’s Statement of Responsibility, and Verification of Required Information forms were received and monitored during the visit. Currently, this family child care home operates with a Three (3) Star License, issued March 21, 2025. This program has a current National Association for Family Child Care accreditation, which makes the program eligible to earn a Three Star-Rated license. The last temporary time period visit was completed February 10, 2025. The fire drills, lockdown drills, and outdoor inspections were completed as required. You, owner/operator, G. Royal, were present during today's visit. The program has maintained an 18-month compliance score of 97%. A copy of the compliance history was left with you. The Department of The Secretary of State website was checked prior to the visit under the name Royal 1 Care, LLC. The status of the corporation is current-active. It appears that the corporation is in good standing with the NC Department of the Secretary of State. You confirmed that the phone number and the mailing address have not changed. ADDITIONAL HOUSEHOLD MEMBERS: You stated that you and your husband are the only individuals living in the home. You were observed providing care to two children, ages two to four years of age. Children were observed play with a variety of items, having routine care needs met, and interacting with the caregiver. The interactions observed were nurturing and supportive. A copy of this computer- generated Visit Summary was reviewed and left with you at the conclusion of the visit. All violations were reviewed with you. I used the Annual Compliance Monitoring Checklist for Family Child Care Homes and the Item Number Listing (DCD 0357) as a basic monitoring tool to assess compliance with all applicable child care requirements. The following violations was documented: Violation Number Comment Rule 410 Copy of the required information for an individual was not on file in the home available for review. The qualifying letter for the provider and her husband was not available for review. The health questionnaire, medical information, record of trainings, EMC review, and shaken baby acknowledgement was not available for review. .1702(b)(1-9) 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. The required quarterly lockdown or shelter-in-place drills were not completed at required. The last drill was completed on 2/2025; therefore, a drill was required to be completed on 5/2025 and again on 8/2025. .1719(a )(16) & .1721(e )(7) CONSULTATION AND TECHNICAL ASSISTANCE: The violations documented above must be corrected immediately. A signed and dated letter of compliance must be mailed to me by September 25, 2025, describing how and when the violations were corrected. This two-week time frame is established to allow you time for submitting your compliance letter and is not intended to be used as a guide for correcting violations, as they should be corrected immediately. The letter of compliance should be mailed or emailed to me, Tina Jones, PO Box 10194, Goldsboro, NC 27532. The compliance letter must be submitted from the email address on file for your program. Please be aware that if it is determined that information submitted was inaccurate or willfully falsified, Administrative Action, including Revocation of the facility’s license could be issued. Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with the applicable laws and rules; always whether they have been monitored or specifically discussed with you in the past. The best way for you to ensure that you are meeting all requirements is to periodically review the child care laws and rules on the Division of Child Development and Early Education’s website. HEALTH AND SAFETY TRAINING: Health & Safety (Moodle): The Child Care Development Fund Health and Safety in Child Care Trainings, developed by the North Carolina Child Care Health and Safety Resource Center, are FREE and are located on the MOODLE training platform. You will need to have an NCID - the same NCID that is used to log in to WORKS and/or the CBC Portal. These modules can be found by using the "all courses" function or searching for CCDF - Health and Safety in Child Care in the search box at the bottom of the main Moodle page. The health and safety trainings include 10 topic areas. CPR, First Aid, and Recognizing and Responding to Suspicions of Child Maltreatment must be completed within 90 days of licensure. The remaining topic areas must be completed within your first year of licensure, and every five years thereafter. I encourage you to use the Health and Safety training log to ensure all topic areas are covered. Once you complete the health and safety trainings , print your certificates and maintain on file for review. The Health and Safety trainings will need to be completed by September 20, 2025. Health and Safety trainings are required to be completed so that topics are covered every five years. EPR PLAN: You have completed the required Emergency Preparedness and Response plan training. You will need to create an EPR plan in the Emergency Management portal within four months after completion of the training, which is October 26, 2025. Once completed, you should print it and have it available for review at your facility. You will need to complete and document shelter in place or lockdown emergency drills every three months. I recommend utilizing the Emergency Drill Log, located on the Division’s website, to assist in maintaining this requirement. I provided you a copy of the log today. ABCMS PROVIDER PORTAL: As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. QRIS: QRIS Modernization Update: On June 26th, the North Carolina Rules Review Commission (RRC) approved the QRIS Modernization rules adopted by the NC Child Care Commission during their April meeting. The Division of Child Development and Early Education is preparing a summary of the rules, an online training module for Moodle, and an implementation plan. Webinars were scheduled earlier this month to share the implementation plan and provided resources that can assist you with understanding the new rules. These webinars and additional information about the new QRIS is on the Division’s website. The implementation plan will allow time for administrators/operators to learn and understand the different pathways and the changes within our system. We encourage you to visit the QRIS Modernization page on the DCDEE website to learn more. Child Care Consultants will be having a meeting on September 25, 2027, from 3pm – 5pm at Cunningham Research Station, 200 Cunningham Road, Kinston, NC 28501. We will be discussing the new “Pathway to Stars”. You, Glendora Hinton, operator, assisted me with the visit. A temporary license was issued for this program on September 20, 2024, and will expire on March 20, 2025. The temporary license allows a first shift capacity of 4 children, ages 0 to 12 years old. Restrictions on the license include daytime and overnight care, children in care on ground level only, back door entrance used for child care, and the fireplace/woodstove is not used during operating hours. You are also an affiliated provider with the local air force base. During this visit, we walked through the indoor and outdoor environments and completed a full assessment of the licensed space. A variety of age-appropriate learning materials were observed in the child care area. Program records, including fire drills and monthly outdoor area inspections were also monitored. When asked, you have not been completing an outdoor area inspection each month. We referenced the outdoor inspection form today. You printed the form and posted it in the room used to care for children. Both fire drills and outdoor playground inspections must be maintained on file for one year after completion and kept on file for review. Your records and one child’s record were also reviewed. One child is currently enrolled in your program and the child was present today. He was observed sleeping upon arrival. He awoke during the visit and interacted with you. The following violations were documented today: Violation Number Comment Rule 702 Potentially hazardous items, including but not limited to, power tools, nails, chemicals, propane stoves, lawn mowers, and gasoline or kerosene, whether or not intended for use by children were not stored in locked areas, removed from the premises, or otherwise inaccessible to children. Two grills and a lawn mower were accessible in the back yard area. .1719 (a) (1) 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. Documentation was not on file verifying monthly outdoor inspections have been completed since September 2024. 10A NCAC .1721(e)(5)(A-F) 919 Accurate records were not maintained for all staff and children. The following were not on file for the one (1) child that was enrolled October 18, 2024: neither a hospital nor a physician preference, a copy of a medical exam, receipt of the shaken baby syndrome and abusive head trauma policy, receipt of the summary of NC Child Care law, a completed and signed pet verification form, and a signed written plan of care. G.S. 110-91(9) All violations must be corrected immediately. Item 702 was verified as corrected during the visit. You can still address the correction of the violation in your compliance letter. Please send me a compliance letter stating specifically that the corrections were made and describing in detail how and when they were corrected. The compliance letter must be received no later than December 17, 2024. You can email the compliance letter from the email address on file with the Division as the center’s official email address. Contact me if you have questions regarding correction of the violations documented during today’s visit. If you are unable to correct all violations within the specified timeframe, send a compliance letter regarding violations that were corrected and a specific plan to correct the remaining violations. If an extension is needed to correct any violations, prior approval must be given. You will need to email a request for an extension, if needed. A sample compliance letter will be emailed to you along with the visit summary to see how to create/develop a compliance letter. You should list the item number, specifically stating that the violation has been corrected, explaining when it was corrected, and how it was corrected. Compliance is not considered finalized until all violations have been corrected; therefore, if the compliance letter says an item will be corrected, it should be followed-up with another letter once it actually has been corrected. Please let me know if you have any questions regarding how to complete a compliance letter. TECHNICAL ASSISTANCE & CONSULTATION: OUTDOOR AREA: All licensed child care programs, centers and family child care homes, must have an outdoor play area that can be used by children. The area for outdoor play must remain free of any potentially hazardous items/materials during operating hours. You stated you were not currently using your outdoor play area and would take the child you have enrolled to the park for outdoor play. You showed me a sign you had attached to the door used for child care today regarding not using your outdoor area. You stated you were given permission to not use the outdoor play area from representatives with the local air force base. We reviewed the outdoor play area at your residence must be used for outdoor play and must remain free of potential hazards. You stated you recently used your home for entertaining and things were not properly stored. Your husband was home today, and he put up items such as two grills and a lawn mower. He also removed a four-wheeler and closed the outdoor storage shed. CHILDREN’S RECORDS: Children's records consist of various documentation such as a child's medical and immunization history, emergency medical care information, written plan of care, and parental acknowledgements. This information is the basis for meeting each child's physical, emotional, cognitive and social needs. When enrolling children into the facility all forms are needed on the first day with the exception of the medical and immunization reports, which must be received within 30 days of the enrollment date. Best practice is to also receive medical reports prior to enrollment. We discussed having two files containing all required information for the Air Force and one for required licensing documents/forms. This may help you to separate what is required for both agencies. Some of the information may be repeated but having two separate folders may better assist you to ensure all required information is received and on file. A medical exam can be accepted if completed within the last 12 months prior to a child’s enrollment provided the information listed on DCDEE’s medical form is answered. Ensure the parent completes the top section of the Children’s Medical Report form. A Children’s Records Checklist is available to assist with compliance. The form can be printed from the Provider Documents and Forms tab of the Division’s website. We have reviewed the Children’s Records checklist during previous visits. OUTDOOR INSPECTIONS: Outdoor inspections must be completed and documented monthly on the Outdoor Playground Inspection Checklist for Family Child Care Homes form. Documentation of the completion of a monthly outdoor inspection was not on file. The monthly checklist must be maintained for 12 months in your files. Completing your fire drill and monthly playground inspections on the same day is a great way to ensure compliance with this requirement. You can also set reminders with your phone or document when you plan to conduct a drill on a calendar. Ensure drills are conducted at different times each month. ANNUAL LICENSING FEES: An email was sent recently to remind family child care home providers that invoices for the annual license fee are due by December 31, 2024. We also discussed that the Raise NC newsletter is sent via email periodically to share information and updates. I encouraged you to ensure you are receiving the Division’s emails, and to check your spam or junk folder to ensure these emails are being properly delivered. Adding the Division’s emails to your contact list may also assist in ensuring properly delivery. Please notify me immediately of any changes to your email address. CHILD CARE RULES: You can access the NC Child Care rules from the Division of Child Development and Early Education’s website: https://ncchildcare.ncdhhs.gov, click on the “provider” tab, then “child care rules” tab. Download a copy of the NC Child Care rules onto your computer so you have access to the rules and can reference them as needed. We reviewed this training must be completed for your on or before December 20, 2024. Once completed, maintain a copy of the certificate on file for review. You completed CPR and First Aid on March 14, 2023. The remaining Health and Safety trainings will need to be completed by September 20, 2025. Health and Safety trainings are required to be completed so that topics are covered every five years. EMERGENCY PREPAREDNESS AND RESPONSE TRAINING: (EPR) Training: When asked, you have not yet completed the EPR training. The EPR training is required to be completed within the first year of licensure, on or before September 20, 2025. Once completed, you will need to create an EPR plan in the Emergency Management portal within four months after completion of the training. Once completed, you should print it and have it available for review at your facility. You will need to continue to complete and document shelter in place or lockdown emergency drills every three months. ON-GOING TRAINING: The ongoing training hours (in-service) will be monitored at your first annual compliance visit based on when you were initially licensed. No training hours were monitored today. The requirements are based on education and experience. You are required to complete twenty (20) hours of training annually. You may count the health and safety trainings toward this requirement. Maintain copies of your training certificates on file for verification. You can also use the On-Going Training Record Log to document the trainings you complete and attach your certificates to the log. Rated License: We discussed that you may voluntarily request to apply for a Two through Five Star Rated License at the end of the Temporary Time Period. A Three to Five Star license is required to participate in the Subsidized Child Care Program. We discussed that the Star Rated License is composed of the Program Standards Component, the Staff Education Component, and a quality point. An accumulation of points will determine how many Stars will be earned. I will bring you a copy of the Family Child Care Home Initial Rated License application during the next temporary time period visit. Staff Education: Points in this component will be determined by the Early Childhood courses you have completed at the college level. WORKS is a self-service applicant portal that will be used by an individual applicant to register and maintain their information to support education requirements. The WORKS portal is available on the DCDEE website and requires that the user maintain an individual North Carolina Identity Management (NCID) username and password. At this time, you have not created a WORKS account, therefore no education information has been submitted. When asked, you have not completed the North Carolina Early Childhood Credential. You stated you are currently enrolled at James Sprunt Community College but had some technical difficulties. You are unsure how the rest of the semester will be. Your license will not be delayed due to education information not being processed when education information is submitted to the Workforce Education Unit at DCDEE. It typically takes WORKS 4-6 weeks to complete an evaluation. We reviewed points you may be eligible for during today’s visit without the completion of the North Carolina Early Childhood Credential. Program Standards: Once the facility is in compliance with applicable child care requirements, no additional action is needed to meet one point in this component. In order to earn two or more points, you will need to complete the 3-month long self-study and self-assessment using the FCCERS-R rating scale tool. In order to earn three or more points, you must have the FCCERS completed by the North Carolina Rated License Assessment Project and score a minimum of 4 points. I referred you to the NCRLAP website, (www.ncrlap.org), for technical assistance and information regarding the 3-month self-study and self-assessment. Maintain documentation of completion of the three-month self-study on file for review and include dates when information is documented if you choose to complete the self-study. I am going to email you information regarding the three-month self-study. You can contact Wayne County’s Partnership for Children to request any on-site technical assistance and assistance with a self-study. We discussed the FCCERS is voluntary and if not completed during your initial Rated License assessment you can request it later. Quality Point: The various options for earning a quality point will be discussed during the next visit. COMPLIANCE HISTORY: It is very important that you maintain compliance with all applicable requirements at all times since you will be building a compliance history. In order to move to a star rated license at the end of the Temporary Time Period, you must have at least a 75% compliance history. Programs whose compliance history is below 75% could be issued an administrative action including denial of a license at the end of the Temporary License period. The violations documented today may affect your compliance history score. Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. I encourage you to visit the DCDEE website and click on the “What’s New” tab to stay abreast of all updates.
