A checklist was used to note the requirements I monitored today. The staff and training worksheets were provided during today’s visit. The visit was completed with D. Lucas, Administrator. You were notified of the purpose of the visit upon my arrival, and you were available for consultation at the conclusion of the visit. Currently this center operates with a Four Star-Rated license, issued March 28, 2024, earning seven (7) points in the Education Component, two (2) points in the Program Component and 1 Quality point based on meeting the educational option when the program has earned at least 4 points in education and the program has a combined turnover rate of 20 percent or less for the administrator, program coordinator, lead teacher, teacher, and group lead positions over the last 12 months. The last annual compliance visit was completed on March 11, 2025. The last sanitation inspection was completed on September 3, 2025, earning a “Superior” classification. The last fire inspection was completed on February 6, 2026, and the center was approved for daytime care only. Prior to today’s visit, the facility earned a Compliance History score of 96% over an 18 - month time- period. A copy of the compliance history was provided to you. Children were observed participating in group activities, free play in the indoor and outdoor environment, completing transitions, completing routines such as handwashing and toileting and eating lunch. Lunch consisted of peanut butter and jelly sandwich, banana, salad, and milk. All staff provided care in a nurturing manner. Staff child ratios were in compliance and adequate supervision was provided for all children. Infants were observed participating in tummy time, interacting with caregivers, and having routine care needs met. I used the Annual Compliance Monitoring Checklist for Child Care and the Child Care Center Item Number Listing (DCD 0357) as a basic monitoring tool to assess compliance with all applicable child care requirements. A computer-generated visit summary was reviewed with you, and a copy was provided to you. All violations were reviewed with you at the conclusion of the visit. The following violations were documented: Violation Number Comment Rule 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. One child did not have a health assessment on file before or within 30 days of admission. The child enrolled in March 2025 and the health assessment was dated for September 2025. GS110-91(1) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Staff member. J. Baker did not complete the required training within 90 days of employment. This corporation was researched in the North Carolina Secretary of State corporation database and found to have a Current/Active status. The contact information for the owner and facility was reviewed and contained correct information, as documented with the Division of Child Development and Early Education. You stated the mailing address, phone number and email address listed for your child care center are correct. Education Standards – There are six lead teachers including one teacher who also serves as the Group Leader for the school-age children and the program coordinator, which require education evaluations for this standard. It appears the center will meet 7 points in Education Standards. As administrator, A. Faulk appears to receive 7 points by having Level III Administrator with more than five years of administrative experience, and BSAC training. We discussed that the center must always maintain staff at the 7-point level, due to the points earned on your license in this component. Program Standards – With correction of the violations documented during today's visit, the center will be meeting minimum child care requirements (including current operational/administrative/personnel policy requirements). The center continues to meet enhanced space. The administrator has declined the Environmental Rating Scale assessment. Quality Point Options – The center meets these criteria by meeting the Educational option of: when the program has earned at least 4 points in education and the program has a combined turnover rate of 20 percent or less for the administrator, program coordinator, lead teacher, and group leader positions over the last twelve months. A total of 7 points in Education Standards, 2 points in Program Standards, and 1 Quality Point equals to 10 points. Your program will increase to a 4 Star-Rated license. RULE REMINDERS: CHILDREN’S RECORDS – Six (6) children’s records were monitored. Two children’s medical assessments were not received within thirty days of enrollment. One child’s immunization record was not received within thirty days of enrollment. You were encouraged to utilize the children’s file checklist to ensure all requirements are in compliance with the requirements. Due to the children receiving the health assessments and immunizations, this violations has been corrected. SAFE ENVIRONMENT – In space 2, a bottle of sanitizer and disinfectant were being stored on the counter, less than five feet from the floor. The teacher, K. Mabe stated that the children would not be able to reach the sanitizers and disinfectants; however, I reminded Ms. Mabe that these items must be maintained at least five feet from the floor to ensure the safety of children. This was corrected during the visit when the Ms. Mabe moved the sanitizer and disinfectant to a shelf five feet from the floor. TECHNICAL ASSISTANCE: PAINT/ASBESTOS TESTING: Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. We reviewed that you are in the prep year of Co-Hort 1. We reviewed that you are not required complete the reassessment until your assessment period; however, you stated that you are not interested in having the Environmental Rating Scale assessments completed; therefore, you would like to move forward with the reassessment. Education Standards – There are six lead teachers including one teacher who also serves as the Group Leader for the school-age children and the program coordinator, which require education evaluations for this standard. It appears the center will meet 7 points in Education Standards. As administrator, A. Faulk appears to receive 7 points by having Level III Administrator with more than five years of administrative experience, and BSAC training. We discussed that the center must always maintain staff at the 7-point level, due to the points earned on your license in this component. Program Standards – With correction of the violations documented during today's visit, the center will be meeting minimum child care requirements (including current operational/administrative/personnel policy requirements). The center continues to meet enhanced space. The administrator has declined the Environmental Rating Scale assessment. Quality Point Options – The center meets these criteria by meeting the Educational option of: when the program has earned at least 4 points in education and the program has a combined turnover rate of 20 percent or less for the administrator, program coordinator, lead teacher, and group leader positions over the last twelve months. A total of 7 points in Education Standards, 2 points in Program Standards, and 1 Quality Point equals to 10 points. Your program will increase to a 4 Star-Rated license. RULE REMINDERS: CHILDREN’S RECORDS – Six (6) children’s records were monitored. Two children’s medical assessments were not received within thirty days of enrollment. One child’s immunization record was not received within thirty days of enrollment. You were encouraged to utilize the children’s file checklist to ensure all requirements are in compliance with the requirements. Due to the children receiving the health assessments and immunizations, this violations has been corrected. SAFE ENVIRONMENT – In space 2, a bottle of sanitizer and disinfectant were being stored on the counter, less than five feet from the floor. The teacher, K. Mabe stated that the children would not be able to reach the sanitizers and disinfectants; however, I reminded Ms. Mabe that these items must be maintained at least five feet from the floor to ensure the safety of children. This was corrected during the visit when the Ms. Mabe moved the sanitizer and disinfectant to a shelf five feet from the floor. GOOD REPAIR – The baseboard was detached from the bottom of the wall in space 2. Ensure all walls and ceilings, including doors and windows, are kept clean and in good repair. TECHNICAL ASSISTANCE: PAINT/ASBESTOS TESTING: Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation. There are webinars that child care providers can watch to get instructions. Currently operating child care facilities are required to test for lead in paint and asbestos once unless the consultant notes a concern during a visit and refers the program to environmental health. Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. Today’s visit was completed with you, D. Lucas, Administrator. You were notified of the purpose of the visit upon arrival, and you were available for consultation at the conclusion of the visit. The last sanitation inspection was completed September 8, 2023, with a “Superior” classification. The last fire inspection was completed February 24, 2023, and the program was approved for day- time care only. There was one new staff that has been hired since the last Annual compliance visit dated March 15, 2023. The staff/training worksheet for this new staff was completed and the staff’s personnel file and medical file was reviewed today. Thirty (30) children were observed in care ranging in age from infant to four years of age. Children were engaged in free choice play in various activity areas, participating in outdoor play, completing toileting and hand washing routines, and interacting with caregivers. Staff interacted with children in a positive and nurturing manner. Activities provided were developmentally age appropriate for children in care. The center is owned by AVP Enterprises, Inc. This corporation was researched in the North Carolina Secretary of State corporation database and found to have a Current/Active status. The contact information for the owner and facility was reviewed and contained correct information, as documented with the Division of Child Development and Early Education. Prior to today’s visit, the facility has earned a Compliance History score of 94% over an 18 - month time- period. Due to technical issues, a one-page visit summary was left with you; however, I reviewed all violations with you at the conclusion of the visit. The computer-generated visit summary will be emailed to you. I used the Child Care Center Item Number Listing (DCD 0357) as a basic monitoring tool to assess compliance with all applicable child care requirements. The following violations were documented. Ensure all parental authorizations are renewed prior to the expiration of the previous authorization.
